Çalışma Süleyman Demirel Üniversitesi Tip Fakültesi Anesteziyoloji ve Reanimasyon Anabilim Dalında majör alt ekstremite operasyonu planlanan 60 ASA I-II olgu üzerinde yapıldı. Çalışmada farklı anestezi türlerinin stres hormonları, sitokinler ve katekolaminler üzerine etkisinin araştırılması planlandı. Bu amaçla olgular randomize olarak üç ayrı gruba ayrılarak 1. gruba epidural anestezi (Grup EA), 2. gruba epidural anesteziye eşlik eden yüzeyel genel anestezi (Grup EA+GA), 3. gruba ise klasik genel anestezi (Grup GA) uygulandı. Cerrahi travmaya bağlı stres yanıtı değerlendirmek amacıyla ameliyat salonuna gelişte (preoperatif), cerrahi insizyondan yarim saat sonra (peroperatif), postoperatif 12. ve 24. saate CRP, fibrinojen, T3, TSH, GH, kortizol, IL-1, IL-6 ve norepinefrin düzeyleri bakıldı, kontrol değerleriyle ölçüm yapılan değerler karsılaştırıldı. Ayrıca hemodinamik stabiliteyi değerlendirmek amacıyla ameliyat boyunca SAB, DAB, OAB, KAH ve SpO2 düzeylerine bakıldı. Olgularda baktığımız bu biyokimyasal parametrelerin Grup EA+GA da peroperatif ve postoperatif dönemde kontrol değerleriyle karşılaştırımında Grup EA ve Grup GA ya göre daha stabil seyrettiği, hemodinamik parametrelerin ise her 3 grupta stabil seyrettiği gözlendi. Araştırmamız sonuçlarına göre; alt ekstremite majör ameliyatlarında cerrahi travmaya bağlı stres yanıtı en aza indirgemek açısından araştırdığımız üç ayrı anestezi yöntemi arasında en idealinin epidural anestezi ile kombine edilmiş yüzeyel genel anestezi olduğunu düşünüyoruz.
This study was performed in 60 major lower extremity operations in Süleyman Demirel University Medical Faculty Anesthesiology and Reanimation Department. In this study the investigation of different anesthesia techniques on stress hormones, cytokines and catecholamines was planned. For this purpose patients were randomly divided into three groups, and epidural anesthesia was planned for the first group (Group EA), epidural anesthesia combined with light general anesthesia was planned for the second group (Group EA+GA) and classical general anesthesia was planned for the third group (Group GA). For evaluating the stress response to surgical trauma; C-reactive protein (CRP), fibrinogen, triiyodotronin (T3), tiroid stimulating hormone (TSH), growth hormone (GH), cortisol, interleukin-1, interleukin-6 and norepinephrine values were investigated at the periods of before operation (preoperative), half an hour after surgical incision (peroperative), 12 hours after operation (postoperative 12th hour), and 24 hours after operation (postoperative 24th hour); the values which are measured in these intervals and the control values were compared. Also systolic arterial pressure (SAP), diastolic arterial pressure (DAP), mean arterial pressure (MAP), heart rate (HR) and periferical oxygen saturation (SpO2) were measured intraoperatively for evaluating hemodynamical stability. It is observed that the biochemical parameters of Group EA+GA followed a stabile route than Group EA+GA followed a stabile route than Group EA and Group GA in peroperative and postoperative periods compared to control values and also it was observed that the hemodynamical parameters were stabile in all groups. We think that for minimizing the stress response related to surgical trauma, among the techniques which we had investigated the ideal one is epidural anesthesia combined with light general anesthesia.
