İnsan mezenkimal kök hücrelerinin osteoblastlara veya adipositlere farklılaşabilmesi ve obez bireylerde kemik mineral dansitesinin yüksek olduğuna dair bilgiler kemik metabolizması ile yağ hücrelerinin doğrudan etkileştiğini düşündürmektedir. Biz de bu çalışmada Tip 2 diyabeti (DM) ile osteoporozu olan ve olmayan postmenopozal kadınlarda leptin, adiponektin ve insülin direnci ile kemik mineral dansitesi arasında ilişki olup olmadığını belirlemeyi amaçladık. Çalışmaya toplam 76 postmenopozal kadın hasta alındı. Osteoporoz için tedavi almakta olanlar, 65 yaş üstü olanlar, kemik metabolizmasını etkileyen ilaç alan ve hastalığı olanlar dışlandı. Hastaların L2-L4 vertebra ve femur kemik mineral dansitesi (KMD), yağ yüzdesi, yağsız vücut kitlesi ve yag kitlesi ölçüldü. Vertebra ve/veya femur T skoru -2'nin altında olanlar çalışmaya dahil edildi. Hastalar DM ve osteoporoz tanılarına göre dört gruba ayrıldı. Birinci grup Tip 2 DM ve osteoporoz tanısı olan 19 , ikinci grup Tip 2 DM tanısı olan ve osteoporoz tanısı olmayan 20, üçüncü grup Tip 2 DM tanısı olmayan ve osteoporozu olan 19, dördüncü grup Tip 2 DM tanısı olmayan ve osteoporoz tanısı olmayan 18 kadın hastadan oluşuyordu. Çalışmanın sonucunda osteoporozu olan ve olmayan kadınlarda leptin, adiponektin ve insülin direnci arasında bir farklılık saptanmadı. Aynı zamanda bu hastalarda leptin, adiponektin ve insülin direnci ile KMD arasında herhangi bir birliktelik saptanmadı. Tip 2 diyabetik osteoporozu olan ve olmayan kadınlarda leptin , adiponektin ve insülin direnci ile L2-L4 vertebra, femur boyun ve trokhanter KMD arasında herhangi bir birliktelik saptanmazken leptin ile femur wards üçgeni KMD arasında negatif birliktelik saptandı. Tip 2 diyabetiklerde leptin, adiponektin ve insülin direncini kemik metabolizması üzerine etkileri ile ilgili daha ileri çalışmalara ihtiyaç vardır.

Both the information on the differantion capacity of human mesenchymal stem cells into ostoeblasts or adipocytes and the increased bone mineral density in obese individuals suggest that bone metabolism and adipocytes directly influence each other. In this study, we aimed to determine whether there is an relation between leptin, adiponectin and insulin resistance and bone mineral density (BMD) in postmenapausal women with and without type 2 diabetes mellitus (DM) and osteoporosis. In total, 76 postmenapausal women were included into the study. Osteoporosis therapy, the age over 65, women who have bone metabolism disease or having drugs that interfere with bone metabolism were exclusion criteria. BMD of L2-L4 spine and femur, fat percentage, lean body mass and fat mass of the patients were measured. The patients whose spine and/or femur T score under -2 were included into the study. Patients were divided into 4 groups according to diagnosis of DM and osteoporosis. First group consisted of 19 women both with type 2 DM and osteoporosis; second group consisted of 20 women with type 2 DM but without osteoporosis; third group consisted of 19 with osteopororsis but without type 2 DM; fourth group consisted of 18 women with neither type 2 DM nor osteoporosis. At the result of the study, there was no difference between leptin, adiponectin and insulin resistance in women with or without osteoporosis. In addition, there was no any association between leptin, adiponectin and insulin resistance and BMD in these patients. In the patients with and without type 2 diabetic osteoporosis, there was no any association between leptin, adiponectin, insulin resistance and L2-L4 spine, femur neck and trochanter but there was an inverse association between leptin and BMD of femur wards triangle. In conclusion, further studies about the effects of leptin, adiponectin and insulin resistance on bone metabolism are required.
