Diz eklem yüzeylerini bozan primer ve sekonder osteoartrit, romatoid artrit gibi patolojilerde, uygulanan konsevatif ve cerrahi tedaviler sonucunda semptomların arttığı hastalarda, ağrısız, hareketli ve stabil bir eklem sağlamak için etkin tedavi diz protezidir. Son 30 yıldır protez tasarımlarında ve cerrahi tekniklerde kaydedilen gelişmelerle diz artroplastisinde başarılı sonuçlar elde edilmiştir ve bu gelişmeler neticesinde farklı protez tasarımlan ortaya çıkmıştır. Hareketli ve sabit insertli diz protezlerinin her iki tipiyle de başarılı klinik sonuçlar elde edilebilmektedir. Bununla birlikte hareketli insertler, sabit polietilen İnsertli protezlerin bir kısmında uzun dönemde karşılaşılabilen aşınma, osteolizis ve komponent gevşemesi sorunları azaltmak üzere tasarlanmıştır. Biz yaptığımız çalışmada diz protezi uyguladığımız hastalarda sabit ve hareketli polietilen insertleri olan diz protezlerinin erken dönem klinik ve radyolojik sonuçlarını değerlendirdik. Çalışmamızda 53 hasta 54 diz değerlendirildi. 34 olguda hareketli insertli, 20 olguda sabit insertli diz protezi kullandık. Kullandığımız cerrahi teknikler her iki grupta da temel olarak aynıydı. Hastalarımızın preoperatif ve erken postoperatif değerlendirmelerini Diz Cemiyeti diz ve fonksiyon skorlama sistemlerini kullanarak yaptık. Sonuç olarak her iki diz protezinin erken dönem klinik sonuçlan arasında istatistiksel olarak anlamlı bir fark olmasa da hareketli insertli diz protezlerinin klinik sonuçlarının daha iyi olduğu kanısına ulaştık.
Knee prosthesis is an effective treatment method to obtain a painless, mobile and stable joint in some diseases such as secondary osteoarthritis, romatoid arthritis which degenerate the knee joint surfaces and in case of increased symptoms after conservative or surgical treatments. In last 30 years, successful outcomes have been established in knee artroplasthy in parallel to the improvements seen in prosthesis design and surgical technique areas and various prosthesis designs have been developed as a result of this improvement. There are successful clinical outcomes in both knee prosthesis types with mobile and fixed bearing. Beside this, prosthesis with mobile insert has been designed and developed to overcome some problems such as abrasion, osteolysis and componential loosening which can be seen in some fixed polyethylene inserted prosthesis. In this study, we evaluated the short - term clinical and radiologic outcomes of fixed and mobile polyethylene bearing prosthesis in patients who have undergone knee prosthesis surgery. A total of 53 patients and 54 knee joints have been evaluated. We used knee prosthesis with mobile bearing in 34 cases and with fixed bearing in 20 cases. We used the same surgical method in both groups. We evaluated the patients before and after the surgery by using the Knee Society knee and function scoring systems. As a result, there was no statistically significant difference between the two groups, however, we concluded that clinical outcomes were better in mobile bearing knee prosthesis group.
