Kolanjiokarsinoma (KK) safra yolları epitelinden köken alan, agresif bir kanser türüdür. Günümüzde yeni tanısal tekniklerin ortaya çıkmasına rağmen KK tanısını koymak ve cerrahi öncesi değerlendirmesini yapmak güçtür. Tanı ve preoperatif değerlendirmede Transabdominal Ultrasonogafi (USG), Bilgisayarlı Tomografi (BT), Endoskopik Ultrasonografi (EUS), Magnetik Rezonans Koloanjiopankreatikografi (MRKP) ve Endoskopik Retrograd Kolanjiopankreatikografi (ERKP) en çok tercih edilen yöntemlerdir.EUS safra yollarını değerlendirmek için en uygunyöntemlerden biridir. Ancak literatürde KK'nın EUS bulguları net olarak tanımlanmamıştır. Çalışmamızın amacı, KK tanısı konulan hastalarımızda retrospektif olarak EUS görüntülerinin değerlendirilerek BT ve ERKP görüntüleri ile karşılaştırılması ve KK.nın endosonografik bulgularını ortaya koymaktır. Materyal ve Metod Çalışmamıza Süleyman Demirel Üniversitesi Tıp Fakültesi (SDÜTF) Gastroenteroloji Bilim Dalı Endosonografi ünitesinde Ocak 2008 - Ocak 2011 tarihleri arasında EUS uygulanan 1524 hasta arasından KK tanısı konmuş 35 hasta dahil edilmiştir. Klinik ve laboratuvar sonuçları ile KK tanısı konmuş hastaların BT, EUS, ERKP görüntüleri ile varsa EUS İİAB, operasyon notları ve postoperatif patoloji raporları değerlendirilmiştir. Ana safra kanalının intrapankreatik kesimindelokalize lezyonlar distal KK, bu kesimin proksimalindeki lezyonlar proksimal KK olarak sınıflandırılmıştır. Bulgular Çalışmaya dahil edilen 35 olgunun 22 tanesinde BT görüntülemesi mevcut olup 10.unda (% 45.4) kitle tespit edilmişti. Bu 10 olgunun 5 tanesinde distal hipodens kitle lezyonu, 5 tanesinde ise proksimal hipodens kitle lezyonu tariflenmişti. Bu olguların EUS görüntülemelerinde de aynı lokalizasyonda kitle tespit edildi. ERKP uygulanan 27 olgunun tümünün ERKP ve EUS incelemesinde tümör lokalizasyonu açısından farklılık saptanmadı. EUS.da tespit edilen lezyonların 33 tanesi (% 95) hipoekoik, 2 tanesi izoekoik eko paterninde idi. 15 olguda kitle sınırları belirgin ve düzenli iken, 20 olguda sınırları düzensiz ve belirsiz kitleler görülmüştür. Bu bulgular ışığında; 1. Düzensiz sınırlı, hipoekoik, safra yolları ile ilişkisi net olarak gösterilemeyen, 2. Düzenli sınırlı, hipoekoik, safra kanallarıyle ilişkisi gösterilebilen ve bazı olgularda lümene polipoid çıkıntı yapan veya ana safra kanalında diffüz, fusiform kalınlaşma şeklinde görülen 2 patern tespit edilmiştir. Sonuç Yaptığımız retrospektif çalışmada KK olgularımızın tümünde EUS ile lezyon tespit edilirken, BT ile olgularımızın %55.inde lezyon görüntülenememiştir. ERKP yapılan olgularda, tanı ve tümör lokalizasyonu açısından bir olgu dışında EUS 50 bulguları ile ERKP bulguları tam bir uyum göstermiştir. EUS bulguları incelendiğinde KK tanısı için temel olarak 2 farklı görünüm paterni elde edilmiştir. Sonuç olarak, KK olgularında 2 farklı EUS paterni olabileceği gözönünde tutulmalı ve tanıda BT, ERKP ve EUS birlikte kullanılmalıdır.

Cholangiocarcinoma (CC) is an agrresive cancer arising from epithelium of bile ducts. Despite recent developments in diagnostic methods, preoperative diagnosis and evaluation of CC is difficult. Abdominal ultrasonography (USG), computed tomography (CT), endoscopic ultrasonography (EUS), magnetic resonance cholangiopancreatography (MRCP) and endoscopic retrograd cholangiopancreatography (ERCP) are the most preferred methods for the diagnosis and preoperative evaluation of CC. Although EUS is one of the most suitable methods to evaluate the biliary tract, however, EUS features of CC have not been clearly described in the literature. The aim of our study is to determine the EUS features of CC, and to compare the EUS findings with the CT and ERCP findings in patients with CC. Material and Method Totally 1524 patients have undergone EUS examination during the period of two years between January 2008 and January 2011 at the Division of Gastroenterology of Suleyman Demirel University Faculty of Medicine. Among these patients, thirty-five patients with CC included the study. CT, EUS, ERCP images and if available results of EUS FNA, operation notes and postoperative pathology reports of patients diagnosed as CC are evaluated. Lesions localized in the intrapancreatic portion of common bile duct are classified as distal CC, whereas lesions localized in the proximal part of common bile duct are classified as proximal CC. Results CT images are available in 22 of 35 patients. A mass lesion was detected in 10 of the 22 patients (45,4%). Adistal mass lesion was demonstrated in 5 of these 10 patients, while CT revealed a proximal mass lesion in 5 patients. EUS showed a mass lesion in the same part of bile tract of these patients. ERCP and EUS findings were concordant in terms of localisation of tumor, in 27 patients in whom ERCP was performed. EUS showed an hypoechoic mass lesion in 33 (95%) patients. and an isoechoic mass lesion in the two patients. The margin and contour of the mass lesions were regular and well-defined in the 15 patients, whereas were irregular and illdefined in the 20 patients. Taking into consideration these features, we described two pattern of CC on the EUS; 1. An hypoechoic lesion with irregular and ill-defined border, of which relation to bile ducts is not clearly demonstrated. 2. An hypoechoic lesion with regular and well-defined border, of which relation to bile ducts is clearly demonstrated. In some cases, the mass lesion can be seen as polypoid lesion in the lumen or as a diffuse and fusiform thickening of common bile duct. 52 Conclusion In this retrospective study, EUS revealed a mass in all of the patients with CC. However, the mass could not be demonstrated with CT in 55% of the cases. EUS and ERCP findings were concordant in terms of diagnosis and localisation of tumor, except for one patient. Two ultrasonographic pattern for the diagnosis of CC were determined in EUS. In conclusion, it should be noted that, CC may be presented with two distinct view on the EUS examination. CT, ERCP and EUS findings should be considered together for the diagnosis of CC.
