Amaç: Asetaminofen, yaygın olarak kullanılan ağrı kesici ve ateş düşürücü bir ilaçtır. Asetaminofenin günlük tavsiye edilen doz sınırının üzerinde kullanılmasının, karaciğer üzerinde akut yetmezliğe kadar varan çeşitli olumsuz etkileri vardır. Yapılan bazı çalışmalarda, karaciğer parankiminde üretilen bazı metabolitlerin de bu sürece katkı sağladığı bildirilmiştir. Nitrik oksit (NO), yangısal medyatörlere karşı sentezlenen oldukça reaktif bir maddedir. Bu çalışmada, değişik dozlarda uygulanan asetaminofenin karaciğer üzerindeki muhtemel yapısal etkilerinin iNOS enzimi, NO ve diğer biyokimyasal parametrelerle olan bağlantısını incelemeyi amaçladık. Gereç ve Yöntem: Bu çalışmada, toplam 70 adet Wistar albino erkek sıçan kullanıldı. Hayvanlar, periton içi yoldan enjekte edilen asetaminofen dozuna göre yedi eşit gruba ayrıldı: Grup-I (Kontrol, n=10), Grup-II (5 mg/kg, n=10), Grup-III (10 mg/kg, n=10), Grup-IV (20 mg/kg, n=10), Grup-V (100 mg/kg, n=10), Grup-VI (200 mg/kg, n=10) ve Grup VII (500 mg/kg, n=10). Periton içi enjeksiyondan 24 saat sonra, hayvanlar anestezi altında açıldı. Karaciğer doku ve kan örnekleri alındı. Alınan doku örnekleri yapı ve lipit peroksidasyonu yönünden, kan örnekleriyse serum NO, süperoksit dismutaz (SOD) ve glutatyon peroksidaz (GSH-Px) seviyeleri bakımından incelendi. Bulgular: Grup-II, Grup-III ve Grup-IV'ten elde edilen veriler kontrol grubuyla karşılaştırıldığında, arada yapısal veya biyokimyasal yönlerden herhangi bir farklılığın olmadığı görüldü (p > 0,05). Diğer yandan, Grup-V, Grup-VI ve Grup-VII ile kontrol grubu arasında, karaciğerdeki yapısal değişiklikler, iNOS enzimine ait reseptör dağılımı ve serum NO seviyeleri yönünden anlamlı bir farklılık vardı (p < 0,05). Sonuç: Elde edilen bulgular, NO'nun asetaminofenin karaciğer üzerindeki etkilerine paralel olarak artmasının lipit peroksidasyonunu engellemesinin yanı sıra, farklı mekanizmalar üzerinden doku hasarlanmasında da rol oynayabileceğine işaret etmektedir. Objective: Acetaminophen is a widely used analgesic and antipyretic drug. Excessive administration of acetaminophen above the limits of daily recommended dose range have some negative effects on liver that may even progress to acute failure. In some studies, it has been reported that some metabolites produced in liver parenchyme contribute to this process. Nitric oxide (NO) is a highly reactive substance which is synthesized in response to inflammatory mediators. In this study, we aimed to investigate the relationship between the possible structural effects of acetamiophen at different doses on liver with iNOS enzyme, NO and other biochemical parameters. Materials and Methods: A total of 70 male Wistar albino rats were included in this study. Animals were equally divided to seven groups according to the injected acetaminophen dose as follows: Group-I (Controls, n=10), Group-II (5 mg/kg, n=10), Group-III (10 mg/kg, n=10), Group-IV (20 mg/kg, n=10), Group-V (100 mg/kg, n=10), Grup-VI (200 mg/kg, n=10) ve Grup VII (500 mg/kg, n=10).After 24 hours from intraperitoneal injection, animals were sacrifized under anesthesia. Liver tissue and blood samples were collected. Tissues have undergone analysis with structure and lipid peroxidation aspects, whereas blood samples were analyzed by means of serum NO, superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px). Results: When Group-II, III and IV were compared with the controls, there was no difference by means of structural and biochemical aspects (p > 0,05). Whereas, there was a significant difference between Group-V, VI, VII and the controls by means of hepatic structural changes, iNOS receptor distribution and serum NO levels (p > 0,05). Conclusion: Current evidence suggest that increase in NO in parallel to the effects of acetaminophen on liver may play a role in tissue damage via different mechanisms, beside its preventive effect on lipid peroxidation.
