Yüzeyel venöz yetmezlik ve varislerin tedavisinde uzun süredir cerrahi yöntemler kullanılmaktadır. Bu yöntemlerin genel anestezi, hastanede kalış süresinin daha fazla olması, artmış komplikasyonlar gibi dezavantajları vardır. Bu dezavantajları ortadan kaldırabilmek için yeni tedavi yöntemlerine ihtiyaç duyulmuştur. Yüzeyel venöz yetmezlik tedavisinde son yıllarda Endovenöz Lazer Ablasyon (EVLA) ve köpük skleroterapi yöntemleri cerrahiye alternatif en güncel tedavi yöntemleridir. Bu çalışmada, alt ekstremite yüzeyel venöz yetmezlik ve buna bağlı gelişen varislerin tedavisinde EVLA ve köpük skleroterapi yöntemlerinin etkinliği araştırıldı. Semptomatik safen ven yetmezliği tespit edilen 55 hasta çalışmaya dahil edildi. 60 alt ekstremitede 73 EVLA ve eş zamanlı köpük skleroterapi uygulandı. İşlem öncesinde venöz hastalığın şiddetini belirlemek amacıyla Venöz Klinik Şiddet Skorlaması (VCSS) ve Vizüel Analog Skala (VAS) kullanıldı. İşlem sonrası hastalar 6 ay süreyle klinik olarak ve Doppler US ile takip edildi. VCSS ve VAS skorları takibin 6. ayında tekrarlandı. Takibin 6. ayında safen venlerde %98.64 (72/73) oranında tam oklüzyon, %1.36 oranında rekanalizasyon saptandı. Perforan venlerde tam oklüzyon oranı %75 (18/24), rekanalizasyon oranı ise %25 (6/24) idi. Majör komplikasyon gözlenmedi. VCSS ve VAS skorları işlemden önce ve 6 ay sonra karşılaştırıldı ve skorlarda anlamlı düşme izlendi. İzole safen ven yetmezliği olan (36/60) ve safen ile birlikte perforan ven reflüsü olan (24/60) hasta grupları karşılaştırıldı. VAS skorları izole safen ven yetmezliği olan grupta daha belirgin olarak düşmüştü. Bu düşüş anlamlıydı. VCSS da bu grupta diğer grupla karşılaştırıldığında daha fazla düşüş olmasına rağmen bu düşüş istatistiksel olarak anlamlı bulunmadı. EVLA ve eş zamanlı köpük skleroterapi, iyi kozmetik ve klinik sonuçlarıyla, düşük komplikasyon oranlarıyla etkin, güvenilir ve minimal invaziv bir tedavi seçeneğidir.

Surgical procedures have been used in the treatment of superficial venous insufficiency and varices for a long time. These procedures have some disadvantages such as general anesthesia, longer hospital stay, more complications. New treatment methods are needed to overcome these disadvantages. EVLA and foam sclerotheraphy are most current treatment methods alternative to surgery in the treatment of superficial venous insufficiency. Efficiency of endovenous laser ablation and foam sclerotheraphy methods in the treatment of lower extremity superficial venous insufficiency and varices was studied. Fifty-five patients with symptomatic sapheneous vein insufficiency were included in the study. Seventy-three EVLA and concomitant foam sclerotheraphy were performed for 60 lower extremities. To determine the severity of the venous disease Venous Clinical Severity Score (VCSS) and Visual Analogue Scale (VAS) were administered before the procedures. Patients were followed clinically and with Doppler ultrasonography for 6 months after the procedures. VCSS and VAS scores are repeated at 6th month of the follow-up. At the 6th month of the follow-up for the saphenous veins; the total occlusion rate was 98.64% (72/73), recanalization rate was 1.36%. For the perforating veins , total occlusion rate was 75% (18/24), recanalization rate was 25%. No major complications were detected. VCSS and VAS scores before and 6 months after the procedure were compared and significant decrease was observed. The patients who have isolated sapheneous vein insufficiency (36/60) and those who have saphenous and perforating vein reflux (24/60) were compared. VAS scores were more prominently decreased after the treatment in the isolated sapheneous vein insufficiency group. This decrease was significant. VCSS were also decreased more prominently in this group when compared to the other group but the decrease is not statistically significant. EVLA and concomitant foam sclerotheraphy is an effective, safety, minimally invasive treatment option with good cosmetic and clinical results and lesser complication rates.
