Çocuk yoğun bakım (ÇYB) üniteleri, haftada 7 gün, günde 24 saat kesintisiz hizmet veren ve multidisipliner yaklaşım gerektiren bir veya birden fazla organ yetersizliğinden dolayı ya da geçirdikleri cerrahi uygulamalardan ötürü yaşamsal bulguları tehdit altında olan çocukların bakım ve tedavilerinin yapıldığı ünitelerdir. Ülkemizde ÇYB ünite ve yatak sayısı yetersiz olup bu alanda yapılan çalısmalar da sınırlı sayıdadır. Biz bu çalısmada 01 Haziran 2009 ve 31 Mayıs 2012 tarihleri arasında ÇYB ünitemize yatan hastaları geriye dönük olarak inceleyerek, kliniğimizin verilerini ortaya koyup mortalite ve buna etki eden risk faktörlerini belirlemeyi amaçladık.Çalışmaya alınan 470 hastanın 235'si (%50) kız, 235'i (%51,5) erkek olarak tespit edildi. Yoğun bakıma en sık yatış nedeni (184 hasta, %39,1) zehirlemeler idi. Hastaların 146'sında (%31,1) yoğun bakım yatışları esnasında eşlik eden kronik hastalığı vardı. Mekanik ventilatör ihtiyacı olan hasta sayısı 59 (%12,6) ve hastaların ventilatörde kalış süresi ortanca değeri 3,0 gün (1-174) olarak tespit edildi. Çalışmaya alınan hastaların 25'sinde (%5,3) dissemine intravasküler koagülasyon, 26'sında (%5,5) çoklu organ yetmezliği, 75 hastada (%16) hiperglisemi, 9 hastada (%1,9) hipoglisemi, 27 hastada (%5,7) trombositopeni tespit edildi. Pozitif inotrop destek alan hasta sayısı 53 (%11,3) olup hastalardan 6'sı (%1,3) renal replasman tedavisi almıştı. Çocuk yoğun bakım ünitemizin mortalitesi %6,8 olarak tespit edildi. Yüksek PRISM skoru, hipoglisemi, trombositopeni varlığı, pozitif inotrop destek alma, MV ihtiyacı, DĠK, ÇOY, kronik hastalık varlığının mortaliteyi artırdığını tespit ettik. Mortaliteye etkili olarak bulunan parametreler lojistik regresyon analizi ile birlikte değerlendirildiğinde PRISM skoru ve DĠK ile mortalite arasında anlamlı bir ilişki olduğu görüldü.

Pediatric intensive care units (PICU) providing uninterrupted service with a multidisciplinary approach on a 24/7 basis are health care facilities dealing with the treatment and care of the children having life threatening conditions due to failure in one or more of the organ systems or having undergone a surgical procedure. There are an insufficient number of PICU and beds in Turkey, and there are limited studies performed on this field. The aim of the present study was to retrospectively review patients hospitalized in the PICU between June 2009 and May 2012 and to present the data of the clinic and determine mortality rate and associated risk factors.Of 470 patients included in the study, 235 (50%) were girls and 235 (50%) were boys. The most common cause of hospitalization in the intensive care unit was intoxication (184 patients, 39.1%). Of the patients, 146 (31.1%) had concurrent chronic illness on admission to intensive care unit, and 59 patients (12.6%) required mechanical ventilator support, and median mechanical ventilation time was 3.0 days (1-174). Of these patients, 25 (5.3%) had disseminated intravascular coagulation, 26 (5.5%) had multiple organ failure, 75 (16%) had hyperglycemia, 9 (1.9%) had hypoglycemia, and 27 (5.7%) had thrombocytopenia. Of patients, 53 (11.3%) received positive inotropic support and 6 patients (1.3%) received renal replacement therapy. The mortality rate was 6.8% in the pediatric intensive care unit. High PRISM score, hypoglycemia, thrombocytopenia, positive inotropic support, need for MV, DIC, MOF, and presence of chronic diseases were found to be associated with increased mortality rate. The parameters that exhibited an effect on the mortality were further analyzed using logistic regression analysis in which the PRISM score and DIC appeared as factors significantly associated with mortality.
