Bölgesel anestezi günübirlik ortopedik cerrahide önemli ve artmakta olan bir role sahiptir. Bu çalışmada günübirlik diz artroskopisi yapılacak olan hastalarda spinal anestezi ile kombine siyatik ve femoral sinir bloğunun karşılaştırılması amaçlandı. ASA I-II grubuna giren, rastgele seçilmiş 40 hasta iki gruba ayrıldı. Grup S (n=20) olgulara unilateral spinal anestezi düşük doz levobupivakain ile gerçekleştirildi (1 mL % 0.05.lik levobupivakain + 1 mL SF, toplam 2 mL). Grup B (n=20) olgulara kombine siyatik ve femoral sinir bloğu için, 30 mL % 0.05 levobupivakain + 10 mL % 2 lidokain olacak şekilde toplam 40 mL olarak hazırlanan karışım 15 mL siyatik sinire, 25 mL femoral sinire uygulandı. SAB, DAB, OAB, KAH, SpO2.nin bazal değerleri ve ilaç uygulamasını takiben, 1., 3., 5., 10., 15., 20., 30., 45., 60., 90., 120. Dk değerleri kaydedildi. Teknik uygulama süresi, cerrahiye teslim etme süresi, maksimum motor ve duyusal blok seviyeleri, intraoperatif anestezi kalitesi, operasyon süresi, intraoperatif ve postoperatif dönem komplikasyonları, motor blok süresi, postoperatif ilk idrara çıkma zamanı, postoperatif ek analjezik yapılan hasta sayısı, postoperatif analjezinin süresi, postoperatif 24 saat toplam analjezi tüketim miktarı tespit edildi. Gruplar arasında demografik veriler ve operasyon süresi yönünden bir fark saptanmadı. Grup S.te peroperatif hemodinamik parametrelerde değişiklikler gözlenirken Grup B.de hemodinami stabil seyretti. Teknik uygulama süresi Grup S.te çok anlamlı olarak kısa olmakla birlikte (p<0.0001), cerrahiye teslim etme süresi iki grupta benzer bulundu. Maksimum motor blok Bromage skoru opere olan tarafta Grup S.te Grup B.den daha yüksekti (p=0.008). Ayrıca Grup S.te opere olmayan tarafta 5 hastada motor blok gelişti (p=0.022). Maksimum duyusal blok seviyesi de opere olan tarafta Grup S.te Grup B.den daha yüksekti (p<0.0001). Opere olmayan tarafta ise Grup S.de sadece 8 hastada duyusal blok oluşmadı (p<0.0001). Grup S.te intraoperatif dönemde hastaların hiçbiri ek sedasyon ve analjezik ihtiyacı duymazken Grup B.de 6 hastada sedasyon amacıyla peroperatif dönemde midazolam verildi. 1 hastada ise ağrı duyduğu için fentanil ilavesi gerekti. Her iki grupta intraoperatif bulantı-kusma, hipotansiyon, bradikardi ve diğer yan etkiler görülmezken Grup B.de 2 hastada turnike ağrısı gözlendi. Postoperatif dönemde Grup S.te 1 hastada baş ağrısı, Grup B.de 2 hastada iğne yerinde ağrı gözlendi. Diğer postoperatif komplikasyonlar her iki grupta da gözlenmedi. İlk idrara çıkma zamanı yönünden gruplar arası fark anlamsız iken, motor blok süresi Grup B.de istatistiksel olarak çok anlamlı uzundu (p<0.0001). Operasyon sonrası ek analjezik gereksinim yönünden gruplar arası farklılık anlamlı bulundu (p<0.0001). Ek analjezi ihtiyacı duyan hastalara analjezi yapılma süreleri yönünden de fark anlamlı bulundu (p=0.032). Sonuç olarak, her iki teknik yeterli anestezi sağlamakla birlikte unilateral spinal anestezi uygulamasında minimum dozda levobupivakain kullanmamıza rağmen kombine siyatik femoral blok uygulamasında hemodinami daha stabil seyretmiştir. Bu sebeple özellikle yüksek riskli hastalarda kombine siyatik femoral bloğun tercih edilmesi gerektiğini; levobupivakain kullanarak yapılan kombine siyatik femoral blok uygulmasında motor blok süresinin çok uzun olması nedeniyle günübirlik olgularda kısa etkili lokal anestezik kullanılmasının daha uygun olacağını düşünmekteyiz. Günübirlik hastalarda uygulanacak en uygun anestezik tekniğin uzmanlar tarafından bütün bunlar göz önüne alınarak değerlendirilmesi gerektiği düşüncesindeyiz.

