Platelet rich plazma (PRP) canlının kendi kanından elde edilen platelet içeriği bakımından yoğun otolog bir üründür. Plateletler akut yara iyileşmesinde yara yerine gelen ilk hücrelerdendir. Koagulasyon içerisindeki 10 dakikada plateletler içerdikleri granüllerden birçok protein salgılarlar. Bu proteinler yara iyileşmesi sürecinde rol alan hücrelere bağlanarak hücre proliferasyonu, matriks oluşumu, osteoid oluşumu ve kollajen sentezi gibi işlemleri düzenlerler ve yara iyileşmesinde etkin rol alırlar. Cilt, oral mukoza benzeri birçok bölgede PRP kullanılmış olup, literatürde nazal mukoza ile ilgili histopatolojik çalışmasına rastlanmamıştır.Biz bu çalışmamızda PRP’nin nazal mukozada yara iyileşmesi üzerine olan etkisini araştırıp, nazal mukozanın hasarlandığı durumlarda (septoplasti, endoskopik sinüs cerrahisi, epistaksis) mukozal yara iyileşmesinin daha hızlı ve kaliteli olmasını sağlayacak ve böylece komplikasyonları minimal seviyede tutabilecek bir tedavi yöntemi elde etmeyi amaçladık.Bu çalışmada ağırlıkları 2.8-3.5 kg (ortalama 3.2 kg) ağırlığında 32 adet erkek Yeni Zelanda beyaz tavşanı kullanıldı. Sağ nazal kavitelerine girilerek interdental fırça yardımı ile (brushing technique) septum, konka ve lateral nazal duvarlarında mukozal hasar oluşturuldu. Hayvanlar dört gruba ayrıldı. İlk iki grup birinci hafatada, ikinci iki grup ise üçüncü hafatada sakrifiye edildi. Histopatolojik inceleme için biyopsi örnekleri Hematoksilen-Eozin ve Masson-Trikrom boyaları ile boyandı. Histopatolojik skorlama sonuçlarına göre değerlendirildi.Histopatolojik skorlama sonuçlarına göre birinci haftada goblet hücre hiperplazisi, ödem, konjesyon, silya kaybı, fibrozis ve yassı epitel hücre metaplazisi parametreleri açısından iki grup arasında istatistiksel olarak anlamlı fark izlenmedi.Histopatolojik skorlama sonuçlarına göre birinci haftada inflamasyon şiddeti açısından istatistiksel olarak anlamlı fark saptandı (p=0.009). PRP inflamasyonu azalttı.Histopatolojik skorlama sonuçlarına göre üçüncü haftada goblet hücre hiperplazisi, ödem, konjesyon, silya kaybı, fibrozis, inflamasyon ve yassı epitel hücre metaplazisi parametreleri açısından iki grup arasında istatistiksel olarak anlamlı fark izlenmedi. Yukarıdaki sonuçlar göz önüne alındığında PRP’nin nazal mukoza hasarı sonrası akut dönemde inflamasyonu azaltarak yara iyileşme sürecini etkileyebileceği düşünülmüştür.

Platelet rich plasma (PRP) is defined as a volume of the plasma fraction of autologous blood with a platelet concentration above baseline. Platelets are among the first cells to respond a wound site, and are critical to the initiation of wound healing. Within 10 minutes of blood coagulation, platelets release a burst of proteins from their granules. The network of activated growth factors induces intracellular signaling pathways that lead to the production of proteins essential to the regenerative processes, such as cell proliferation, matrix formation, osteoid production and collagen synthesis. PRP has been studied lots of tissues such as skin, oral mucosa and muscle, except of nasal septal mucosa.In this study we plan to use PRP to treat nasal traumatic wounds. Our aim is to assess the effects of PRP on nasal mucosal wound healing. It is important for the situations, in which nasal mucosa gets hurt such as septoplasti, FESS, epistaxis. The question is “is the PRP can be used to heal the wounds faster and proper to decrease complications”. If it is, that provides us a simple and useful medical treatment option.In this study, 32 young adult male white New Zelland Rabits weighing between 2800-3500 gram were used. A unilateral wound (right) in the nasal mucosa was injured using the brushing technique. The animals were randomly divided into four groups. The first two grups were sakrificed in day seven (control, PRP). The second two groups were sacrificed in day 21th (control, PRP). Biopsy specimens were stained with hematoxylin-eosin and Masson-Trichrome. The results were evaluated according to the histopathological scoring results.According to the histopathological scale, the groups of the first week were compared and the results of goblet cell hyperplasia, , loss of cilia, fibrosis, squamous metaplasia, edema and congestion were not significantly different.According to the histopathological scale, on first week the results of inflamation were statistically significant (p=0.009).According to the histopathological scale, the groups of the third week were compared and the results of goblet cell hyperplasia, inflamation, loss of cilia, fibrosis, squamous metaplasia, edema and congestion were not significantly different. Considering the above results, it can be concluded that PRP could decrease the inflammation on the acute stage of wound healing and can effect nasal mucosal healing and increase the postoperative success.
