Biz bu çalışmada; alt eksterimite periferik arter hastalıklarını saptamada, BTA tetkikini referans olarak alıp, kontrastsız bir 3D FSE yöntemi olan NATIVE SPACE MRA yönteminin kullanılabilirliğini ve tanısal gücünü araştırmayı amaçladık.Çalışmamıza Haziran 2014 – Ekim 2014 tarihleri arasında Süleyman Demirel Üniversitesi Tıp Fakültesi Hastanesi Radyoloji Bölümüne PAH ön tanısı veya tanısı ile yönlendirilen 19'u (%82,6) erkek, 4'ü (%17,4) kadın, yaşları 21-86 arasında değişen toplam 23 hasta dahil edildi. Arteriyel vasküler yapılar toplam 23 segmente ayrılarak BTA ve NATIVE SPACE MRA ile değerlendirme yapıldı. Kontrastsız MRA sekansının her bir segmentindeki görüntü kalitesi, değerlendirme yapılabilme açısından 5 gruba ayrıldı. Sonrasında diagnostik kabul edilen segmentler de darlık derecesi açısından sınıflandırıldı. Referans olarak kabul ettiğimiz BTA ile karşılaştırıldığında NATIVE SPACE kontrastsız MRA tetkikinin sensitivite, spesifite, pozitif prediktif değer (PPD), negatif prediktif değer (NPD) ve doğruluk oranları hesaplandı.Hem BTA hem MRA tetkiki uygulanan ve değerlendirmeye alınan toplam 529 segmentin 469'unda uygun görüntü elde edildi. Görüntü kalitesi okuyucular tarafından sırasıyla 469 segmentin 310'unda ( %66,1) ve 320'sinde ( %68,2) tanısal olarak değerlendirildi. Darlık derecesi açısından ise toplamda okuyucu 1 tarafından 325 segment, okuyucu 2 tarafından 335 segment incelendi. BTA'nın referans olarak alındığı ve NATIVE SPACE sekans MRA'nın genel tanısal performansının ölçüldüğü veriler %95 güvenirlilik aralığı (CI) ile beraber; okuyucu 1'de sensitivite %85,7 (72,7-94,0), spesifite %85,5 (80,8-89,4), tanısal doğruluk %85,5 (76,4-93,8)iken; okuyucu 2'de sensitivite % 66,7 (57,9-74,6), spesifite %93,1 (88,7-96,2), tanısal doğruluk %82,7 (70,2-91,2) olarak hesaplandı. Ayrıca okuyucular arası performansa dayalı ölçümlerde ise genel olarak sensitivite %76,2, spesifite %89,3, tanısal doğruluk %84,1, pozitif prediktif değer %68,8 ve negatif prediktif değer %89,1 olarak bulundu.Sonuç olarak; çalışmamızda negatif prediktif değerin yüksek bulunması PAH'tan şüphelenilen genç hastalar ile ateroskleroz riskinin yüksek olduğu ve kontrast madde kullanımının kontraendike olduğu kronik böbrek yetmezliği olan hastalarda NATIVE SPACE'in ekartasyon amacıyla kullanılabileceğini göstermektedir. Yine NATIVE SPACE tetkikinin kontrastlı incelemeden önce kullanılabilecek bir ilk basamak görüntüleme yöntemi olabileceği kanaatindeyiz.

The purpose of this study was to evaluate the diagnostic performance and feasibility of unenhanced 3D FSE MR angiography sequence (NATIVE-SPACE MRA) in detection of lower limb peripheral arterial diseases and compare the results with computed tomography angiography (CTA) which is the referance standart.Patients with PAD diagnosis or preliminary diagnosis who attended to Suleyman Demirel University Faculty of Medicine Hospital Department of Radiology between June 2014 and October 2014 were enrolled to the study. 23 patients, 19 (82.6%) male, and 4 (17.4%) female with the age range between 21-86, were included to the study. Arterial vascular structures were divided into 23 segments and evaluated with NATIVE SPACE MRA and CTA. Unenhanced MRI image quality in each segment of the sequence were divided into 5 groups according to their diagnostic value. Segments classified as diagnostic were grouped according to the degree of stenosis. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy values of NATIVE SPACE were calculated and compared with CTA as the referance standard.Both CTA and MRA imaging were applied to total of 529 segments and appropriate image were obtained in 469 segments. Image quality were evaluated as diagnostic in 310 (66.1%) and 320 (68.2%) segments by reader 1 and 2 respectively.325 segments and 335 segments were investigated in terms of degree of stenosis by reader 1 and 2 respectively. Results of NATIVE SPACE sequence MRA with 95% confidence interval (CI) were: sensitivity 85,7% (72,7-94,0%), specificity 85,5% (80,8-89,4%), diagnostic accuracy 85,5% (76,4-93,8%) for the reader 1 and sensitivity 66,7% (57,9-74,6%), specificity 93,1% (88,7-96,2%), diagnostic accuracy 82,7%(70,2-91,2%) for the reader 2. Besides interreader performance based measurements were as follows: sensitivity 76,2%, specificity 89,3%, diagnostic accuracy 84,1%, positive predictive value 68,8% and negative predictive value 89,1%. As a result, high negative predictive value shows that NATIVE SPACE can be choice of imaging technique in young patients with the suspicion of PAD and patients with chronic renal failure who are at high risk of atherosclerotic disease in whom use of contrast agent is contraindicated because of increased risk of gadolinium induced nephrogenic systemic fibrosis for elimination. Also we believe in that NATIVE SPACE imaging can be used as a first step imaging technique before contrast enhanced imaging techinques.