Regional anesthesia has an increasing role and is important in orthopedic surgery of outpatients. In this study we aimed to compare unilateral spinal anesthesia technique with low dose Levobupivakain and combined sciatic femoral block in outpatients who will undergo arthroscopic knee surgery. 40 patients included in the ASA I-II group were randomly divided into 2 groups. Low dose Levobupivakain (1 mL 0.05 % levobupivakain + 1 mL 0.9 % NaCl, totally 2 mL) was administered via unilateral spinal anesthesia to the individuals in Group S (n=20). And to provide combined sciatic and femoral nerve blockage in Group B (n=20) 15 mL of the total 40 mL (30 mL 0.05 % levobupivakain + 10 mL 2 % lidocaine) was administered to the sciatic nerve, and 25 mL to the femoral nerve. SAP, DAP, MAP, HR and SpO2 values were recorded initially and 1, 3, 5, 10, 15, 20, 30, 45, 60, 90, 120 minutes after drug administration. Duration of the technical application, the period of time between admission and delivery to surgery, the maximum motor and sensorial block levels, the quality of intraoperative anesthesia, the duration of the operation, intraoperative and postoperative complications, the duration of the motor block, the first urination time after the operation, the number of the patients requiring analgesics after the operation, duration of the postoperative analgesia, the amount of analgesic consumption within 24 hours after the operation were determined. There was no significant difference between the groups with respect to demographic data and the operation time. We observed alterations in perioperative hemodynamic parameters in Group S while hemodynamics were stable in Group B. The period of time between admission and delivery to surgery was similar in both groups, while the duration of technical application was significantly shorter in Group S (p<0.0001). Maximum motor block bromage score for the operated part was higher in Group S than it was in Group B (p=0.008). Also a motor block occured in the unoperated part in 5 patients (p=0.022). Maximum sensorial block level in the operated part was higher in Group S than it was in Group B (p<0.001). There were only 8 patients in Group S that didn.t develop sensorial block in the unoperated part (p<0.0001). While none of the patients in Group S needed any additional sedation and analgesics in the intraoperative period, 6 patients in Group B were given midazolam for sedation in perioperative period. And 1 patient was given additional fentanil for pain relief. There were no intraoperative nausea, hypotension, bradycardia or other side effects in any of the groups when 2 patients in Group B were observed to have tourniquet pain. 1 patient from Group S had headache and 2 patients from Group B had pain at the injection area in the postoperative period. The other postoperative complications were not seen in either of the two groups. Motor block duration was significantly longer in Group B (p<0.0001), while there was no significant difference between the groups with regard to the first urination time. The difference between the two groups with regard to the need of analgesics in the postoperative period was significant (p<0.0001). Also the difference between the groups in the times of analgesic administiration to the patients who need additional analgesics was significant (p=0.032). Consequently, while both of the techniques provide sufficient anesthesia, though we used minimum doses of levobupivakain for unilateral spinal anesthesia application, hemodynamics were more stable with combined sciatic femoral block application. Therefore, we believe that the use of short-term local anesthetics in outpatient cases is more appropriate since sciatic femoral block application performed using levobupivakain results in longer motor block duration. We also believe, particularly for patients at high risk, that combined sciatic femoral block should be preferred. We are convinced the most appropriate anesthetic technique that will beused in outpatients should be evaluated by the specialists after all these are taken into account.
