Ocak 1999-Ekim 1999 tarihleri arasında Süleyman Demirel Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları polikliniğine altını ıslatma şikayeti ile başvuran toplam 125 olgudan primer enürezis noktürnalı 98 olgu çalışmaya alındı. Tüm olgular anamnez, fizik muayene ve laboratuar tetkikleri ile değerlendirildi. Olgular rast gele yöntemle üç gruba ayrılarak birinci gruba davranış tedavisi, ikinci gruba alarm yatağı ile şartlandırma tedavisi ve üçüncü gruba ise 20 mikrogram desmopressin nazal sprey tedavisi uygulandı. Tedavi üç ay sürdü. Olgular ikinci hafta, birinci ay, üçüncü ayda tedavi başarısı yönünden değerlendirilmek üzere; altıncı ay sonunda da relaps oranları için kontrol edildi. Hastaların tedavi öncesinde altını ıslatma sıklığı haftada üç gece ve üzerinde idi. Her üç grup arasında istatistiksel olarak fark yoktu. Her üç tedavi yöntemi ile tedavi sırasında ortalama kuru gece sayısında artma oldu. Fakat alarm yatağı ve desmopressin tedavisinin başarısı davranış tedavisinden yüksekti ve aradaki fark istatistiksel olarak anlamlıydı. Tam düzelme oranı üçüncü ay sonunda davranış tedavisinde %29.03, alarm yatağında %64.7Q ve desmopressin tedavisinde %66.66 olarak tespit edildi. Altı ay sonunda relaps oranı davranış tedavisi alan grupta %33.3, alarm yatağı grubunda %40.9 ve desmopressin grubunda ise %72.7 olarak bulundu. Bu bulgularla enürezis nokturna tedavisinde alarm yatağı ile şartlandırma tedavisinin başarı oranının yüksek ve relapsların düşük olması nedeniyle en etkin tedavi modeli olarak görülmektedir. Fakat biz basamaklı tedavi modelini öneriyoruz. Birinci basamak davranış tedavisi, ikinci basamak alarm yatağının tek başına veya davranış tedavisi ile kombinasyonu ve son basamakta da ilaç tedavisi olarak desmopressinin seçilebileceğini öneriyoruz.
The study had been performed between January 1999 and October 1999 in the pediatric polyclnics of Süleyman Demirel University. 98 out of 125 patients with primary enuresis were eligible for the research. AN of the cases were evaluated by their medical stories, physical examinations and laboratory findings. The cases were randomly separated into 3 different treatment groups: behavioral therapy, conditioning treatment with alarm-bed, and 20 microgram desmopressin nasal aerosol treatment. The patients were controlled in the second week, first month, and third month for evaluating the success of the treatment and also for the frequency of relapses at the end of the sixth month. The frequency of enuresis in all of the patients were more than 3 nights per week before the treatment. Dry nights increased in all of the three groups after treatment. The groups using alarm-bed and desmopressin treatments were more successful than behavior therapy and the difference was statistically significant. The percentage of the complete cure was 29.0%, 64.7%, and 66,6% respectively, as far as behavior, alarm-bed, and desmopressin treatment groups were considered after 3 months of treatment. The relapse rates for the same groups were 33.3%, 40.9%, 72.7% after 6 months in the same order. According to our findings, conditioning treatment with alarm-bed is more successful than the other therapies, as far as its high success rate and low relapses. However we suggest a treatment model which precedes step by step. The first step should be behavioral therapy. The second step is alarm-bed treatment, which may be added to the first step or accomplished by itself. In the last step, drug treatment should be considered, and desmopressin is the preferred one.
