Radyolojide sık olarak kullanılan kontrast maddelerin (KM), istenmeyen en önemli etkilerinden birisi de nefrotoksisitedir. Kontrast madde kaynaklı nefrotoksisitede (KMN) iskemi, oksidatif stres, osmolaliteye bağlı sitotoksik etkiler gibi mekanizmalar suçlansa da, altta yatan neden henüz net olarak aydınlatılamamıştır. KMN etyopatogenezinde oksidatif stresin ve antioksidan enzimatik mekanizmaların rolü, histopatolojik değişiklikler, oluşan nefrotoksisitede antioksidan özelliği bilinen bir ajan olan kafeik asit fenetil ester (CAPE)'in olası koruyucu etkisi, deneysel olarak ortaya konmaya çalışıldı. Wistar albino erkek sıçanlar 10'arlı 3 gruba ayrıldı; kontrol grubuna (I) i.p. yoldan salin, KM grubuna (II) i.p. yoldan tek doz 10 ml/kg KM ve CAPE grubuna (III) tek doz 10 ml/kg/gün KM, 10umol/kg/gün dozunda ip yoldan iki gün süreyle CAPE verildi. Antioksidan enzimlerden SOD ve CAT aktivitesi ve lipid peroksidasyonunun göstergesi olan MDA düzeyi ölçüldü, böbrek dokusundaki histopatolojik değişiklikler incelendi. Araştırmamızda; yalnızca KM verdiğimiz sıçanların böbrek dokusunda MDA düzeyleri yüksek iken, KM+CAPE verilen grupta MDA seviyesi düşük bulundu. SOD aktivitesinde KM grubunda, kontrol grubuna göre belirgin artış izlendi. KM+CAPE verilen grupta SOD aktivitesinin, KM grubuna göre düşük olduğu tespit edildi. CAT enzim aktivitesi KM ile birlikte CAPE verilen grupta kontrol grubuna göre yüksek, KM grubuna göre düşük olarak izlendi. Histopatolojik incelemelerde KM grubundaki hasar skorlarının diğer gruplara göre yüksek olduğu gözlenirken, KM ile birlikte verilen CAPE'nin tubulointerstisyel hasarı anlamlı düzeyde engellediği tespit edildi. Sonuç olarak, biyokimyasal ve histolojik veriler ışığında, kontrast maddeye bağlı nefrotoksisitede oksidatif stresin rol oynadığını ve CAPE'nin ortaya çıkan nefrotoksisitede koruyucu role sahip olduğunu gösterdik.

One of the undesired side effects of radiologic contrast media (CM) is nephrotoxicity. Although the underlying mechanisms of contrast-induced nephrotoxicity (CIN) is unclear, some mechanisms are suspected such as ischemic injury, oxidative stres or direct cytotoxicity. The aims of this experimental study were to research the role of antioxidant mechanisms and oxidative stress in CIN and to investigate the possible protective effect of caffeic acid phenethyl ester (CAPE), an antioxidant agent, against CIN. Rats were divided into three groups: Control, CM and CM+CAPE. Saline solution was injected intraperitoneally (i.p.) to the control group rats. CM was administered i.p. with 10 mlkg-1 for single dose to the CM group and CAPE was administered with a dose of 10 umolkg'1 for two days and CM was injected single dose i.p to the CM+CAPE group. The activities of superoxide dismutase (SOD), catalase (CAT) and levels of malondialdehyde (MDA) and histopathologic alterations were studied in the renal tissue of the rats. The level of MDA increased significantly in the CM group compared with the CM+CAPE group. The SOD activity was significantly increased in CM group compared with the control group. SOD activity was found to be lower in the group which had CM+CAPE than CM group. In CM+CAPE group CAT enzyme activity was higher than the control group but lower than the CM group. The damage scores in CM group was found to be higher than the other groups, histhopathologically. CAPE, which was given with CM, caused a marked reduction in the extent of tubulointerstitial damage. In conclusion, these results showed that oxidative stres plays an important role in CIN and CAPE has a protective effect against CIN on the basis of the biochemical and pathological findings.
