MnSOD gen polimorfizmi ile böbrek hastalıkları arasındaki ilişki birçok çalışmada gösterilmiştir. Bu çalışmada idiyopatik nefrotik sendrom (İNS) hastalarında MnSOD A/V genotip dağılımı ve genotip ile hastalığın klinik seyri arasındaki ilişkiyi değerlendirmek amaçlanmıştır. Çalışmaya 12 steroide dirençli nefrotik sendrom (SDNS), 51 steroide yanıtlı nefrotik sendrom (SYNS) hastası ve 72 sağlıklı kontrol olgu alındı. SDNS olgular hastalığın şiddetine göre kötü ve iyi seyirli, SYNS olguları ise çok ve az relaps yapanlar şeklinde alt gruplara ayrıldı. İstatistiksel karşılaşmada ki-kare, Kruskal- Wallis, Mann-Whitney-U ve t testleri kullanıldı. Çalışmamızda İNS olguları arasında en sık görülen genotip MnSOD VV homozigot olup kontrol grubu ile arasında anlamlı bir fark saptanmadı (%44.e karşı %31, OR: 1,7, p>0,05). Erkek ve kız İNS olgularında VV genotip sıklığı (%52,5 karşı %30,4) farklı değildi (OR: 3,9, p>0,05). Ancak V allel sıklığı İNS erkeklerde kız olgulardan (%70.e karşı %52) yüksek saptandı (p<0,05). Erkek İNS olgularında VV genotip oranı, kontrol grubundaki erkeklere göre yine yüksek bulundu (%52.e karşı %27, p<0,05). SDNS ve kontrol grubu olgular arasında VV genotip sıklığı açısından anlamlı bir fark yoktu (OR: 2,9, p>0,05). Kontrol grubuna göre kötü seyirli SDNS hastalarında VV genotip sıklığındaki fark anlamlık düzeyine yakındı (% 32.e karşı %80, OR: 8,52, p>0,05). SYNS ve kontrol grubu arasında MnSOD genotip dağılımı ve allel sıklığı açısından anlamlı fark saptanmadı. Çok ve az relaps yapan SYNS gruplarında VV genotip sıklığı benzer bulundu. Sonuç olarak İNS olguları ve MnSOD A/V gen polimorfizmi arasında doğrudan bir ilişki bulunmamaktadır. Fakat erkek İNS olgularda ve kötü seyirli nefrotik sendromlarda VV genotip etkili olabilir. Yapılacak daha ileri çalışmalar ile sonuçların karşılaştırılması uygun olacaktır.

The relation between MnSOD gene polymorphism and renal diseases has been shown by many studies. In this study, our aim was evaluating MnSOD A/V genotypes of idiopathic nephrotic syndrome (INS) and the relationship of genotype with clinical prognosis. Twelve steroid resistant nephrotic syndrome (SRNS) and 51 steroid sensitive nephrotic syndrome (SSNS) patients, in addition to 72 healthy control cases was included. SDNS cases were separated into 2 subgroups: well prognosed and bad prognosed; SYNS cases were also separated into 2 subgroups: unfrequent relapsing and frequent relapsing. For statistical analyses, chi- square, Kruskal-Wallis, Mann- Whitney-U and t tests are used. In our study, the most common genotype in INS cases was MnSOD VV homozygote and no significant difference was established with the control group (44 %to 31%, OR: 1,7, p>0,05). VV genotype frequency (52,5% to 30,4%) was not different in male and female cases of INS groups (OR: 3,9, p>0,05). However V allele frequency was higher in the male than the female INS patients (70% to 52%, p<0,05). VV genoype was more frequent in male INS group than control males (52% to 27%, p<0,05). There is was no significant difference of VV genotype frequency between SRNS and control group cases (OR: 2,9, p>0,05). VV genotype frequency difference between control group and bad prognosed SRNS patients was slightly under the significance (32% to 80%, OR: 8,52, p>0,05). For MnSOD genotype ratio and allele frequency o SSNS and control groups no significant difference was established. VV genotype frequencies of frequent and unfrequent relapsing SSNS subgroups were similar. As result, there is not any direct relation between INS cases and MnSOD A/V gene polymorphism. But VV genotype may be effective on male INS cases and bad prognosed nephrotic syndrome patients. It is needed to compare the results of further studies.
