Cerrahi travmanın tetiklediği stres yanıt, patolojik bir süreç olarak kabul edilmektedir. Olguların perioperatif devreyi daha güvenli geçirmesinde stres yanıtın baskılanması önem kazanmaktadır. Çalışmamız Süleyman Demirel Üniversitesi Tıp Fakültesi Anesteziyoloji ve Reanimasyon Anabilim dalında majör alt ekstremite operasyonu planlanan 43 ASA I-III olgu üzerinde yapıldı. Çalışmada epidural anestezi ile iki farklı lokal anestetik ajanın hemodinamik yanıt ve stres hormonları üzerine etkisinin araştırılması planlandı. Bu amaçla standart non invaziv monitorizasyon uygulanan olgulara girişimden önce tercihen sol el dorsalinden 18 gauge intravenöz kateter takılarak 500 ml dengeli elektrolit solüsyonuyla prehidrasyon yapıldı. Olgulara oturur pozisyonda L4-L5 aralıktan median yaklaşımla 18 G Tuohy epidural iğne ile direnç kaybı yöntemiyle epidural alana girilerek kateter yerleştirildi. Bu aşamadan sonra olgular randomize olarak iki ayrı gruba ayrılarak Grup L (n=22).deki olgulara %0,5 15 mL levobupivakain + 5 mL serum fizyolojik, Grup B(n=21).deki olgulara %0,5 15 mL bupivakain + 5 mL serum fizyolojik epidural aralıktan uygulandı. Cerrahi travmaya bağlı stres yanıtı değerlendirmek amacıyla ameliyat salonuna gelişte (preoperatif), cerrahi insizyondan yarım saat sonra (peroperatif) ve operasyon sonrası 24. saatte (postoperatif) plazmada GH, sT3, TSH, kortizol düzeyleri bakıldı, kontrol değerleriyle ölçüm yapılan değerler karşılaştırıldı. Her iki grupta epidural girişimin hemodinamik stabilite üzerine yaptığı etkileri değerlendirmek amacıyla epidural girişim öncesi, ameliyat süresince ve sonrasında belirli periyotlarla SAB, DAB, OAB ve KAH düzeyleri değerlendirildi. Grup L olgularda GH değerinde postoperative 24. saatte kontrol değerlerine ve peroperatif değere göre anlamlı artış (p<0,001, p=0,007); St3 değerinde ise postoperatif 24. saatte kontrol değerine ve peroperatif değere göre anlamlı azalma (p<0,001, p<0,001) tespit edildi. Grup B olgularda ise GH değerinde postoperatif 24. saatte kontrol değerlerine ve peroperatif değere gore anlamlı artış (p=0,003, p=0,023); St3 değerinde ise postoperatif 24. saatte kontrol değerine ve peroperatif değere gore anlamlı azalma (p<0,001, p=0,002) tespit edildi. Hemodinamik parametreler her iki grupta da operasyon esnasında genellikle stabil seyretti. Çalışmamızın sonucunda alt ekstremite majör ameliyatlarında cerrahi travmaya bağlı stres yanıtı en aza indirgemek açısından araştırdığımız iki farklı lokal anestetik ajanın hemodinamik parametreler ve stres yanıtın baskılanması açısından aralarında farklılık olmadığı kanaatine vardık.

Stress response that surgical trauma induced is accepted as a pathological process. By that mean, attenuating stress response for a safe perioperative period gains importance. Our study was performed in Suleyman Demirel University School of Medicine Department of Anaesthesiology and Reanimation in 43 ASA I-III cases that lower limb surgery has been planned. Investigating the effects of two different local anaesthetic agents on hemodynamic response and stress hormones.was planned. For this aim, standart non-invasive monitorization was performed on cases and prehidration with 500 ml balanced electrolyte solution was administered by inserting 18 G intravenous catheter on the dorsal side of left hand. At the sitting position epidural catheter was put through L4-L5 space with median approach by using 18 G Tuohy needle with resistance loss technique. Cases were divided randomly into two groups after then this step. 0,5% 15 mL levobupivacain + 5 mL saline was administered in Group L (n=22) cases and 0,5% 15 mL bupivacain + 5 mL saline was administered through epidural space in Grup B cases. For evaluating stres response due to surgical trauma; plasma GH, sT3, TSH, cortizol levels were investigated at the arriving to the operation room (preoperative), 30 minutes after surgical insicion (peroperative) and at 24. hour after the operation (postoperative) and compared with control values. For evaluating the effect of epidural procedure on hemodynamic stability in both groups; SAP(systolic arterial pressure), DAP(diastolic arterial pressure) MAP(mean arterial pressure) and HR (heart rate) were recorded before and immediately after epidural procedure and with intermittan periods during the operation. In Group L; significant increase in GH in postoperative 24. hour according to control and peroperative values (p<0,001, p=0,007) was observed; significant decrease in St3 in postoperative 24. hour according to control and peroperative values ( p<0,001, p<0,001) was observed also. In Group B; significant increase in GH in postoperative 24. hour according to control and peroperative values (p=0,003, p=0,023) and significant decrease in St3 in postoperative 24. hour according to control and peroperative values (p<0,001, p=0,002) was observed. In both of the groups hemodynamic parameters were stabil generally during the operations. At the end of our study; we concluded that the two different local anaesthetic agents which we have searched for minimizing the stress response due to surgical trauma have no difference between each other respect to attenuation of stress response and hemodynamic parameters.
