Nonsteroidal antiinflamatuvar ilaçlar (NSAİİ) etkilerini prostaglandin sentezini inhibe ederek gösterirler. Prostaglandinlerin sentezlerime yollarından biri de siklooksijenaz (COX) enzimidir. İki tip izoenzim bulunmuştur. (COX-1, COX-2 olmak üzere) NSAİİ'lerin analjezik ve antiinflamatuvar etkilerinden COX-2 inhibisyonu sorumlu iken, istenmeyen yan etkilerinden COX-1 inhibasyonu sorumludur. Piroksikam içeren preperatm sıçan karaciğer, böbrek ve akciğer dokuları üzerine farklı dozlara bağlı olası etkilerini araştırmak amacıyla her grupta 5'er sıçan olmak üzere 3 grup oluşturuldu. Kontrol (Grup I), Piroksikam içeren preperat 0.250 mg/kg/gün (Grup II) , Piroksikam içeren preperat 0.500 mg/kg/gün (Grup III). Planlanan dozlar 5 gün süre ile intramüsküler uygulandı. Deney sonunda Grup Fe ait sıçanların karaciğer, böbrek ve akciğer dokularında normal histolojik yapılar gözlendi. Grup II karaciğer dokusunda hepatositlerde granüler dejenerasyon, bazı hepatositlerde piknotik çekirdekler, sinüzoidal konjesyon, portal alanda mononüklear hücre infiltrasyonları izlendi. Grup IIFte bu yapısal değişiklikler daha ciddi düzeyde izlendi. Aynı zamanda, bazı hepatositlerin nekroza gittiği görüldü. Grup II böbrek dokusunda vasküler konjesyon, perivasküler mononüklear hücre infiltrasyonları ve atrofik glomeruller izlendi. Grup III'te bu bulgulara ilaveten proksimal ve distal tübül epitel hücrelerinde hidropik dejenerasyon gözlendi. Grup II akciğer dokusunda interalveolar septumda kalınlaşma, vasküler konjesyon, alveoler konjesyon, parankimde mononüklear hücre infiltrasyonları, bronşiol duvarında düz kas liflerinde artma gözlendi. Grup IIFteki sıçanların akciğerinde bu bulgular daha belirgin şekilde izlendi. Piroksikam preperatı uygulanan sıçanların karaciğer, böbrek ve akciğer dokularında doza bağlı olarak belirgin yapısal değişiklikler meydana geldi. Proksikam içeren preparatm söz konusu dokular üzerine hasar verici etkilerinin sadece proksikama mı yoksa, ilacın içindeki diğer maddelere (etanol, propilen glikol, vs.) mi bağlı olduğu sorusu bu sahada yapılacak daha ileri seviyedeki çalışmaları gerektirmektedir. Nonsteroidal antiinflammatory drugs (NSAID) show their effect via inhibiting prostaglandin synthesis. Cyclooxygenase (COX) enzyme involves in prostaglandin synthesis pathways. There are two isoenzymes of COX: COX- 1 and COX- 2. While COX- 2 inhibition is responsible for analgesic and antiinflammatory effects of NSAIDs, COX- 1 inhibition accounts for unsolicited side effects. In order to investigate the possible dose-dependent effects of drug with piroxicam content on adult liver, kidney and lung tissues, three groups each consisting of five animals have been constituted. These were control (Group I), 0.250 mg / kg / day (Group II) and 0.500 mg / kg / day (Group III) drug given groups. The planned doses were administered intramuscularly for five days. At the end of the study, the liver, kidney and lung tissues of Group I were normally observed. Granular degeneration and piknotic nuclei in hepatocytes, sinusoidal congestion and mononuclear cell infiltration in portal fields were observed in the liver tissues of Group II. However, the level of these structural changes was more severe in Group III. Also, it has been observed that some hepatocytes have undergone necrotic process. Vascular congestion, perivascular mononuclear cell infiltrations and atropic glomeruls were present in kidney tissues of Group II. In addition to these findings, there was hydropic degeneration in proximal and distal tubules of Group III. Interalveolar septum thickening, vascular and alveolar congestions, parenchymal mononuclear cell infiltrations and smooth muscle fibril increment in bronchiole walls were observed in lung tissues of Group II. These findings were more evident in lung tissues of Group III. Also, overall structural changes in Group III were more prominent than Group II. The question that whether the destructive effects of drug with piroxicam content on rat liver, kidney and lung tissues are related to only piroxicam or other drug contents (ethanol, propylene glycol, etc) warrants further studies in this field.
