Obstetrik cerrahide en önemli girişim sezaryendir ve gittikçe artan bir insidansla tüm doğumların yaklaşık %25.ini oluşturmaktadır. Son yıllarda rejyonal anestezi, hastanın bilincinin açık olması, potansiyel depresan ilaca bebeğin daha az maruz kalması, maternal pulmoner aspirasyon riskinin az olması, arzu edilirse çocuğunun doğumunda annenin uyanık olması, postoperatif ağrı tedavisi seçeneğinin bulunması, aspirasyon riski taşımaması, yenidoğanda solunum depresyonu yapmaması, uterus atonisine yol açmaması gibi avantajları nedeniyle tercih edilmektedir. Çalışmamız elektif sezaryen operasyonu planlanan 40 ASA I-II olgu üzerinde yapıldı. Çalışmada epidural anestezi ile iki farklı lokal anestetik ajanın hemodinamik yanıt, yenidoğan APGAR skoru ve kord kan gazı parametrelerine olan etkisinin araştırılması planlandı. Epidural katater takılarak anestezi uygulanan olgular randomize olarak iki ayrı gruba ayrıldı. Grup L (n=20)'deki olgulara % 0,5 20 mL levobupivakain, Grup B (n=20).deki olgulara % 0,5 20 mL bupivakain epidural aralıktan uygulandı. Duyusal blok başlama, duyusal bloğun T6.ya ulaşma süresi, motor blok başlama, gerileme, kalkma süresi, iki segment gerileme süresi, ilk analjezik ihtiyaç süresi, bulantı-kusma, hipotansiyon, bradikardi gibi yan etkiler, hasta ve cerrah memnuniyeti, analjezi kalitesi kaydedildi. Bebekler doğurtulduktan sonra yenidoğanın umblikal veninden heparinli enjektörle 2 mL kan alındı. Alınan kan örneklerinin (pH, pCO2, pO2, BE) analizi yapıldı. Yenidoğanın değerlendirilmesi bir pediatrist tarafından yapıldı. APGAR 1. ve 5. dk kaydedildi. Her iki grupta epidural girişimin hemodinamik stabilite üzerine yaptığı etkileri değerlendirmek amacıyla epidural girişim öncesi, ameliyat süresince ve sonrasında belirli periyotlarla SAB, DAB, OAB ve KAH düzeyleri değerlendirildi. Grupların kord kan gazı değerlerine ve bebeklerin 1. ve 5. dakikalardaki apgar skorlarına bakıldığında ise sadece Grup B.de 5. dakikadaki apgar skorlarının Grup L.ye göre anlamlı olarak yüksek olduğu görüldü Grup L olgularda motor blok üst seviyeye ulaşma süresi ortalaması grup B'deki olgulardan istatistiksel olarak daha düşük bulundu. Grupların duyusal blok başlama, duyusal blok gerileme, motor blok başlama ve gerileme süresi, duyusal bloğun T6'ya ulaşma süresi, ilk analjezik ihtiyacı süresi, operasyon süresi ortalamaları arasında istatistiksel olarak anlamlı fark gözlenmedi. Hemodinamik parametreler her iki grupta da operasyon esnasında genellikle stabil seyretti. Çalışmamızın sonucunda elektif sezaryen ameliyatlarında epidural yoldan uygulanan iki farklı lokal anesteziğin bebek apgar, kord kangazı ve anne hemodinamisi açısından aralarında farklılık olmadığı kanaatine vardık.

Lately regional anaesthesia has been preferred because of some advantages as follows: presence of consciousness in patient, less exposure of baby to potential depressive drugs, minor risk for maternal pulmonary aspiration, mother can stay awake during the birth of her baby .if desired, the availability of an option to allow post-operative pain management, there is no risk of aspiration, there is no aspiratory depression, and it doesn.t lead uterine atony. Our study was conducted among 40 ASA I-II cases undergoing elective caesarean section. We aimed to investigate the effects of epidural anaesthesia and two other local anesthetic agents on hemodynamic response, newborn APGAR score and on cord blood gas parameters. Cases were randomly seperated into two groups as anaesthesia was performed by placing epidural cathater. 0,5% 20mL of levobupivakain was administered into the epidural space of patients in Group L (n=20), and 0,5% 20mL of bupivakain was administered into the epidural space of patients in Group B (n=20). We recorded the sensorial block onset time, the time taken for sensorial block to reach T6, motor block onset, regression and disapperance, two segment regression time, postoperative first analgesic requirement, side effects such as nausea, hypotension and bradycardia, patient and surgeon satisfaction, and quality of analgesia. After the delivery, 2 mL of blood was taken from umblical vein of newborn with a heparinised needle. The obtained blood samples were analysed (for pH, pCO2, pO2 and BE). Examination of the newborn was done by a pediatrician. APGAR scores at 1 and 5 minutes were recorded. SAB, DAB, OAB and KAH levels were periodically assessed before epidural procedure, during and after the surgery in order to evaluate the effects of epidural procedure on hemodynamic stability in both groups. Considering cord blood gas levels and APGAR scores at 1 and 5 minutes, it was observed that only apgar score at 5 minute in Group B was significantly higher than in Group L. The mean time taken for motor block to reach upper levels in Group L was found to be statistically lower than in Group B. There was no statistically significant difference between two groups with regard to the onset and regression time of sensorial block, the onset and regression time of motor block, the time taken for sensorial block to reach T6, first analgesic requirement and the mean duration of surgery. Hemodynamic parameters were stable in both groups during the surgery. As a result we reached the conclusion that there is no difference between two different local anesthetics administered through epidural space in elective caesarean surgery with regard to newborn APGAR, cord blood gas and maternal hemodynamics.
