Ovarian kistlerin tedavisinde laparoskopik eksizyon kullanılması jinekolojik cerrahlar arasında gittikçe artan oranlarda kullanılan ve kabul görmüş bir tedavi yöntemidir. Kist duvarının laparoskopik olarak soyulması sırasında bipolar koagülasyon kullanılması güvenli bir yaklaşımdır. Aynı zamanda eksizyon sonrasında over dokusunda oluşacak kanamanın kontrolü için sıklıkla bipolar elektrokoagülasyon kullanılmaktadır. Bununla birlikte bu cerrahi prosedürün rezidüel ovaryan parankim üzerine olan yan etkileri konusundaki veriler henüz yeterli değildir. Bizim çalışmamızın amacı laparoskopik ovaryal kist eksizyonu işlemlerinde en sık tercih edilen elektrokoagülasyon ve intracorporael sütür uygulamalarının, over rezerv belirteçleri üzerine etkisini araştırmaktır. Bu amaçla araştırmaya katılmayı gönüllü olarak kabul eden, menstruel siklusları düzenli seyreden, perimenopozal semptomları bulunmayan, beden-kitle indeks değeri normal sınırlar içinde olan, perimenapozal semptomları olmayan, herhangi bir endokrin patolojisi saptanmamış, ailesinde prematür over yetmezliği hikayesi olmayan 60 hasta çalışmaya dahil edildi. Hastalar yaş ortalaması eşit olacak şekilde iki gruba bölündü. Hastalar preoperatif dönemde sikluslarının üçüncü günü çağrılarak serum bazal FSH, LH, östradiol ve inhibin B hormon seviyeleri tespit edildi. Operasyon sonrası 1. ve 3. ayında tüm hastalar kontrol amacı ile çağrıldı. Hastaların siklusun üçüncü gününde serum bazal FSH, LH, östradiol ve inhibin-B hormon tetkikleri tekrarlandı, transvaginal ultrasonografi ile bazal over hacmi ölçümü ve antral follikul sayımı yapıldı. Tüm ultrasonografik ölçümler tek bir araştırmacı tarafından gerçekleştirildi. Yapılan değerlendirmede bipolar elektrokoagülasyon yöntemi ile kanama kontrolü yapılan hastaların hormonal over rezerv belirteçlerinde, postoperatif 1. ve 3. ay ve preoperatif dönemler arasında FSH sevyelerinde istastistiksel olarak anlamlı bir artış saptandı. Diğer hormonal over rezerv belirteçlerinde, postoperatif ve preoperatif dönemler arasında istastistiki bir farklılık saptanmadı. Ultrasonografi belirteçler olan bazal over hacmi ve antral follikul sayisinda postoperatif dönemde sütür grubuna kıyasla istatistiki bir düşüş izlendi. Bu bulgular literaturdeki, over rezerv belirteçlerinden ultrasonografik bulguların daha erken dönemde etkilendiği bilgisi ışığında değerlendirilerek, bipolar elektrokoagülasyon yöntemi ile kanama kontrolü yönteminin over rezervine olumsuz etkisinin olabileceği düşünülmüştür. Ancak bu konuda daha uzun postoperatif dönem takipli çalışmalara ihtiyaç vardır. Sütür ile kanama kontrolü yapılan hastaların ise hormonal ve ultrasonografik over rezerv belirteçlerinde, postoperatif ve preoperatif dönemler arasında istastistiki bir farklılık saptanmadı. Dolayısı ile bu tekniğin over rezevine olumsuz etkisi olduğu söylenilememektedir.

Laparoscpic excision with stripping of the cyst wall is considered an adequate treatment for ovarian cysts. İt is a safe aproach to use bipolar coagulation while stripping the cyst wall with laparoscopic excision. Bipolar electrocoagulation is also use to control the bleeding at the overial tissue after the excition.however, there is no adequate data of the sideeffects of this surgical procedure on the residuel ovarian parenchyma. The aim of our study is to determine the effects of the most frequently utilized methods of bipolar electrocoagulation and intracorporeal suture application in laparoscopic ovarian cystectomy procedures on the ovarian reserve indicators. With this purpose, 60 patients, who volunteered to participate in the study; with regular menstrual periods and BMI.s in the normal range; without premenopausal symptoms, any endocrinological pathologies, and familiy histories of premature ovarian failure were included in the study. The patients were placed into two groups with similar mean ages. The basal serum levels of FSH, LH, estradiol and inhibin-B hormone were obtained on the third day of menstruation in the preoperative period. All patients were reexamined 1. and 3. months after their operation. Repeated FSH, LH, esradiol and inhibin B hormone basal serum levels and ultrasonographic measurements of total basal ovarian volume and anthral follicule count were obtained from the same patients on the third day of menstruation. All ultrasonographic data were obtained by the same operator. In patients in whom ovarian cystectomy was performed by means of electrocoagulation, at the hormonal ovarian reserve indicators between the postoperative first and thirth month and preoperative periods statistically significant differencences of FSH levels were detected. There were no statistically significant differences detected between postoperative and preoperative other hormonal ovarian reserve indicators. A statistically significant reduction in the ultrasonographic indicators of ovarian reserve basal ovarian volume and anthral follicule count . was detected in the postoperative period when compared with the suture group. Judging from the recent knowledges in literature that ultrasonographic indicators give earlier signs of changes in the ovarian reserve than hormonal indicators, use by electrocoagulation is thought to have adverse effects on the ovarian reserve. However, more long term studies on the subject are required. In patients in whom ovarian cystectomy was performed by the application of suture, no statistically significant differences either in the hormonal or the ultrasonographic ovarian reserve indicators between the postoperative and preoperative periods were detected. Consequently, it can not be said that this technique has adverse effects on the ovarian reserve.
