Günümüzde, patella kırıklarının cerrahi tedavisinde farklı tespit yöntemleri kullanılmaktadır. Bu çalışmada, dana patellasında oluşturulan transvers kırık tespitinde uyguladığımız üç farklı yöntemin distraksiyon kuvvetlerine karşı dayanıklılıklarını tespit etmeyi amaçladık. Tespit yöntemleri olarak; en sık kullanılan modifiye gergi bandı tekniği, malleol vidasıyla fiksasyon ve başsız kanüle herbert vidası yoluyla gergi bandı uyguladığımız üç yöntemi kullandık. Her bir grup 10.ar adet dana patellasından oluşturuldu. Tespit sonrası, diz eklemini 45o fleksiyonda sabit tutmamızı sağlayan özel bir aparat yardımıyla materyal çekme-basma deney sistemine yerleştirildi. Her bir test grubuna eşit zamanlı ve eşit olarak artan distraksiyon kuvvetleri uygulandı. Her bir grup için ayrı ayrı tespitte yetersizlik oluşturan kuvvet ortalamaları tespit edildi. Bu ortalamalar; birinci grupta 350,5ł81,4 newton, ikinci grupta 977,90ł345,66 newton ve üçüncü grupta ise 957,30ł355,94 newton idi. Her üç gruba ait veriler birbirleri ile Mann Withney-U testi kullanılarak karşılaştırıldı. Grup 1 ve 2 arasında ikinci grup lehine anlamlı fark vardı (p=0,0013). Grup 1 ve 3 arasında üçüncü grup lehine anlamlı bir fark vardı (p=0,0002). Grup 2 ve 3 arasında anlamlı bir fark saptanmadı (p=0,6232). Sonuçta, distraksiyon kuvvetlerine dayanıklı en rijit tespit grup 2.de malleol vidası ile fiksasyon ve grup 3.de herbert vidası yoluyla gergi bandı tekniği uyguladığımız yöntem de tespit edildi. Birinci grupta ise dayanıklılık en azdı.

Nowadays, different stabilization methods are used for surgical treatment of patella fracture. In this study, we aimed to determine the resistant against of distraction forces of three different methods which has used for fixation the transvers patella fractures in calf patella. We used three methods which are most frequent methods as fixation modified tension band technique, malleol screw and headless cannulated herbert screw and tension band applied with this screw. Each group has 10 calf patella. The material was placed into the experimental pull push system using a special equipmment (device) by which we managed to keep knee joint in a position of 450 flexion. Each test group were applied in equal time and equal remain distraction forces. For each group, determined seperately avarage strength insufficiency of stabilization. First group 350,5ł81,4 newton, second group 977,90ł345,66 newton and third group 957,30ł355,94 newton. Data of each three group compared each other with Mann ithney-U test. Between group 1 and 2, there more significant difference as benefit of group 2 (p=0,0013). İn group 1 and 3, there was significantly difference as benefit of group 3 (p=0,0002). There was no significant difference between group 2 and 3 (p=0,6232). Consequently, the most rigid fixotion against distraction forces was determined in group 2 with fixation by malleolus screw and in group 3 with the method that we applied tension band technique by herbert screw. In first group resistance was least.
