Günümüzde en sık görülen romatizmal hastalıklardan biri olan Romatoid Artrit sistemik bir hastalık olmasına ragmen esas olarak sinovyal eklemleri tutar. Hastalık kısa sürede destrüktif hale gelip ciddi deformiteler ve özürlülük gelişebilir. Bu nedenle erkenden hastalık modifiye edici ajanlarla tedaviye başlanmalıdır. Çalışmalarda hastalık modifiyeedici ajanlar içinde etki-toksisite profili en iyi olanı metotreksat olarak bildirilmiştir. Ancak yine de bazı toksisiteleri nedeniyle etkili olmasına ragmen metotreksat kullanımına son verilebilmektedir ve bu yan etkiler nedeniyle metotreksat'i 1 yıl içinde kesme oranı çalışmalarda yaklaşık % 30 olarak bildirilmektedir. Yine son yıllarda metotreksat tedavisinin Romatoid Artrit'li hastalarda plazma homosistein düzeylerinde yükselme yaptığı ve bunun çeşitli vasküler hastalıklar için bağımsız bir risk faktörü olduğu, prematür vasküler bozukluklar nedeniyle morbidite ve mortalitede artış olduğu görüsü kabul görmektedir. Metotreksat tedavisini kesmeyi gerektirecek gastrointestinal, hematolojik, hepatik ve renal yan etkilerin, ayrıca metotreksat'ın yol açtığı hiperhomosisteinemi'nin folik asit verilmesiyle metotreksat'ın etkinliğinde herhangi bir azalma yapmaksızın önlenebileceği görüsü yaygın hale gelmiştir. Biz de folik asit desteğinin metotreksat'in tüm bu yan etkileri ve etkinliği üzerine etkilerini belirlemeyi amaçladık. Bu amaçla 1987 ARA kriterlerine uyan 31 Romatoid Artrit'li hasta çalışmaya alınarak 2 gruba ayrıldı. İlk gruba (n=14) yalnızca metotreksat, ikinci gruba (n=17) 1/1 oranında metotreksat+folik asit verilerek hastalar 6 ay takip edildi. İki grup arasında MTX'in sık görülebilen yan etkileri açısından fark yoktu. Bu tip yan etkiler az ve geçici idi. Hastaların hiçbirisinde ciddi toksik yan etkiler görülmedi. Bu hasta sayısının az ve takip süresinin kısa olmasına bağlandı. Hastaların tümünde bazal plazma homosistein düzeyleri relatif olarak yüksek bulundu ve bu yükseklik yalnızca metotreksat kullanan grupta artarken metotreksat yanında folik asit desteği alan grupta homosistein düzeyleri düştü. Her iki grupta da altı ay sonra klinik ve laboratuar olarak hastalık aktivitesinde gerileme oldu, yani ilave folik asit'in metotreksat'in etkinliğinde azalma yapmadığı saptandı. Bu elde edilen sonuçlara dayanılarak metotreksat'ın Romatoid Artrit tedavisinde etkili olduğu, yan etkilerinin sık olmadığı, ancak prematür vasküler hastalıklar için risk faktörü olduğu kabul edilen plazma homosistein düzeylerini yükselttiği, uygun dozda verilecek olan folik asit desteğinin homosistein düzeylerini düşürürken metotreksat'ın etkinliğinde azalma yapmadığı sonucuna varıldı. Rheumatoid Arthritis (RA) one of the most commonly seen rheumatic diseases today, although it is a systemic disease, involves mainly the synovial joints. The disease becomes destructive in a short period of time and serious deformities and disability may occur. For this reason treatment by using disease modifying antirheumatic drugs (DMARDs) should be started as soon as possible. Among DMARDs, Methotrexate (MTX) is reported as having the best effect-toxicity profile. Nevertheless, although it is effective, MTX can be stopped because of some toxicities and in some studies, the ratio of stopping MTX because of these side effects in 1 year is reported approximately as 30 %. Again, in the last few years , it is accepted that in patients with RA treatment with MTX causes an increase in plasma homocysteine levels and this is an independent risk factor for some vascular diseases and there is an increase in mortality and morbidity because of premature vascular disorders. The opinion is common that, by giving folic acid the gastrointestinal, he matologic, hepatic and renal side effects that cause to stop treatment with MTX and also the hyperhomosisteinemia that is caused by MTX can be prevented and the effectiveness of MTX is not decreased. We aimed to determine the effects of folic acid supplementation on all of these side effects on the effectiveness of MTX. For this purpose, 31 patients that fulfill the 1987 ARA criteria was taken into the study and divided into two groups. The patients were followed up by giving only MTX to the first group (n=14), and MTX+folic acid in 1/1 ratio to the second group (n=17). There was no difference between the 2 groups about the commonly seen side effects of MTX. These type of side effects were small and temporary. No serious toxic side effects was seen in any patients. This was related to the small number of patients and the short follow-up period. In all of the patients the basal plasma homocysteine levels were found relatively high and these levels were increased in the group that use only MTX, but homocysteine levels in the group that get folic acid supplementation were decreased. In booth of the groups 6 months later the clinical and laboratory activity of the disease was diminished, so it was determined that additional folic acid does not decrease the effectiveness of MTX. According to these results, it was concluded that MTX is effective in the treatment of RA, its side effects are not common, but it increases plasma homocysteine levels which is accepted as a risk factor for premature vascular diseases and folic acid supplementation in appropriate doses decrease homocysteine levels and does not diminish the effectiveness of MTX.Key words: Rheumatoid arthritis, methotrexate, side effects, hyperhomocysteinemia, folic acid.
