Akut karın ağrılı hasta acil serviste sık karşılaşılan hasta grubudur. Ayırıcı tanının yapılması çoğu zaman hayati öneme sahiptir. Kolay elde edilebilirlik ve yaygın kullanım imkanı akut karın ağrılı hastaların erken tanısında biyokimyasal markırları oldukça önemli hale getirmektedir. Bu çalışmada akut karın ağrısı ile AS'e başvurup yatışı yapılan hastalarda MPV'nin tanısal değerinin araştırılması planlandı.Çalışmaya 27.03.2013 tarihinde SDÜ Tıp Fakültesi Klinik Araştırmalar Etik Kurulunun 88 sayılı kararı ile onay alındıktan sonra 1 Ocak 2012 ile 1 Mart 2013 tarihleri arasında SDÜ acil servise başvuran toplam 857 olgu dahil edildi. Olguların 812'si hasta grubu olup, 45'i kontrol grubu idi. Çalışmadaki 812 hasta ile 45 sağlıklı kişinin MPV düzeyleri karşılaştırıldı. Arşiv taraması yaparak hasta grubundaki olguların; yaş, cinsiyet, ağrı başlangıcı ile AS'e başvurana kadar geçen süre, fizik muayene bulguları, tedavi şekli, çıkış durumu, 5'inci gün MPV, platelet, nötrofil lenfosit oranı, WBC, CRP ile MPV değerleri aralarındaki ilişki incelendi ve MPV'nin nontravmatik akut karın ağrılarındaki tanısal değerinin bulunup bulunmadığı araştırıldı.Başvuran hastaların cinsiyet dağılımı 467 erkek, 345 kadın idi. Erkek hastalar toplam hastaların %57,5'ni oluştururken kadın hastalar %42,5'ni oluşturmakta idi. Kontrol grubunun en genci 42 yaşında iken en yaşlısı 79 yaşında idi. Kontrol grubunun yaş ortalaması 58,2 idi. Kontrol grubunun %53,3'nü kadınlar, %46,7'sini erkekler oluşturmakta idi. Ağrı başlangıcı ile acil servise gelene kadar geçen süreye göre hastaların %38,5'i AS'e 24 saattir var olan karın ağrısı, %27,3'ü 48 saattir var olan karın ağrısı, %34,1'i 48 saatten daha uzun süredir var olan karın ağrısı nedeni ile başvurdu.AS'e başvuran karın ağrılı hastaların kesin tanıları incelendiğinde en sık rastlananlar sıklık sırasına göre safra kesesi ve koledok patolojileri %18,7, NSKA %17,3, ileus volvulus %13,8, akut pankreatit %11,7, apandisit %10,8 tespit edildi. Çalışmaya alınan 812 hastanın 5'inci gün MPV ortalama değeri 8,358±1,0460 fL, platelet değerleri ortalama 246,82±91,3 u/L, nötrofil lenfosit oranı ortalama değeri 10,025±12,75 u/L olarak bulundu. Çalışmadaki akut karın ağrılı hasta grubunda giriş MPV değerleri ortalama 7,869±1,0718 fL, sağlıklı kontrol grubu MPV değerleri ortalama 8,47±1,0588fL olup, hasta grubunun ortalama MPV düzeyi, sağlıklı bireylerden daha düşüktü. Ancak sadece akut pankreatit ve ileus volvulus tanısı alan hastalarda bu düşüklük istatistiksel olarak anlamlı bulundu.Bu çalışmadan elde ettiğimiz verilere göre; pankreatit ve ileus volvulus tanısı alan hastalarda arada kalınan durumlarda MPV'nin tanıya yardımcı bir markır olabileceğini düşünmekle birlikte, diğer akut karın ağrılı hastalarda serum MPV düzeylerini tanısal parametre olarak kullanmak uygun değildir. Bu hastalarda MPV'nin tanısal parametre olarak kullanılması konusunda geniş hasta grupları ile yapılacak prospektif çalışmalara ihtiyaç vardır.

Patients with acute abdominal pain is the most common patient group in the emergency department. Differential diagnosis if often vital. The biochemical parameters becomes significant with the opportunity of easy availability and widespread use in early diagnosis of patients with acute abdominal pain. In this study it has been planned to investigate the diagnostic value of MPV of patients that attend emergency department and become an inpatient with acute abdominal pain.The study after having a confirmation on 27.03.2013 from The Clinical Research Ethics Committee of the SDU Faculty of Medicine decision no.88 there has been 857 patients in total that attended to the emergency department of SDU between dates 01.01.2012-01.03.2014. 812 of the patients were the ill group whereas 45 were the control group. The MPV levels of 812 patient group with 45 healthy control group has been compared.By scanning archives of the patients; age, sex, pain onset and time passed until attending to the emergency department, physical examination findings, type of treatment, hospital discharge status, 5th day MPV, platelets, neutrophils, lymphocytes ratio, WBC, the relationship between CRP and MPV were examined and MPV’s nontraumatic acute abdominal pain was investigated for the presence of diagnostic value.The gender distribution of patients were 467 male and 347 female. Male patients formed 57,5% in total patients and female patients formed 42,5% . The youngest in control group was 42 years old and the oldest was 79 years old. The average age in the control group was 58,2. The 53,3% of the control group were formed by females and 46,7% were formed by males. From the time of pain onset and the time passed attending to the emergency department the 38,5% of patients had 24 hours of abdominal pain, 27,3% had 48 hours of abdominal pain and 34,1% had more than 48 hours of abdominal pain. After investigating the final diagnosis of the patients that attended to the emergency department with abdominal pain, the most common starting from the most frequent were found to be 18,7% gallbladder and bile duct, NSKA 17,3%, ileusvolvulus 13,8%, 11,7% acute pancreatitis and 10,8% appendicitis.812 patients in the study found to have an average value of 8,358±1,0460 fL MPV on the 5th day, average platelet value of 246,82±91,3 u/L, and 10,025±12,75 u/L of mean value of neutrophil-lymphocyte ratio.The first average MPV values of patients with acute abdominal pain in the study were 7,869±1,0718 fL, healthy control groups average MPV values were 8,47±1,0588fL, shows that the average MPV level were lower than the healthy group. However, this decrease was statistically significant only in patients diagnosed with acute pancreatitis and ileus volvulus. According to the data we have obtained from this study; patients presented with pancreatitis and volvulus, MPV may be a diagnostic co-marker when diagnosis is not certain but it is not suitable to use diagnostic parameters and serum MPV levels of patients with acute abdominal pain, large prospective studies are needed to use MPV diagnostic parameters on these patients.
