İnflamatuvar barsak hastalığı (İBH), doku hasarı ve proinflamatuvar mediatörlerin salınımı ile karakterize inflamatuvar bir hastalıktır ve barsak dışı çeşitli organ tutulumları gösterebilmektedir. Yapılan klinik çalışmalarda, İBH'lı hastaların %1-26.sında asemptomatik/semptomatik akciğer tutulumu saptanmıştır. Bununla birlikte İBH.lı hastalarda akciğer tutulumunun histopatogenezi ile ilgili bilgilerimiz son derece sınırlıdır. Çalışmamızda İBH.da akciğer tutulumunu doku düzeyinde gösterebilmek ve muhtemel etyolojik faktörleri ortaya koymak amacıyla, deneysel kolit modelinde akciğer tutulumunun histopatolojik olarak incelenmesi ve doku vasküler endotelial growth faktör (VEGF) ve Tümör nekrozis faktör alfa (TNF-.) düzeylerinin araştırılması planlanmıştır. Çalışmaya 24 tane Wistar albino sıçan alındı ve 4 gruba ayrıldı; 2 ayrı grupta Dekstran sülfat sodyum (DSS) ve Trinitrobenzen sulfonik asid (TNBS) ile akut kolit oluşturuldu, diğer 2 grup ise her bir kolit modeli için kontrol grubu olarak alındı. Sıçanların kolonları ve akciğerleri çıkarılarak, her iki dokunun makroskopik (kolonda Wallace skorlaması esas alınarak) ve akciğerlerin histopatolojik incelemesi yapıldı. Akciğer dokusundaki VEGF ve TNF-. konsantrasyonları ELISA yöntemiyle ölçüldü. Her iki kolit grubunda da makroskopik kolit skoru kontrol gruplarından anlamlı derecede yüksekti (p=0.002 ve p=0.002). TNBS ve DSS kolit oluşturulan grupların akciğer dokularının histopatolojik incelemesinde yaygın alveolar hemoraji görüldü. Kontrol gruplarının akciğer dokularının histopatolojik incelemesi normaldi (p=0.015 ve p=0.015). Akciğer dokusundaki VEGF (p= 0.002 ve p=0.004) ve TNF-. (p= 0.002 ve p=0.002) düzeyleri kontrol gruplarına göre anlamlı olarak yükselmişti. Kolon dokusunda TNF-. düzeyi kontrol grubuna göre daha yüksek bulundu (p=0.002). Sonuç: İBH oluşturulan sıçanların akciğerlerinde, kontrol grubundan farklı olarak alveolar hemoraji oluştuğu gözlenmiştir. İBH'lı sıçanların akciğer dokularında aynı zamanda VEGF ve TNF-. düzeyleri de artmaktadır. Bu bulgular, İBH.nın sistemik tutulumunda TNF-..nın bir proinflamatuvar sitokin olarak önemli bir rol oynadığını, alveolar hemorajiye akciğerde VEGF artışının eşlik etmesi nedeniyle, alveolar permeabilite artışının indüklediği alveolar hemoraji ile başlayan, pulmoner hemosiderozis ve fibrozise kadar gidebilecek bir patolojik sürecin söz konusu olduğunu düşündürmektedir.

Inflammatory bowel disease (IBD) is characterized by tissue damage and proinflammatory mediator secretion and may show various organ involvements apart from bowel. In the clinical studies, it was found out that %1-26 of IBD patients had asymptomatic/symptomatic lung involvement. However, our knowledge on histopathogenesis of lung involvement in IBD patients is limited. In this study it is aimed to investigate histopathological features of lung involvement in experimental colitis model, and the tissue levels of vascular endothelial growth factor (VEGF), tumor necrosis factor alpha (TNF-.) to show lung involvement histologically, and possible etiological factors, . The study included 24 Wistar albino rats which were classified as four groups; in the first two groups, acute colitis was induced with Dextran sulfate sodium (DSS) and Trinitrobenzene sulfonic acid (TNBS). The second two groups were taken as control groups for each colitis type. After extraction of colons and lungs of rats, both tissues were analysed macroscopically (by using Wallace score index for colon), and the lungs were examined histopathologically. The VEGF and TNF-. concentrations in the lung tissue were measured by ELISA method. In both colitis groups macroscopic colitis scores were significantly higher than the control groups (p=0.002 and p=0.002). The histopathological examination of the lung tissues of TNBS and DSS colitis groups showed a widespread pulmonary hemorrhagea. The histopathological features of lung tissues was normal in the control group (p=0.015 and p=0.015). VEGF (p= 0.002 and p=0.004) and TNF . (p= 0.002 and p=0.002) levels in lung tissue were significantly increased in the control groups compared to those of the colitis groups. TNF-. level in colon tissue was found to be higher than that of the control group (p=0.002). Conclusion: In the lungs of rats with IBH, the features of alveolar hemorrhage are observed in contrast to control group. This histological feature is associated with an increase in the levels of VEGF and TNF-. in the lung tissues. Based on these findings, it is considered that TNF-. plays an important role in systemic involvements of IBD as a proinflammatory cytokine; and since VEGF increase in the lung accompanies alveolar hemorrhage, there is a pathological process which starts with alveolar hemorrhage induced by alveolar permeability increase and can result in pulmonary hemosiderosis and fibrosis.
