Bu çalışmanın amacı KBAS hastalarında farklı dozlarda uygulanan terapötik US tedavisinin klinik iyileşme ve SDY.nin üzerine etkilerini değerlendirmek; terapötik US.nin hangi dozda daha etkili olduğunu saptamaktır. Çalışmaya üst ekstremite KBAS tip 1 tanısı almış 40 hasta ile 45 kontrol alındı. Hastalar çift kör ve randomize olarak üç gruba ayrıldı. Her gruba TENS, fluidoterapi, kontrast banyo, egzersiz programı ve randomizasyon sonuca göre stellar ganglion üzerine 0,5 watt/cm2, 3watt/cm2 veya plasebo kesikli US tedavisi verildi. Ağrı değerlendirilmesi VAS ile, duyu muayenesi monofilaman testi ile yapıldı. El pulpa-distal kıvrım mesafesi ve el kavrama kuvvetleri ölçüldü. İnce el becerisi Grooved pegboard testi ile değerlendirildi. Fonksiyonel yetersizlik DASH, yaşam kalitesi SF-36 anketi ile değerlendirildi. SDY ölçümleri latans ve amplitüd değerleri alınarak kaydedildi. Kontrollere göre VAS skoru ile DASH skoru yüksek (p<0.001, p<0.001), SF-36 tüm parametreleri düşük (p<0.001), el parmak pulpa-distal kıvrım mesafesi uzamış (p<0.001), el kavrama ve tutma güçleri düşük (p<0.001), pegboard yerleştirme ve toplama zamanı uzamış bulundu (p<0.05). Hastaların tutulan ekstremitelerinde monofilaman testi değerleri yüksek saptandı (p<0.05). Tedavi grupları kendi içerisinde tedavi öncesi ve sonrası karşılaştırıldığında değerlendirme parametrelerinde düzelme saptanırken (p<0.05), SDY amplitüd ve latans değerlerinde değişiklik tespit edilmedi (p>0.05). Tedavi sonrası değerlerin post hoc analizlerinde grupların plaseboya üstünlüğü saptanmadı (p>0.017). KBAS hastalarında stellar gangliona terapötik US uygulanması hastaların klinik iyileşmesi ve SDY.na etkisi açısından plasebodan farklı bulunmamıştır. Düşük doz ve yüksek doz terapötik US uygulamaları arasında etkinlik açısından fark saptanmamıştır.

The aim of this study is to evaluate the effect of therapeutic US treatment applied at different doses on clinical improvement and SSR; to determine which dose is more effective as applying therapeutic US treatment in CRPS patients. Forty patients diagnosed as upper extremity CPRS type 1 and 45 control were participated in this study. Patients were divided as double blind and randomly into the three groups. To each group TENS, fluidoterapi, contrast bath, exercise program and according to the randomization results 0,5 watt/cm2, 3 watt/cm2 or placebo pulse US treatment on stellate ganglion was given. Pain was assessed with VAS, sensory examination was performed with monofilament test. Finger pulp-distal crease distance and hand grip strength were measured. Manual dexterity was tested with Grooved Pegboard Test. Functional disability was tested with DASH and quality of life was measured by using SF-36. SSR measurements were recorded by taking latency and amplitude values. VAS and DASH scores high (p<0.001, p<0.001), SF-36 all parameters were low (p<0.001), finger pulp-distal crease distance was prolonged (p<0.001), grip and pinch strengths were low (p<0.001), and pegboard placement and collection time was found prolonged (p<0.05) compared to controls. Monofilament testing values of patients affected extremities were found higher (p<0.05). While improvement was detected in assessment parameters within treatment groups before and after treatment (p<0.05), no changes were detected in SSR amplitude and latency values (p>0.05). Post hoc analysis of values after treatment, superiority of placebo has not been detected in the groups (p>0.017). Therapeutic US applicitation on stellate ganglion in CRPS patients were detected no difference than placebo in terms of clinical improvement and efficacy in SSR. No difference has been detected between low and high dose therapeutic US applicitation in terms of efficacy.
