AMAÇ: Uzun süreli antiepileptik kullanan epilepsi hastalarında oluşabilecek serebellar atrofi bulgusuna yönelik beyincik/beyin hacim oranlarının Cavalieri prensibi ile hesaplanması ve kullanılan tekniğin planimetrik, volumetrik MRG gibi diğer zaman alıcı ve operatör konsolunu meşgul edici yöntemler yerine kullanılmasının uygunluğunun değerlendirilmesi. GEREÇ VE YÖNTEM: Çalışmaya klinik olarak epilepsi tanısı almış, uzun süreli fenitoin ve karbamazepin tedavisi alan hastalar dahil edildi. Görüntüleme 0.2 T açık MR da gerçekleştirildi. MR görüntülerinde öncelikli proçesin progresif atrofiye sebep olma riskinden dolayı serebrovasküler hastalığı olduğuna dair kanıtlar bulunan bireyler, kafa travmasına maruz kalan, alkol tüketimi, steroid kullanımı ve beyni etkileyebilecek herhangi bir önemli medikal veya psikiyatrik rahatsızlık geçmişleri olan hastalar çalışma dışında bırakıldı. Aranan özelliklere sahip 15 fenitoin, 15 karbamazepin tedavisi alan hasta ve aynı özelliklere sahip 15 kontrol olgusu çalışmaya alındı. Hacim ölçümleri ve hacim oranları Cavalieri prensibine dayanan noktalı alan ölçüm yöntemi kullanılarak gerçekleştirildi. Elde olunan T1 ağırlıklı sagital beyin kesitleri üzerine şeffaf bir asetat üzerine basılan noktalı alan ölçüm cetveli rastgele olarak atıldı. Her bir kesitte ilgili yapıların kesit yüzey alanı ile çakışan nokta sayısı hesaplandı. Kesit yüzeyine denk gelen noktaların sayımı sonrası ilgili formüller yardımı ile hata katsayıları, hacim değerleri ve oranları her hasta için hesaplandı. BULGULAR: Cavalieri prensibi ile nokta sayım tekniği kullanılarak fenitoin, karbamazepin kullanan epilepsi hastalarında ve normal grupta beyin, beyincik hacim değerleri ve beyincik/beyin hacim oranları ile hastaların gruplara göre hesaplanan ortalama beyincik ve toplam beyin hacimleri ile bunların birbirine oranları ve standart sapmaları hesaplandı. Tüm gruplar birbirleriyle karşılaştırıldığında istatiksel olarak anlamlı farklılık bulunmadı. Ancak fenitoin kullanan grupta 1 olguda hacim oranının düşük değerde olduğu görüldü. Bu hastanın beyincik kesitleri görsel inspeksiyon ile incelendiğinde görünümün serebellar atrofi ile uyumlu olduğu izlendi. Hesaplamalar her hasta için 25dk -1/2 saat arasında süre aldı. SONUÇ: Fenitoin kullanan 15, karbamazepin kullanan 15 hastanın ve 15 olgudan oluşan kontrol grubunun hesaplanan hacim değerleri ve hacim oranları arasında istatiksel olarak anlamlı farklılık bulunmamıştır. Kullanılan yöntem rutin çekimler sonrası elde edilen filmler üzerinde uygulanabilen, MR konsolunun ve operatörün meşgul edilmediği, günlük yoğun çalışma temposu içerisinde, kısa bir zaman diliminde sonuca ulaşabilme ve takip hacim değişikliği değerlerinin gerek organ morfometrik ölçümlerinin gerekse de tedaviye bağlı kitle boyut farklılıklarının objektif değerlerle belirlenmesi yeteneğine sahip bir hacim ölçüm tekniği olarak karşımıza çıkmaktadır. PURPOSE: To calculate the cerebral-cerebellar volume ratios for cerebellar atrophy which can be seen in epileptic patients that use long term antiepileptics by Cavalieri principle and to evaluate the method's usefulness by comparing with the other methods that occupies the operator like planimetric, volumetric MRI. MATERIALS and METHODS: Clinically diagnosed epileptic patients who recieved long term phenytoin and carbamazepine included to our study. Imaging was performed in 0.2 T open MR. Individuals with evidence of cerebrovasculer disease on MR imaging, in whom the underlying process could result in progressive atrophy, who had head injuries, alcohol consumption, steroid use, any significant medical or psychiatric history that can effect the brain were excluded from the study. Fifteen patients that use phenytoin, 15 patients that use carbamazepine and 15 control patients who meet the criteria included in the study. The measurements of the volume and volume ratios made by the Cavalieri principle by using the point-counting methods. The point counting grid, which has some point sets at distinct densities on a transparent sheet randomly positioned to the T1 weighted sagittal brain sections. In every section the number of points that collide with the section surface field counted. The coefficient of error, volume measurements and ratios calculated for each of the patients with the related formulas of the count of the points that suit with the section plane. RESULTS: By using the point-counting methods with Cavalieri principle cerebral-cerebellar volume measurements and cerebellar/cerebral volume ratios, mean cerebral and cerebellar brain volumes calculated according to the patient groups and their ratios and standard deviations in the epileptic and normal groups. There were no significant difference between the groups. But in phenytoin group one patient has low volume ratio. The cerebellum sections of this patient is examined with visual inspection and the aspect is well-adjusted with cerebellar atrophy. The measurement last for 25 min-1/2 hour for every patient. CONCLUSION: There were no significant difference in the estimated volume values and volume ratios between the three groups. The method which we use can be applied on the films that are get after the rutine draws, doesn't preoccupied the MR console and the operator with in the daily working rutine, results in a short time period, and determined the pursuing volume change values both organ morphometric measurements and also the difference of mass dimension due to the treatment with the objective values.
