Amaç: Nikotinin A1zheimer hastalığı gibi bazı nörodejeneratif rahatsızlıklarda teropatik etki ve santral sinir sisteminde nöroprotektif etki gösterebileceği bildirilmiştir. Bu çalışma, iki haftalık nikotin bandı tedavisinin Hafif Kognitif Bozukluğu (MCI) olan hastalarda bilişsel fonksiyonlara etkisini incelemek için düzenlendi. Materyal ve Metod: Çift kör, plasebo kontrollü bir çalışma düzenlendi. MMSE (Mini-Mental State Exarnination) skoruna göre (20-23) MCI tanısı konulan 24 hasta çalışmaya dahil edildi. Nikotin (n=12) ve plasebo (n=12) grubu olarak iki grup oluşturuldu. Nikotin grubuna 24 saat boyunca 7 mg/gün dozunda nikotin bandı verilirken diğer gruba plasebo bantlar verildi. Tedavi öncesinde ve sonrasında kognitif performansı değerlendirmek için hastalara psikiyatrik test bataryası uygulandı. Ayrıca lipid peroksidasyonu (MDA) ve antioksidan enzim aktivitelerini (SOD, GPx ve Cat) incelemek için hastalardan tedavi öncesi ve sonrası kan örnekleri alındı. Sonuçlar: Nikotin bandı uygulanan grupta MMSE (p=0,003) ve Blessed (p=0,002) Test skorlarında tedavi sonrasında öncesine göre istatistiki olarak anlamlı fark bulundu ancak plasebo grubunda anlamlı bir değişiklik gözlenmedi. Her iki grupta da tedavi öncesi değerler tedavi sonrasıyla kıyaslandığında MDA düzeylerinde ve antioksidan enzim aktivitelerinde anlamlı bir fark bulunmadı. Nikotin bandı tedavisinin MCI'li hastalarda kognitif fonksiyonlarda düzelme yapabileceği ve nikotin uygulamasının oksidatif strese herhangi bir etki etmediği gösterilmiştir. Bu sonuçlar, nikotin bandı tedavisinin Alzheimer hastalarında, özellikle MCI'lilerde yeni bir tedavi stratejisi olabileceğini düşündürmüştür.
Obcettive: Nicotine has been reported to be therapeutic in some patients with certain neurodegenerative diseases such as Alzheimer's disease, and to have neuroprotective effects in the central nervous system. The present study was undertaken to evaluate the clinical and neuropsychological effects of transdermal nicotine in patients with mild cognitive impairment (MCI) over a 2-week period. Material and methods: The double blind, placebo controlled study designed. 24 subjects with a diagnosis of MCI according to their Mini-Mental State Examination (MMSE) score (20-23) were recruited. They were divided into two groups; nicotine (n=12) and placebo (n=12) receiving group. Patients in the nicotine group wore the nicotine patch for 24 h a day at 7mg/day dose for two weeks while the other group wore placebo patch. Before and after treatment to assess cognitive performance, subjects were given a computerized test battery. Blood samples were also taken before and after treatment to evaluate lipid peroxidation (MDA) and antioxidant enzymes (SOD, GPx, Cat). Results: Nicotine significantly increased MMSE scores (p=0.003) and decreased Blessed Test scores (p=0.002) in nicotine patch treatment group but there were no significant change in plasebo group. In both groups there were no significant change in MDA levels and antioxidant enzyme activities, before and after treatment. Nicotine skin patch treatment was found to significantly improve cognitive function in patients with MCI. Nicotine administration neither cause oxidative stress and nor show antioxidant properties. This findings suggest that nicotine administered through a dermal patch could be a new treatment strategy In Alzheimer's disease especially patients with MCI.
