Akustik travma kısa süreli yüksek şiddette sese maruz kalma sonucu ortaya çıkan geçici veya kalıcı işitme kaybı ile sonuçlanabilen bir durum olup gelişmiş ülkelerde isitme kayıplarının ana nedenlerindendir. Bugün bu konuda birçok tedavi ve profilaksi yöntemleri-ajanları denenmiş olsa da; etkili bir tedavi modalitesi bulunamamıstır. Bu nedenle akustik travmaya bağlı kalıcı kayıpların önlenmesine ve tedavisine yönelik çalışmalar sürmektedir.Çalışmamızda TNF alfa reseptör blokörü olan etanercept ve deksametazonun akustik travmaya bağlı işitme kayıplarındaki proflaktik etkisi arastırıldı. Çalışmaya dahil edilen 30 rat üç eşit gruba ayrıldı. Öncelikle kontrol otoakustik emisyonları yapılarak ratların kohlear fonksiyonlarının normal olduğu teyyid edildi. Birinci grup kontrol grubu, ikinci grup etanercept grubu, üçüncü grup deksametazon grubu olarak belirlendi. Birinci gruba 1 ml serum fizyolojik ip, ikinci gruba 2,5 mg etanercept (Enbrel®) ip., üçüncü gruba 1 mg/kg deksametazon (Dekort®) ip. uygulandı.Otoakustik emisyon ölçümleri yapılan tüm ratlar ses geçirmez kabin içerisinde AC 40 model odyometri cihazı (Interacoustics Co. Assens, Danimarka) kullanılarak 40 dakika süre ile, 4 kHz frekansında 120 dB saf ton sese maruz bırakıldı. Deney hayvanlarının 24. saatte, 3. günde ve 7. günde distorsiyon ürünü otoakustik emisyonları (DPOAE) ölçüldü. Çalısmamızda etanercept grubu kontrol grubuna göre 1. günde 3,4,6 kHz de istatistiksel olarak akustik travmadan anlamlı koruyucu iken, 8 kHz de korumamıstır. Bu durum 3. gün yapılan emisyon kontrollerinde bütün frekanslarda istatistiksel olarak anlamlı koruyuculukta çıkmıştır. Yedinci gün kontrollerimizde 4,6,8 kHz de istatistiksel olarak anlamlı koruyuculuğu devam ederken, 3 kHz de koruyucu bulunmamıştır. Deksametazon grubu kontrol grubuyla karşılaştırıldığında; 1. günde 4 ve 6 kHz de istatistiksel olarak akustik travmadan anlamlı olarak korur iken, 3 ve 8 kHz de korumamıştır. Bu durum 3. gün yapılan emisyon kontrollerinde bütün frekanslarda istatistiksel olarak anlamlı koruyuculukta çıkmıştır. Yedinci gün kontrollerimizde 4 ve 6 kHz de istatistiksel olarak anlamlı koruyuculuğu devam ederken, 3 ve 8 kHz de koruyucu bulunmamıştır. Etanercept ve deksametazon grupları karşılaştırıldığında ise bütün günlerde ve bütün frekanslarda istatistiksel olarak anlamlı bir fark saptanmamıştır. Etanercept ve deksametazon aynı kaskadın farklı aşamalarından etki etmekle beraber, akustik travmadan koruyucu özellikleri büyük benzerlik göstermektedir. Her ikisi de en etkin koruyucu özelliklerini en çok etkilenen işitme aralığında 4-6 kHz de göstermektedir. Etanerceptin iç kulakta kullanımı ile ilgili çok sınırlı sayıda çalışma olması ve çalışmamızda akustik travmada ki koruyucu etkisinin gösterilmesi bu konudaki çalışmaların artmasına katkıda bulunacağını umudumuzu güçlendirmektedir.

Acoustic trauma is a situation resulting from short term high intensity sound exposure and may results in either temporary or permanent hearing loss. It is also one of the main causes of hearing losses in the developed countries. Though many treatments and prevention methods-agents on this topic are now a days tried, an effective treatment modality couldn’t be found. Therefore, studies for the prevention and treatment based on the permanent losses due to acoustic trauma are heging on.The prophylactic effects of etanercept, that is a TNF alpha receptor bloker, and dexamethasone on the acoustic trauma were investigated. Thirty rats included in this study were equally divided into three groups. Initially, cochlear functions of the rats were proved to be normal by measuring otoacustic emmissions. The first, second and third groups were determined as control, etanercept and dexamethasone groups respectively. The first, second and third groups were intraperitoneally administered 1 mL isotonik saline, 2,5 mg etanercept (Enbrel®) and 1 mg/kg dexamethasone (Dekort®), respectively. All the rats with normal otoacoustic measurement results were exposed to pure tone sound with the 4 kHz frequency and 120 dB intensity by an audiometry device of AC 40 model (Interacoustics Co. Assens, Denmark) within a sound proof cabin. Distortion product otoacoustic emissions (DPOAE) of experimental animals were measured at 24th hour, 3rd and 7th days. In our study, etanercept group was found to be protected from acoustic trauma in 3, 4, 6 kHz frequencies but not from in 8 kHz frequency at 1st day measurement. However, third day emission measurement resulted in statistically significant protectivity in all frequencies. Our seventh day inspections demonstrated statistically significant protectivity of etanercept at 4, 6, 8 kHz frequencies, while not protective at 3 kHz.When the dexamethasone group was compared to control group, dexamethasone was found to have statistically significant protective function against acoustic trauma in first day, at 4 and 6 kHz, but not at 3 and 8 kHz frequencies. Statistically significant protectivity was obtained in third day emissions at all frequencies. In our seventh day of inspection, statistically significant protection was continous at 4 and 6 khz, but not at 3 and 8 kHz frequencies. When etanercept and dexamethasone groups were compared to each other, statistically significant difference was determined at all frequencies in all days. Although etanercept and dexamethasone have effects on different levels of the same cascade, their protective properties against the acoustic trauma have great similarities. Both of them demonstrate their protective effects at range of 4-6 kHz frequencies, which are the most effected hearing range. Limited number of studies related with the use of etanercept in the inner ear and the experience of protective effect of it on the acoustic trauma strengthens our hope for the increase in the number of studies about this issue.
