Yetersiz barsak uzunluğundan ya da özel fonksiyonları olan barsak bölümlerinin kaybından kaynaklanan ve malabsorpsiyona ait sorunları taşıyan patolojiler zinciri olan kısa barsak sendromu (KBS), gerek çocuk ve gerekse erişkin hastalarda, çoğu zaman uzun dönem takip ve tedavi gerektiren önemli bir sorundur. Bu sendromda barsaklardaki emilim yüzeyi azalarak yaşamı devam ettirici temel besin maddelerinin canlı organizma tarafından alınamamasına bağlı birbiri ile ilişkili gastrointestinal ve sistemik değişiklikler söz konusudur. KBS.lu hastaların yaşam süreleri ve beslenmeleri geride kalan barsakların uzunluğu ve adaptasyon yetenekleriyle yakından ilişkilidir. Bu tezin amacı, kısa barsak sendromu oluşturulan ratlarda hiperbarik oksijen tedavisinin intestinal adaptasyon üzerindeki etkinliğinin araştırılmasıdır. Bu amaçla toplam 40 adet wistar rat, sham operasyonu (laparotomi + transeksiyon / anastomoz) yapılan grup ve deneysel olarak kısa barsak sendromu oluşturulan grup olarak sınıflandırıldı. Her bir grup kendi içinde hiperbarik oksijen tedavisi verilen / verilmeyen gruplar olarak sınıflandırıldı. Postoperatif 3. gün başlanan hiperbarik oksijen tedavisi; 4 gün boyunca, %100 oksijen 2.8 atm basınçta, günde 2 defa 90 dakika uygulandı. Postoperatif 7. günde rezeksiyon yapılarak ileum, jejunum, kolon örnekleri %10 formalin içinde korundular. Jejunum, ileum ve kolon preparatları; proliferasyon indeksi, apopitozis indeksi ve mukozal hasarın derecesi açısından değerlendirildi. İstatistiksel analiz Tanımlayıcı İstatistiksel Testler, OneWay ANOVA ve Ki-Kare testleriyle yapıldı. Günümüzde medikal ve cerrahi tedaviye alternatif çeşitli tedavi yöntemleri denenmektedir. Bu tedavilerde hedeflenen sonuç hastanın enteral yol ile metabolik gereksinimlerini karşılayabilir duruma gelmesidir ve bu da intestinal adaptasyona bağlıdır. Sonuç olarak, yaptığımız çalışmada HBOT intestinal adaptasyonu olumlu olarak etkilemiştir ve KBS.lu olgularda yardımcı tedavi aracı olarak kullanılabilir.

Short bowel syndrome (SBS), which is caused by inadequate bowel length or loss of the bowel sections with special function, is the chain of pathologies containing malabsorption problems. It is a serious problem that often requires long term follow-up and treatment for both children and adults. In this syndrome, there are interrelated gastrointestinal and systemic changes due to decreased intestinal absorption surface that causes deficiency of vital nutritional elements in live organism. It is obvious that survival and nutrition are dependent on bowel length and intestinal adaptation in patients with SBS. The aim of this thesis is to investigate effectiveness of hyperbaric oxygen therapy on intestinal adaptation in rats with SBS. For this purpose, 40 Wistar albino rats were divided into two groups as shamoperated (laparotomy plus transection/anastomosis) and short bowel, and two subgroups were identified for each group as given hyperbaric oxygen therapy (HBOT) or not given. HBOT, which was started on the postoperative 3rd day, was applied with 100% oxygen at 2.8 ATA pressure 2 times a day for 90 minutes for 4 days. Sacrification was performed on the postoperative 7th day, and jejunal, ileal and colonic specimens were obtained from all groups and then all of the specimens were put into 100% formaline solution. Histopathological investigations were performed to evaluate proliferation index, apoptosis index and degree of intestinal mucosal damage. Statistical analyzes were made by descriptive statistical tests, one way ANOVA and Chi-Square tests. Today, various treatment methods are being tested in addition to conventional medical and surgical treatment for SBS. The expected result of such studies is to gain ability to provide metabolic necessity of the patients via enteral nutrition which is dependent on intestinal adaptation. In conclusion, HBOT has a positive effect on intestinal adaptation in our study; we suggest that HBOT may be used as an alternative treatment method in patients with SBS.
