Kalça ve diz artroplastisi uygulanan hastalarda sement uygulanmasının derin ven trombozu üzerine olan etkisi hakkındaki tartışmalar halen devam etmektedir. Potansiyel trombojenik etkiye sahip olması dolayısıyla sement uygulamasının derin ven trombozu görülme sıklığını ve trombüs boyutunu artırdığını öne süren bazı çalışmalar olmasına rağmen, son zamanlarda sement uygulamasının bu olaylar üzerindeki etki mekanizmasına ilişkin herhangi bir çalışma yapılmamıştır. Bu çalışmada, kliniğimizde sementli ve sementsiz olarak uygulanan total diz, total kalça protezleri ve hemiartroplastilerin renkli doppler ultrason taraması ile araştırılması ve sonuçta sement kullanımının derin ven trombozu insidansi üzerinde etkisi olup olmadığı araştırılmıştır. Çalisma prospektif, randomize ve kontrollü olarak yapılmıştır. Çalışma grubundaki bütün hastalara düşük molekül ağırlıklı heparin profilaksisi uygulanmıştır. Biz yaptığımız çalışmada derin ven trombozu oranlarını kalça ve diz artroplastilerini sementli ve sementsiz olarak karsılaştırdığımızda sementli kalça protezi uygulanan 41 hastanın 7'sinde (% 17), sementsiz kalça protezi uygulanan 39 hastanın 4'sinde (% 10.2) derin ven trombozu saptadık. Diz protezlerinde ise; sementli total diz protezi uygulanan 17 hastanın 2'sinde (% 11.7), sementsiz total diz protezi uygulanan 27 hastanın 2 sinde (% 8.6) derin ven trombozu saptanmıştır. Çalışmamızın sonucunda sementli olarak uygulanan artroplastilerde DVT oranı daha yüksek bulunmuş olsa da sement uygulamasının DVT insidansı üzerine olan etkisi istatistiksel olarak anlamlı bulunmamıştır. (p<0.05).
Currently, there is a controversy about the effects of the cement procedure on deep vein thrombosis in patients who have undergone hip and knee arthroplasty operations. Although there are some studies suggesting that cement procedure increases the incidence of deep vein thrombosis and thrombosis size as it has potential thrombogenic effect, there are no recent study regarding the effect mechanism of cement underlying these events. In our study, we have evaluated the cemented and uncommented total knee and total hip prosthesis and hemiarthroplasty operations by using colored Doppler ultrasonography method and investigated whether cement procedure has an effect on deep vein thrombosis development. This study was conducted prospectively, randomized and controlled. All patients in the study group were given low molecular weight heparin (Dalteparin and Enoxoparin) prophylaxy treatment. When deep vein thrombosis incidence rates of the total hip and total knee arthroplasty groups were compared by means of cement usage, it has been found 7 of 41 patients (17 %) in cemented and 4 of 39 patients (10.2 %) in uncemented total hip prosthesis operation have deep vein thrombosis. Whereas in total knee prosthesis operations, 2 of 17 patients (11.7 %) in cemented and 2 of 23 patients (8.7 %) in uncemented group have had deep vein thrombosis. As a result, although there is a higher deep vein thrombosis incidence in cemented arthroplasty operations, we could not find any statistically significant difference by means of the effect of cement procedure on deep vein thrombosis incidence (p > 0.05).
