Ailevi Akdeniz Ateşi (AAA); tekrarlayan ateş, peritonit, sinovit, plörit ve nadiren perikardit atakları ile karakterize otozomal resesif geçişli genetik bir hastalıktır. Hastalığın ve aktivasyonların patogenezinde birçok mekanizma ileri sürülmüşse de neden tam olarak bilinmemektedir. Ataklar esnasında nötrofillerin kemotaktik aktivitesi ileri derecede artmıştır. Tutulmuş olan dokularda belirgin nötrofil hakimiyeti mevcuttur. Bu durum hastalığın aktivasyonunda nötrofil aktivasyonunun rolünü düşündürmektedir. Ayrıca nötrofil aktivasyonunda oksidatif streste artış ve sitozole kalsiyum (Ca+2) girişi rol almaktadır. Bu çalışmaya 6 AAA atak, 6 remisyon, 6 aktif hasta ile 6 sağlıklı birey olmak üzere 24 kişi alındı. Oksidatif stresi değerlendirmek amacıyla lipid peroksidasyonu (LPO) göstergesi olan Malonildialdehit (MDA) ve antioksidanlardan Glutatyon (GSH) ve Glutatyon Peroksidaz (GSH-Px) değerleri nötrofillerde ve serum örneklerinde çalışıldı. Ayrıca sitozole kalsiyum girişi gruplar arasında değerlendirildi. Nötrofillerdeki MDA değerleri AAA atak grubunda kontrol ve remisyon grubuna göre anlamlı oranda yüksek saptandı (p<0.05). Nötrofillerdeki MDA değerleri kolşisinle remisyon sağlanamayan grupta da diğer tüm gruplara göre anlamlı oranda yüksekti (p<0.01). Serumdaki MDA düzeyleri de atak grubunda kontrol ve remisyon grubuna göre, kolşisine cevapsız grupta da remisyon grubuna göre yüksek saptandı. GSH ve GSH-Px değerleri, nötrofil ve serumlarda gruplar arasında anlamlı farklılık göstermedi. Sitozole Ca+2 girişi atak ve tedaviye cevapsız grupta MDA değerleriyle paralel olarak artış gösterdi. Nötrofillerde oksidatif stres ve sitozole kalsiyum girişi aktif ve tedaviye cevap vermeyen AAA hastalarında artmaktadır. Kolşisine cevap veren hastalarda oksidatif stres ve nötrofillerde Ca+2 artışı bulunmamıştır. Kolşisin hem oksidatif stresi hem de nötrofil içi kalsiyum artışını önlemede etkili olabilir.

Familial Mediterranean fever (FMF) is an autosomal recessive genetic disorder which is characterized by relapsing fever, peritonitis, synovitis, pleuritis and rarely pericardititis. Despite, many etiological mechanisms have been proposed in pathogenesis of the disease itself and clinical activations, the etiology is not known exactly. Chemotactic activities of neutrophils are increased during the attacks. In the involved tissues neutrophil predominance is evident. It is suggested that the role of neutrophil activation may play a role in disease activation. Also increased oxidative stress and increase in the cytosolic calcium entry may play a role in neutrophil activation. In this study, we aimed to search the efficacy of colchicines on cytosolic calcium entry and oxidative stress in neutrophils and sera of the FMF patients. Six FMF patients during attack, 6 in remission, 6 non-responsive to colchicine and 6 healthy, a total of twenty four subjects were included to our study. As oxidative stres indicators, malonyldialdehyde levels for lipid peroxidation, GSH and GSH-Px levels for antioxidant status were studied both from fresh neutrophils and sera. And, cytosolic calcium influx in neutrophils was also evaluated between groups. MDA levels in neutrophils were significantly high in attack group than the control and remission groups (p<0.05). Also, MDA levels in neutrophils were significantly high in non-responsive group according to remission group (p<0.01). No differences were found between groups according to GSH and GSH-Px levels both in sera and neutrophils. The increase in cytosolic calcium entry was in parallel to the increase in MDA levels both found in attack and non-responsive groups. Oxidative stress and cytosolic calcium influx in neutrophils are increase in patient with attack and non responsive to colchicine. Colchicine may be effective both in preventing oxidative stress and calcium influx.
