Bu çalışmanın amacı kontrastlı bifazik helikal BT ile kontrastsız ve MnDPDP (mangafodipir) kontrastlı MRG'nin karaciğer tümörlerini saptamadaki duyarlılıklarını karşılaştırmaktır. Histopatolojik olarak tanısı konmuş malign karaciğer tümörlü toplam 19 hasta araştırma kapsamına alındı. Olgular US, bifazik spiral BT kontrastsız ve kontrastlı MRG ile değerlendirildi. Boyutları 8 mm ile 9 cm arasında değişen toplam 63 lezyon saptandı. Lezyonların varlığı US takibi, retrospektif olarak eski filmlerinin değerlendirilmesi ya da histopatolojik değerlendirilmeleri ile doğrulandı. MRG incelemeleri spin-echo (SE) ile gradient eko (GRE) sekanslarda prekontrast, erken postkontrast 40.dakika ve geç postkontrast 24. saatte gerçekleştirildi. Ayrıca FSE sekansında T2A görüntüler de alındı. BT ve tüm MRG sekanslarındaki incelemelerin duyarlılıkları nonparametrik Wilcoxon testi kullanılarak karşılaştırıldı. Erken postkontrast GRE incelemede lezyonların 54/63'ü (% 85), erken postkontrast SE'da 48/63'ü (% 76), prekontrast GRE'da 46/63'ü (% 73), T2A FSE'da 45/63'ü (% 71), geç postkontrast GRE'da 44/63'ü (% 70), bifazik BT'de 42/63'ü (% 67), prekontrast SE'da 38/63'ü (% 60) ve geç postkontrast SE'da 37/63'ü (% 59) saptandı. Duyarlılığı en yüksek inceleme yöntemi GRE erken postkontrast MRG idi. Duyarlılığı en düşük yöntem SE prekontrast MRG idi. Diğer inceleme modalitelerinin duyarlılıkları birbirine yakın olup, aralarında istatistiksel anlamlı farklılık izlenmedi (p> 0.05). MnDPDP'nin İV kontrast madde olarak MRG'de SE ve GRE incelemelerinde kullanımı ile metastatik lezyonların kontrast/gürültü oranının artması sonucunda, GRE ve SE incelemelerde, bifazik BT ve prekontrast MR incelemelerine göre anlamlı oranda daha fazla lezyon saptandı (p<0.005). Kontrastsız MRG'nin duyarlılığı, MnDPDP'nin kullanımı ile postkontrast SE sekansındaki incelemelerde % 60'dan, % 76'ya; postkontrast GRE sekansındaki incelemelerde % 73'den, % 85'e yükseldi. Ayrıca MRG'de GRE incelemeler, SE incelemelere oranla daha hızlı sekanslar olduğundan, hareket artefaktları azaldı ve S/N oranları ile C/N oranları daha yüksek izlendi. Bunun sonucunda da GRE incelemelerde SE incelemelere göre anlamlı oranda daha fazla lezyon saptandı (p<0.05). MRG incelemelerinin diğer noninvaziv yöntemlerle korele değerlendirilmesinin karaciğerin lezyonlarının saptanmasında yeteri olacağını düşünmekteyiz. MR teknolojilerinin gelişiminin, yeni kontrast maddelerin klinik kullanımının yakın gelecekte MRG'nin duyarlılığını ve spesivitesini daha da arttıracağını böylece invaziv modalitelerin kullanımının azalacağını ummaktayız.
The aim of this study was to compare dual-phase helical computed tomography, unenhanced MRI and MnDPDP enhanced MRI the detection of the hepatic tumors. Nineteen histopatologically.confirmed patients with malignant focal hepatic lesions were examined. These cases underwent evaluation with US, dual-phase spiral CT, precontrast and postcontrast MRI. Sixty three lesions, ranging 0.8-9.1 cm in diameter were detected in all imaging procedures (ultrasound, computed tomography and MR imaging). These lessions were confirmed by follow up sonography, retrospective evaluation of CT and histopatologic examination. The MR examination protocol included SE and GRE T1 weighted images, which were acquired before, 40 minute after and 24 hours after admistiration of 0.5 micromol/kg MnDPDP; and fast T2 weighted SE images, which were obtained solely before contrast injection. All MRI sequences and CT were compared by using nonparametric Wilcoxon test. Early postcontrast GRE MR imaging depicted 54/63 (85%); early postcontrast SE MR imaging, 48/63 (76%); precontrast GRE MR imaging, 46/63 (73%); T2W FSE MR imaging, 45/63 (74%); late postcontrast GRE MR imaging, 44/63 (70%); dual phase spiral CT, 42/63 (67%); precontrast SE MR imaging, 38/63 (60%). The best results were obtained in early postcontrast GRE T1 weighted MR imaging and the worst results were obtained T1 weighted SE precontrast MR imaging 38/63 (60%) for detection of liver lesions. The difference in sensitivity between other MRI modalities and CT was not statistically significant (p>0.05). MnDPDP enhanced GRE and SE MR imaging was superior to dual phase spiral CT in detection of focal hepatic lesions. By using MnDPDP at SE and GRE MR imaging, the liver S/N ratio and the contrast/noise ratio of metastatic lesions was increased. So postcontrast MR imaging detected more lessions (p<0.05). The MnDPDP enhanced MRI showed significant improvement in the detection of hepatic malignant tumors with respect to unenhanced scans. On the SE MR imaging sensitivity increased from %60 on the precontrast axemination to 76% on the postcontrast examination (p<0.05); On the GRE postcontrast MR imaging sensitivity increased from 73% to 85% (p<0.05). The sensitivity of GRE and SE MR images for detection of liver lesions was compared in the precontrast, early postcontrast and late postcontrast phase.. The GRE images detect significantly more lessions than SE images (p<0.05); So we had to higher liver S/N ratio and higher C/N ratio in the GRE imaging. The evaluation of MnDPDP enhanced MR imaging with the other noninvasive modalities was enough for detection of liver metastases. We hope that technological development of the MR imaging and the clinical application of new contrast agents will cause more increase in the sensitivity and specificity of MRI imaging in near future; so the need for invasive modalities will decrease.
