MMP-2 ve MMP-9, matriks metalloproteinaz ailesinden olup, tümör gelişimi, invazyonu ve metastazında önemli rol oynarlar. Doku inhibitör metalloproteinaz (TIMP)-1, matriks metalloproteinazların çinko bağlayan aktif bölgesine bağlanır ve tüm MMP.ları inhibe eder. Çalışmamızda ileri evre akciğer kanserli olgularda, tedavi öncesi serum MMP-2,-9, TIMP-1 düzeylerinin ve biyopsi örneklerinde MMP-2, -9 ve TIMP-1 ekspresyonlarının çeşitli klinikopatolojik faktörlerle ilişkisi ve sağ kalıma etkilerinin araştırılması amaçlanmıştır. İleri evre akciğer kanser (28 KHDAK ve 8 KHAK) tanısı almış 36 hastanın biopsi materyalleri ve serumları ile 16 sağlıklı kontrol grubunun serum örnekleri çalışmaya alındı. Dolaşımdaki antijenler ELISA yöntemi ve tümör dokularındaki protein ekspresyonları ise streptovidin biotin immünohistokimyasal boyama yöntemi ile analiz edildi. Hastaların klinikopatolojik faktörleri değerlendirildi. Akciğer kanserli hastaların serum MMP-9 düzeylerindeki artış sağlıklı kontrol grubu ile karşılaştırıldığında istatistiksel olarak anlamlı idi (p= 0,025). Serum MMP-9 düzeyleri açısından epidermoid tip akciğer kanserleri ile adenokanserler arasında istatistiksel olarak anlamlı farklılık vardı (p= 0,013). Serum MMP-9 düzeylerindeki artış, serum albümin (r= -0,317 p= 0,05) ve VKİ (r= -0,394 p= 0,017) ile korele idi. Akciğer kanserli olguların 22 (%69).si MMP-2, 35 (%97) .i MMP-9 ve 21(%58).i TIMP-1 ile immün pozitif olarak boyandı. Doku MMP-2 ekspresyonu ile serum LDH (r= 0,37, p= 0,02) ve albümin düzeyleri (r= -0,38, p= 0,02) arasında ve doku MMP-9 ekspresyonu ile serum LDH (r= 0,45, p= 0,005) ve yaş (r= 0,43, p= 0,009) arasında istatistiksel olarak anlamlı bir ilişki vardı. Cox regression analizinde, tedavi öncesi serum MMP-2, doku MMP-9 ve TIMP-1 ekspresyonu ileri evre akciğer kanserli hastalarda sağ kalımı etkileyen bağımsız prognostik faktörlerdi. Tedavi öncesi serum MMP-2 düzeyi, doku MMP-9 ve TIMP-1 ekspresyonu ileri evre akciğer kanserli hastalarda sağ kalım için prognostik önemi önemi olan faktörlerdir. Serum MMP-9 düzeyi ve doku MMP-2 ekspresyonu ileri evre akciğer kanserli hastalarda görülen malnütrisyonun patofizyolojisinde önemli bir rol oynayabilir.

Matrix metalloproteinase (MMPs) -2, -9 are the members of the MMP family, and it has been reported that they had important roles in tumorigenesis, invasion and metastasis. Tissue inhibitors of metalloproteinases (TIMP-1) bind to active zinc-binding site of the MMPs and therefore inhibit the activation of all MMPs. We performed a prospective study to investigate serum levels of MMP-9,-2, TIMP-1 and tissue expressions of MMP-2, -9 and TIMP-1; and their relations to clinicopathologic factors and prognosis in patients with advanced lung cancer. Thirthy-six newly diagnosed advanced lung cancer patients. (28 non-small cell lung cancer and 8 small-cell lung cancer) biopsy materials and serum samples were enrolled in this study also including 16 serum samples of healthy control subjects. Circulating antigens were measured by ELISA assay and the protein expression in the tumors was analyzed by streptovidin-biotin immünohistochemical staining using spesific monoclonal antibodies. Clinicopathological factors of the patients were reviewed. The serum concentration of MMP 9 in lung cancer patients was significantly elevated compared to that of healthy control subjects (p= 0,025). This elevation correlated with body mass index (r= -0,394, p= 0,017) and serum albumin (r= -0,317, p=0,05). There were statistically significant differences between the mean of MMP-9 concentration of the squamous cell carcinoma and that of the adenocarcinoma (p= 0,013). The positive MMP-2, -9 and TIMP-1 staining in tumour cells were observed in 22 (69%), 35 ( 97%) and 21 ( 58%) cases respectively. The MMP-2 expression was significantly correlated with serum LDH (r= 0,37, p= 0,02) and albumin levels (r= -0,38, p= 0,02), in addition to that, the MMP-9 expression was significantly correlated with serum LDH level (r= 0,45, p= 0,005) and age (r= 0,43, p= 0,009). Pretreatment serum level of MMP-2, and tissue expressions of MMP-9 and TIMP-1 were independent prognostic factors in patients with advanced lung cancer in a multivariate Cox regression analysis. We concluded that pretreatment serum level of MMP-2, and tissue expressions of MMP-9 and TIMP-1 had prognostic values in advanced lung cancer. MMP-9 level of serum and tissue MMP-2 expression may play roles in the pathophysiology of malnutrition in advanced lung cancer patients.
