Kalça ve diz protez enfeksiyonlarının erken ve doğru tespit edilmesi tedavi kolaylığı ve tedavi başarısı açısından oldukça önemlidir. Bu amaçla kullanılan yöntemin sensivite ve spesifitesinin yüksek olması beklenir. Ancak anamnez, fizik muayene, biyokimyasal, mikrobiyolojik ve radyolojik tetkiklerin özgüllükleri düşüktür. Bu nedenle radyonüklid görüntüleme yöntemleri şüpheli eklem replasman enfeksiyonlarının ayırıcı tanısında sıkça kullanılmaktadır. Kemik protez enfeksiyonlarının tanısında üç fazlı kemik sintigrafisi (Ks), işaretli lökosit sintigrafisi (İLs), HIGs etkinliklerini ve birlikte kullanımının değerini araştırmayı amaçladığımız çalışmamızda, protez enfeksiyon şüphesi olan toplam 29 hasta protez yerlerine göre Grup I (kalça protezi) ve Grup II (diz protezi) olmak üzere iki gruba ayrıldı. Hastalara iki gün ara ile üç fazlı Ks, İLs ve HIGs yapıldı. Sintigrafik yöntemlerin görüntüleri birlikte değerlendirilip enfeksiyon açısından pozitif veya negatif olarak yorumlandı. Kesin tanı cerrahi, histolojik ve bakteriyolojik inceleme veya klinik takiple konuldu. DKs, Ks, İLs, HIGs ve ortak raporun ayrı ayrı duyarlılık, özgüllük, doğruluk oranları, PPD, NPD ve J değerleri hesaplandı. Sonuçta kemik protez enfeksiyonlarının ayırıcı tanısında üç fazlı Ks.ne İLs.nin eklenmesinin kalça protezlilerde özgüllük, doğruluk oranı, PPD.i arttırdığı, HIGs.nin kalça protezlilerde Ks-İLs kombinasyonuna anlamlı katkısının olmadığı görüldü. Diz protezlilerde HIGs.nin duyarlılık, doğruluk oranı ve NPD.nin İLs.ne göre daha yüksek olması, Ks.nin özgüllük, doğruluk oranı ve PPD.ni arttırması nedeniyle diz protez enfeksiyon şüphesinde Ks ile birlikte kullanılabileceğini düşündürdü. Her iki grupta İLs yapılamayan durumlarda veya daha kolay hazırlanan bir yöntem olması avantajı ile İLs.ne alternatif olarak Tc- 99m HIGs yapılabileceği sonucuna varıldı.

Early and true diagnosis for enfection of hip and knee artroplasty very important because of easy and successful treatment. In this reason the metods for using to diagnosis must be very sensitive and specific. But specificity of history, physical examination, biochemical, microbiological, radiological metods are low. Radionuclide imaging techniques practices suspected infection of joint replacement in the differential diagnosis frequently. Aim of this study, comparison the efficiencies of three phase bon scintigraphy (Bs), leucocyt labelled scintigraphy (LLs) and HIG scintigraphy (HIGs) and to investigate the value of the combined use. 29 patients with suspected prosthetic infection were divided into two groups to be prosthesis according to location Group I (hip prothesis) and Group II (knee prosthesis). Three phase bone scintigraphy, leucocyt labelled scintigraphy and HIG scintigraphy were taken to all patients two days interval. After comments the images of scintigraphic methods separately and evaluated combine we reached a conclusion to positive or negative in terms of infection. Results have been verified with surgery, histological and bacteriological investigation or clinical follow-up. Sensitivity, specificity, accuracy rates, PPD, NPD and J values were alculated for DB, Bs, LLs, HIGs and combide report separately. t the end of, LLs plus to hree hase Bs have been increased sensitivity and ccuracy rates for differential diagnosis of bone nfection in the hip prosthesis, but IGs didn.t contribute to Bs-LLs combination significant. ecause of sensitivity and accuracy rates of HIGs higher than ILs on the knee prothesis and increased specificity and accuracy rates of Bs, HIGs may be use with Bs. HIGs an alternative technique to LLS can be suggested because of a method to be more easily prepared all groups.
