Larinks patolojili hastaların tanısında sıklıkla endoskopi kullanılır. Bununla birlikte invazivdir ve endoskopinin değeri cerrahın deneyimine bağlıdır. Sanal laringoskopi (SL) larinksin intraluminal yüzeylerinin görüntülenmesine izin veren yeni bir görüntüleme yöntemidir. Özellikle klinik muayeneyi tolere edemeyen hastalarda, enfeksiyon, neoplazi ya da gelişimsel defektler eşlik ettiğinde lümen ve subglottik alanın incelenmesi için kullanılır. Son yıllarda çok kesitli bilgisayarlı tomografi sayesinde iki boyutlu görüntülerin rekonstrüksiyonu ile yüksek görüntü kalitesinde sanal endoskopi görüntülerinin oluşturulması mümkün hale gelmiştir. Elde edilen görüntüler, patolojik oluşumların üç boyutlu olarak daha kolay değerlendirilmesine imkan vermekte, ameliyat öncesi veya ameliyat sonrası kontrollerde cerraha ve radyoloğa ek bilgi sağlamaktadır. Çalışmamızda larinks patolojisi düşünülen 40 hastada SL'nin etkinliği, video laringoskopi bulguları, hastaların ameliyat notları ve histopatoloji sonuçları ile beraber değerlendirildi. SL'nin larinks patolojilerini değerlendirmede duyarlılığı %90, özgüllüğü %55, pozitif tahmin değeri %87,5, negatif tahmin değeri %62,5 ve doğruluğu %82,5 olarak bulundu. SL özgüllüğünün düşük olması ve biyopsi olanağı bulunmaması nedeniyle video laringoskopinin yerini alamaz. Ancak video laringoskopinin yetersiz kaldığı durumlarda tamamlayıcı olarak kullanılmalıdır. SL'nin avantajları noninvaziv olması, farklı açılardan görüntülemeye olanak sağlaması ve darlıkların değerlendirilmesinde ideal bir teknik olmasıdır. Lezyonun transmural uzanımı ve çevre yapılar gösterilebilir. İki tekniğin beraber değerlendirilmesi mükemmel bir üç boyutlu yorumlama sağlar.

Endoscopy used frequently in the patients who has larynx pathology. However, it is an invasive procedure and its. worth is related to the surgeons. experience. Virtual laryngoscopy (VL) is a new imaging technic which allows to provide imaging of the intraluminal surface. İt is used especially to evaluate the lumen and subglottic area, in the patients which can not tolerate the clinical examination or when there is an infection, neoplasm and developmental defects. In the last years, it became possible to constitute high resolution virtual endoscopy images by reconstruction of two dimensional images with the multislice computed tomography. The images achieved, allows three dimensional evaluation of the pathological constitutions, provides additional data to the surgeon and radiologist before and after the operation. In our study, efficiency of VL, findings of video laryngoscopy, patients surgery and pathology reports are evaluated together in the 40 patients who are thought to have larynx pathology. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of VL to evaluate the larynx pathologies were % 90, % 55, % 87,5, % 62,5 and % 82,5 respectively. Because of low specificity and no ability to take biopsy VL can not replace video laryngoscopy. However, when the video laryngoscopy is inadequate it will be complementary. Being a noninvaziv technic and ideal method for stenosis, evaluation from the different angles the advantages of the VL. Demonstration of propagation of the lesions and adjecent tissues is possible. Estimation of two technics together will provide excellent three dimensional interpretation.
