Bifosfonatlarla ilişkili çene osteonekrozu (BİÇO)'nun oral mukozadaki anjiogenezis ve yara iyileşmesini olumsuz etkilenmesi nedeniyle meydana geldiği düşünülmektedir. Bazik fibroblast büyüme faktörü (bFGF), anjiogenezis ve yara iyileşmesinde önemli görevlere sahiptir. Bu çalışmanın amaçlarından biri zoledronik asit (ZA)'in diş çekimi sonrası iyileşmiş yumuşak dokuda anjiogenezis, yara iyileşmesi, büyüme faktörleri ve sitokinler üzerindeki etkilerini araştırmaktır.Çalışmanın diğer bir amacı yumuşak doku iyileşmesi üzerinde olumlu etkileri olduğu bilinen bFGF’nin, ZA'nın olumsuz etkilerini tedavi etme konusundaki etkinliğini değerlendirmektir. Bu tezin en önemli amacı BİÇO’ya sebep olduğu düşünülen ZA’nın bu etkisini nasıl gerçekleştirdiğini araştırmaktır.Bu prospektif, rastgele çalışmada 12 haftalık Sprague Dawley sıçanlar kullanıldı. Sıçanlar öncelikle ZA uygulanan ve uygulanmayan olarak 32'şer adet sıçandan oluşan 2 gruba ayrıldıktan sonra da her iki grup kendi içinde bFGF'nin 4 dozuna (düşük 90 ng/kg, orta 450 ng/kg, yüksek 2275 ng/kg, uygulanmayan) uygun olarak 4 alt gruba ayrıdı. Böylece her grupta 8 sıçan olacak 8 grup oluşturuldu. ZA ve bFGF'nin anjiogenezis, yara iyileşmesi, büyüme faktörleri ve sitokinler üzerindeki etkileri histopatolojik ve immünohistokimyasal olarak değerlendirildi.İstatistik analizde Mann–Whitney U, Kruskal–Wallis ve Wilcoxon testleri kullanıldı.ZA anjiogenezis, yara iyileşmesi, büyüme faktörleri ve sitokinlerin skorlarını düşürürken bFGF, ZA'nın bu etkilerine olumlu etkilerde bulundu (p<0,05). Bununla birlikte ZA, serum bFGF seviyesini önemli miktarda düşürdü (p<0,05).ZA'nın anjiogenezis, yara iyileşmesini geciktirerek, büyüme faktörleri ve sitokinlerin salınımını azaltarak ve serum bFGF seviyesini düşürerek BİÇO tablosuna neden olduğu söylenebilir. Bisphosphonate- related osteonecrosis of the jaws (BRONJ) is thought to be associated with defects in angiogenesis and regeneration of the oral mucosa.Moreover, basic fibroblast growth factor (bFGF) participates in angiogenesis and wound healing. This preliminary study aimed at evaluating the effect of zoledronic acid on angiogenesis, secondary soft tissue healing at the tooth extraction area, growth factors and cytokines. Another aim of this study was evaluating the effect of bFGF against the negative effects of ZOL. On the other hand, We also intended to investigate the etiopathology of BRONJ which is belived to be associated with ZOL.This prospective, randomized study, 12-week-old Sprague–Dawley rats (n=64) were divided into two-groups as ZOL treated group (n=32) and non-treated group (n=32). These 2 groups were subdivided into 4 groups each (n =8 each): groups administered low (90ng/kg), medium (450 ng/kg), and high doses (2,275 ng/kg) of bFGF and non-treated control groups. The effects of ZOL and bFGF on soft tissue healing and angiogenesis following tooth extraction and the expressions of different growth factors-cytokines were examined histopathologically and immunohistochemically. Data were analyzed using the Mann–Whitney U, Kruskal-Wallis and Wilcoxon tests.ZOL decreased expressions of all growth factors and cytokines, angiogenesis and wound healing on biopsy specimens. Meanwhile, bFGF treatment significantly ameliorated the negative effects of ZOL (p<0,05). However, ZOL significantly decreased serum level of bFGF (p<0,05).It is hypothesized that ZOL causes BRONJ formation via decreasing serum bFGF level, secretion of growth factor and cytokines on healing area, and delaying angiogenesis and reganeration Keywords : angiogenesis, basic fibroblast growth factor, bisphosphonate related osteonecrosis of the jaws, growth factors, zoledronic acid.
