Bu çalışmanın amacı bir spesifik protein kinaz A inhibitörü olan H89’un topikal intranazal uygulamasının, nazal mukozadaki Aquaporin 5 dağılımını inhibe ederek alerjik rinitte görülen tipik semptomları ve enflamatuar hücre infiltrasyonunu azaltabileceğini göstermektir. H89’un ve çözücü olarak kullandığımız Dimethyl sulfoxide (DMSO)’in alerjik rinit tedavisindeki etkisini araştırarak tedavide alternatif bir seçenek olup olamayacağını değerlendirmektir. Bu çalışmada, ağırlıkları 250-350 gr. arasında değişen 32 adet beyaz Wistor albino cinsi yetişkin erkek rat çalışmaya dahil edildi. Hayvanlar her grupta 8 rat olmak üzere 4 gruba ayrıldı. 1. gruba hiçbir şey yapılmadı (Kontrol grubu). 2. grup chicken serum albumin ile sensitize edildi ve 2 hafta süreyle DMSO topikal intranazal olarak verildi. 3. Grup Ovalbumin ile sensitize edildi ve başka ilaç verilmedi. 4.grup Ovalbumin ile sensitize edildi ve H89 topikal intranazal yedi gün süreyle verildi.Deney sonrası sıçan konkaları blok olarak çıkarılarak Hematoksilen- Eozin ve AQP5 antikorları ile boyanarak histopatolojik ve immünohistokimyasal olarak değerlendirilerek skorlandı.H89 grubunda silya kaybı, goblet hücre artışı, vasküler konjesyon, eozinofil infiltrasyonu, mukozal bezlerde dejenerasyon alerjik rinit grubuna göre istatistiksel olarak anlamlı derecede düşük tespit edildi (p=0.015, p=0.003, p=0.049, p=0.013, p=0.026). Bağ doku artışında, vasküler proliferasyonda ve enflamatuar hücre infiltrasyonu ise istatistiksel olarak fark bulunmadı (p=0.469, p=0.215, p=0.736).Ancak sadece DMSO verilen sham grubunda bu düzelmelerin vasküler konjesyon haricinde daha fazla olduğu tespit edildi. H89 grubundaki histolojik düzelmelerin çözücü olarak kullandığımız DMSO’ya bağlı olabileceğini düşündük.İmmünohistokimyasal değerlenlendirmede H89 grubunda AQP5 boyamalarında alerjik rinit grubuna kıyasla hafif (+) boyamalar tespit edildi. H89 grubu alerjik rinit grubuyla karşılaştırıldığında AQP5 dağılımının istatistiksel olarak anlamlı olarak azaldığı (p=0.001) tespit edildi. Sham grubunda ise AQP5 seviyesinde alerjik rinit grubuna göre anlamlı azalma tespit edilmedi (p=1.000).Sonuç olarak; H89, AQP5 seviyesini azaltan ancak mukozadaki alerjik bulgular üzerinde çok etkili bir madde olmadığını düşündük. Çözücü olarak kullandığımız DMSO’nun AQP5 seviyesine etki etmediği ancak nazal inflamasyonu azalttığını tespit ettik. Ancak hayvanlarda daha kapsamlı ve ultrastrüktürel çalışmalar ve sonrasında insan çalışmalarının yapılmasına gerek duyulmaktadır.

The aim of this study was to determine whether topical intranasal administration of H89 which is a specific protein kinase A inhibitor could relieve the typical symptoms of allergic rhinitis and attenuate the inflamatuar cell infiltration by inhibition of AQP5 expression. Thus, to elucidate the functions of H89 in allergic rhinitis and eveluate whether H89 and DMSO-we used as the solvent- could be an alternative treatment in allergic rhinitis.In this study, 32 white Wistor albino adult male rats which the weights ranging 250-350 gram were used. Animals divided into 4 groups that each group contains 8 animals. In the first group, no drug was administered (control group). In the second group, sensitization was completed by chicken serum albumin and DMSO (Dimethyl sulfoxide) was administered topical intranasal for 2 weeks. In the third group, sensitization was completed by OVA and no other drug was administered. In the fourth group, sensitization was completed by OVA and after this H89 was administered for 7 days.After the experiment rat conchas were dissected out as block and all the sections were evaluated as histopathologically immunohistochemically by stained with Hematoxylen-Eosine and AQP5 anticors.In group H89 statistically significantly gave rise to lesser inflammatory cells accumulation, ciliary loss of epithelial cells, goblet cell hyperplasia, vascular congestion, eosinophile infiltration, degeneration of mucosal glands compared to allergic rhinitis group (p=0.015, p=0.003, p=0.049, p=0.013, p=0.026). There was no statistically significantly quantity of goblet cells, vascular proliferation, severity of inflammatory cell infiltration (p=0.469, p=0.215, p=0.736 ). However, in the sham group which was given only DMSO, these improvements were found more. DMSO is used as a solvent; may be due to improvements in H89 group thought that the histological.In group H89, examination by staining AQP5 immunohistochemically there was low staining (+) compared to allergic rhinitis. In group H89 statistically significantly gave rise to lesser AQP5 expression compared to allergic rhinitis group (p=0.001). In group sham there was no statistically significancy compared to allergic rhinitis group for AQP5 expression (p=1.000) In conclusion; we thought that H89 can give rise to lesser AQP5 expression but can not effect allergic mucosal signs. DMSO, the solution as we used solvent, can not effect AQP5 expression but DMSO can give rise to lesser nasal inflammation.However, human studies should be performed following more comprehensive and ultrastructural studies on animals.
