2008 - 2009 yıllarında, ASA I-II grubunda, pron pozisyonda genel anesteziyle PNL operasyonu gerçekleştirilmiş; peroperatif PEEP uygulanmayan ve 5-7 cmH2O PEEPuygulanan 40 hasta dosyası taranarak kan gazı, hemodinami ve solunum fonksiyon testleri üzerine etkileri araştırıldı. Arteriyel kan gazları, indüksiyon sonrası supin 5., pron 5., 30., 60. ve 90. dk değerleri kaydedildi. Hemodinamik veriler, operasyon öncesi bazal değer alınarak entübasyon sonrası 1. ve 5. dk, pron pozisyonda 1., 5., 10., 20., 30., 40., 50., 60., 70., 80., 90. dk değerleri kaydedildi. Preoperatif ve postoperatif solunum fonksiyon testleri verileri karşılaştırıldı. Her iki grubun tüm olgularında kontrol değerlerine göre pH değerlerindeki değişimlerin anlamsız düzeylerde olduğu saptandı. Kontrol grubuna oranla Grup PEEP olgularda PaO2 ve % SO2 değerlerindeki yükselmenin daha anlamlı olduğu saptandı. Ancak CO2 atılımında Grup PEEP olgularda anlamlı bir atılım artışı saptanırken, kontrol grubundaki olgularda kontrol PaCO2 değerleri operasyon süresince anlamsız değişiklikler gösterdi. Solunum fonksiyon testlerinin grup içi ve gruplararası karşılaştırılmasında anlamsız değişiklikler saptandı. Pron pozisyon ve bu pozisyonda PEEP uygulamasının hemodinami üzerine olan olumsuz etkisi, ortalama arter basıncında düşme şeklinde gözlendi. Bu düşüş her iki grup olgularda anlamlı düzeyde saptandı. Ancak değerler normal klinik sınırlar içerisinde kaldı. Aynı şekilde kalp tepe atımı değerleri de her iki grup olguda normal klinik sınırlar içerisinde olmasına rağmen anlamlı düzeyde azalma gösterdi. Pron pozisyonda intraoperatif 5-7 cmH2O PEEP uygulamasının; hemodinami üzerine minimal olumsuz etkilerinin olduğunu, bu etkinin normal klinik sınırlar içerisinde bulunduğunu; solunum sistemi üzerine etkisinin ise oksijenizasyonu iyileştirici yönde olduğunu, CO2 atılımını anlamlı derecede artırdığını, postoperatif solunum fonksiyon testlerine yansıyan olumsuz bir etkisinin bulunmadığını saptadık.Anahtar Kelimeler: Genel anestezi; Pron pozisyon; PEEP; Kan gazı analizi;
Hospital records of 40 patients in ASA I-II group, who have undergone PNL operation at prone position with 5-7 cmH2O PEEP application and without preoperative PEEP treatment, were reviewed and the possible effect of PEEP on arterial blood gas, hemodynamic status and respiratory functions was investigated. After the induction, arterial blood gas values were recorded for whole patients at 5th min with supine position and at 5th, 30th, 60th and 90th minutes with prone position. Hemodynamic measurements were performed before the operation, at 1st and 5th minutes after the entubation and during the 1st, 5th, 10th, 20th, 30th, 40th, 50th, 60th, 70th, 80th and 90th minutes intraoperatively at prone position. Also, preoperatively and postoperatively established respiratory function data were compared. It was found that there was no statistically significant difference in both groups by means of pH values, when compared with the controls (p >0,05). The increment in partial oxygen pressure (PaO2) and oxygen saturation percent (SO2 %) values were more prominent in PEEP Group cases, when compared with the controls. However, as there was a significant increase in CO2 exhalation in PEEP Group cases, the partial carbondioxyde pressure (PaCO2) values measured from the control group exerted non-significant changes during the operation. The comparison of respiratory function test values in the same group and between two groups has yielded non significant results (p >0,05). The negative effect of prone position and PEEP application at this position on hemodynamic status was observed as the decrement in mean arterial pressure and this was significant for both groups, but the measured values in normal accepted clinical range. Likewise, heart pulse values were prominently decreased in two groups within normal range. As a result, we have concluded that intraoperative 5-7 cmH2O PEEP application has minimal negative effect on hemodynamic status which is within normal clinical limits, whereas this treatment has ameliorating effect on oxygenization by significantly increasing the CO2 exhalation and has no side effect on post-operatively measured respiratory function tests.
