Şu ana kadar kabul edilen bir hipotez olmamasına rağmen, gatsroözefageal refü (GÖR).rinosinüzitin oluşmasında önemli bir rol oynamaktadır. Bu çalışmanın amacı, taklit edilmiş gastrik içeriğin (pepsin/hidroklorik asit) nazal ve paranazal sinüslere etkilerini araştırmaktır. Yirmi beyaz yenizelanda tavşanı gelişigüzel olarak kontrol (n=5) ve çalışma gruplarına (n=15) ayrılmıştır. Çalışma gruplarına taklit edilmiş gastrik içerik ( 0.01 N hidroklorik asitte 0.5 mg/ml pepsin) ve kontrol grubuna ise tamponize fosfat salin solüsyonu günde iki kez burun deliklerinden infüzyonla verilmiştir. Hayvanlar bir hafta sonra (çalışma grubu, grup I) iki hafta sonra (çalışma grubu, grup II ve kontrol grubu) ve dört hafta sonra (çalışma grubu, grup III) sakrifiye edildi. Sinüzit bulguları bilgisayarlı tomografi (BT) ve histopatolojik olarak skorlanarak değerlendirildi. Bilgisayarlı tomografide total sinüzit skorları, grup II ve grup III'de kontrol grubuna göre daha yüksek bulundu. Histopatolojik total sinüzit skorları ise tüm çalışma gruplarında kontrol grubuna göre anlamlı olarak yüksekti. Sinüzit buguları grup II ve grup III'de daha belirgindi. Bölgelere göre bakıldığında etmoid sinüs ve osteomeatal komplekslerin daha çok etkilendiği saptandı. Taklit edilmiş gastrik içerik burun ve paranazal sinüsleri üç mekanizmayla etkileyebilir: gastrik içerik mukozayı doğrudan hasarlandırabilir, sinüs ostiumunu tıkayabilir veya mukosilier aktiviteyi bozabilir. Bu çalışma GÖR ile rinosinüzit arasındaki ilişkiyi gösteren ilk radyolojik ve histopatolojik çalışmadır. Eğer GÖR'ün rinosinüzit oluşumundaki rolü gösterilebilirse , bu hastalarda tanı ve tedavideki yaklaşımlar da değişecektir. Anahtar kelimeler: Gastroözefageal reflü, rinosinüzit, pepsin.
Although no hypothesis is universally accepted, gastroesophageal reflux (GER) may play an important role in the occurrence of rhinosinusitis. This study was carried out to investigate the effects of repeated simulated gastric juice (pepsin/hydrochloric acid) exposure on the nose and paranasal sinuses of rabbits. Twenty New Zealand white rabbits were randomly assigned into the experimental group (n=15) and the control group (n=5). The experimental groups were infused simulated gastric juice (0.5 mg/ml pepsin in 0,01 N hydrochloric acid) and the control group was infused phosphate-buffered saline solution through the anterior nostril bilaterally two times a day. All animals were sacrifed after surviving for one week (the experimental group I), two weeks (the experimental group II and the control group) and four weeks (the experimental group III). Findings related to sinusitis were evaluated and scored regarding to the computed tomography (CT) and histopathological findings. The total CT staging score of sinusitis was higher in the experimental group II and III than those of the control group. The total histopathological scores of sinusitis were significantly higher in all experimental groups than the control. The findings of sinusitis were determined in the experimental group II and III. The primarly affected regions were ethmoid sinuses and osteomeatal complexes. Simulated gastric juice may influence the nose and paranasal sinuses via three mechanisms: gastric juice may damage the mucosa directly or may obstruct the ostium or may impair mucociliary function. This is the first histopathological and radiological study which demonstrate the relationship between GER and rhinosinusitis. If GER is shown to play a significant contributory role in rhinosinusitis, diagnostic and therapeutic approaches to these patients will be changed significantly.
