Non-obstrüktif azospermik infertil erkeklerde sperm elde edilmesi için çeşitli teknikler kullanılmaktadır.Teknikler arasinda farkliliklar olmakla beraber hemen hepsi invaziv girişimlerdir ve enflamasyon, infeksiyon, hematom gibi komplikasyonlar değişik oranlarda gözlenebilmektedir. Bu komplikasyonlar daha sonra ayni tekniklerin tekrar kullanımını zaman ve teknik seçimi açısından sınırlayabilmektedir. Infertil erkekler, mevcut sperm elde etme tekniklerinin gelişigüzel ve plansız kullanılması sonucu mevcut şanslarını da kaybetme tehlikesi ile karsı karsıya kalabilmektedirler. Bu yüzden mevcut sperm elde etme yöntemlerinin testis ve epididim üzerine olan histo-patolojik etkileri ile anti-sperm antikorları üzerine etkisinin belirlenmesi oldukça büyük önem arz etmektedir. Bu çalışmada her grupta 10 tane olmak üzere ( TESE, TESA, TRU-CUT IGNE BIOPSISI, KONTROL GRUPLARI) toplam 36 erişkin erkek rat üzerinde ilgili tekniklerin uygulanması kararlaştırıldı (kontrol grubu 6 rattan oluşturulmuştur). Ratlarda işlem öncesi ELISA yöntemi ile serum anti-sperm antikor seviyeleri ölçüldükten sonra intraperitoneal ketamin enjeksiyonu ile genel anestezi uygulandı (50 mg/kg) ve 10' una TESE, 10'una TESA, 10'una da Tru-cut iğne biopsisi yapıldı. Açik cerrahilerden sonra yara 5/0 chromic catgut ile kapatıldı. Her gruptan 3 rata işlemden 10 gün, 1 ay ve 4 rata 2 ay sonra önce serum anti-sperm antikor seviyesi için kan alındıktan sonra orsiektomi uygulanarak ratlar sakrifiye edildi (rat sayısı kontrol grubunda zaman sırasına göre 2, 2 ve 2 olarak oluşturuldu). Her üç işlemde de 10.günde belirgin enflamasyon dikkati çekmekteydi. TESE grubunda enflamasyon 60.günde de devam ederken TESA ve Tru-cut biopsi grubunda ise gerilemişti. Histopatolojik hasar Tru-cut grubunda en az gerçekleşirken TESE'de TESA' ya göre daha barizdi. Kalsifikasyon tüm gruplarda karakteristik olarak belirgindi. 10.günde obstrüktif bulgular TESA vakalarının tamamında, TESE vakalarının üçte ikisinde ve Tru-cut grubunda ise sadece 1 vakada belirlendi. 60.gün bulguları ise her üç grupta da benzerdi. İstatistiki olarak anlamsız da olsa TESE'nin immunolojik etki açısından ASA'yi daha fazla artırdığı ve Johnsen skorunu daha fazla bozduğu gözlenmiştir. Non-obstrüktif azospermik erkeklere uygulanan sperm elde etme yöntemleri erken dönemde tubulitis, hematom ve sık gözlenen obstüktif patolojilerle sonuçlanabilmektedir.Bunlar içinde perkütan testis biopsisi diğerlerine göre daha az invaziv gibi gözükmektedir. Ayrıca bu işlemler sonucu oluşabilen anti-sperm antikorlar da sperm hazırlanmasında degisik tekniklerin uygulanmasını gerekli kılmaktadır. Bu yüzden daha sonra tekrar uygulanacak testiküler prosedürler bu immuno-histo-patolojik değişiklikler gerileyip kaybolduktan sonra uygulanmalıdır. Oluşabilecek komplikasyonlar ile bunların rezolüsyon zamanları ve kalıcı hasarlar bu nedenle önem kazanmaktadır. ICSI'de kullanılmak üzere sperm elde etme oranındaki üstünlüğü nedeniyle TESE tercih edilmesi gerekirken, seçilmiş özellikle obstruktif vakalarda komplikasyon oranlarının azlığı nedeniyle Tru-cut biopsi önerilebilir.
Several surgical methods for obtaining testicular sperm in men with nonobstructive azoospermic infertility. Although there are differences between these tecniques almost all are invazive procedures and complications such as enflamation, infection, hematoma can be seen in various degrees. These complications may limit the reuse of these techniques for the time and the choose of technique. Infertile men may lose their chance in the future by the random and unplanned use of these techniques. Therefore it is very important to identify the histopathologic effect on testis and epididym and the effect on ASA of these techniques. The study included totally 36 adult male rats, consisting of ten for each group ( TESE, TESA, TRU-CUT needle biopsy, CONTROL )( Control group includes six rats ). Before the procedure serum ASA levels are measured with ELISA method in rats after that anaesthetized using an intraperitoneal injection with ketamine ( 50 mg/kg ). TESE, TESA and Tru-cut needle biopsy were performed on ten rats in each group. After open biopsy the tunica albuginea and scrotum were closed with 5/0 chromic catgut. Three rats from each group which underwent orchidectomy after collecting blood sample for serum ASA level were killed ten days and one month after the procedure, same procedure was repeated two months after the procedure for four rats. These procedure were performed on two rats in control group for the same time periods. Ten days after all three procedures showed significant inflamation, while the enflamation was continueing at in TESE group. It was regressed in TESA and Tru-cut biopsy groups. Histopathologic injury was minimal in Tru-cut biopsy group and it was obvious in TESE compared to TESA. Calsification was significantly characteristic in all groups. Obstructive histological findings were detected in all TESA cases, 2/3 of TESE cases and only one case in Tru-cut biopsy group at 10th days. The findings were similar in all three groups at 60th days. Even if it was statisticaly insignificant. We found that TESE increases more ASA from immunologic aspects and impairs the Johnsen score much. Sperm obtaining methods in nonobstructive azoospermic males may cause tubulitis, hematoma and frequenr obstructive findings in the early period. Percutan testis biopsy seems to be more minimal invasive according to the other procedures. Antisperm antibodies formed after these procedures requires different tecniques in preparing the sperm. Therefore the testicular procedures that will be performed later must be done after these immuno-histopathological changes regressed and disappeared. Therefore complications, resolution time and permanent injuries get more important. While TESE must be preferred for it's superiority in sperm obtaining for ICSI, Tru-cut biopsy can be suggest in selected especially obstructive cases because of having lower complication rates.
