Bu çalışmanın amaçları; i) koroner arter bypass greft cerrahisi (KABGC) uygulanan hastalarda erken postoperatif dönemde aspirin direnci gelişme sıklığı ve bu direncin geri dönüşümlü olup olmadığını ve ii) KABGC sonrası erken dönemde aspirin direnci gelişmesinde tromboksan A2 üretiminin yetersiz inhibisyon mekanizmasının etkinliğini araştırmaktır Çalışmaya, Şubat 2009-Mart 2010 tarihleri arasında, koroner arter hastalığı tanısı ile kliniğimizde elektif KABGC uygulanan 80 hasta (ortalama yaş 63.1ł9.19, 55 erkek)alınmıştır.Hastalardan preoperatif, postoperatif yedinci ve on beşinci günlerde alınan venöz kan örnekleri PFA.100 cihazı ile değerlendirildi. Kollajenepinefrin kapanma zamanının 186 sn.den düşük olması durumunda aspirin direnci tanısı konuldu. Ayrıca postoperatif birinci gün alınan idrar örneklerinde 11-dehidro tromboksan B2 düzeyleri ölçüldü.Preoperatif dönemde 23 hastada (% 28.75), postoperatif yedinci günde 31 hastada (%38.75) ve postoperatif on beşinci günde 25 hastada (% 31.25) aspirin direnci saptandı.Postoperatif yedinci günde aspirin direnci olan hastalardaki 11-dehidro tromboksan B2 düzeyleri, aspirin direnci olmayan hastalardaki düzeylere göre anlamlı derecede daha yüksekti (p < 0.001). Postoperatif yedinci günde yeni aspirin direnci gelişen hastaların ortalama aort kross-klemp süresi (66.41 ł 29.44 vs 50.73 ł 18.78, p=0.003) ve kardiyopulmoner bypass süresi (105.22 ł 42.23 vs 89.28 ł 28.74; p=0.029) aspirin direnci olmayan hastalardaki sürelere göre anlamlı derecede daha yüksekti.Bu çalışmanın sonuçalrı;i) kardiyopulmoner by pass ile KABGC sonrası ilk yedi gün içinde aspirin direnci geliştiği ve bu direncin de büyük oranda geçici olduğunu ve ii) KABGC sonrası erken dönemde aspirin direnci gelişmesinde tromboksan A2 üretiminin aspirin tarafından yetersiz inhibe edilmesi mekanizmasının etkili olduğunu desteklemektedir.

The aims of this study are to investigate i) the frequency of development of aspirin resistance in the early postoperative period in patients undergoing coronary artery bypass grafting (CABG) and whether this resistance was reversibl and ii) the efficiency of the mechanism of incomplete inhibition of thromboxane A2 in development of aspirin resistance in early postoperative period after CABG. Between February 2009 and March 2010, 80 patients (mean age 63.1ł9.19,55 male) with coronary artery diases diagnosis who underwent elective CABG in our clinic were included into the study. The venous blood samples were collected from patients and processes by PFA-100 in preoperative period and in postoperative seventh and fifteenth days. Aspirin resistance diagnosis was described as a collagenepinefrin closure time lower than 186 second. Urine levels of 11-dehidro thromboxane B2 were also measured on postoperative first day. Aspirin resistance was found in 23 patients (28.75 %) in the preoperative period, in 31 patients (38.75 %) on postoperative seventh day and in 25 patients (31.25 %) on postoperative fifteenth day. The urine levels of 11-dehidro thromboxane B2 in patients with asprin resistance on postoperative seventh day were significantly higher than the levels in patients without aspirin resistance (p < 0.001).The mean aortic cross clamping time (66.41 ł 29.44 vs 50.73 ł 18.78, p=0.003) and cardiopulmonary bypass time (105.22 ł 42.23 vs 89.28 ł 28.74; p=0.029) in the patients with aspirin resistance on postoperative seventh day were significantly higher than the levels in patients without aspirin resistance.The results of this study suggest that i) aspirin resistance develops inpostoperative seven day after CABG and this resistance is highly reversibl and ii) the mechanism of inadequate inhibition of thromboxane A2 by aspirin is efficient during development of aspirin resistance in early postoperative period after CABG.
