Bu çalışmada amaç mitomisin ile yapılan intravezikal termokemoterapinin normal mesane dokusu üzerine olan histopatolojik etkilerini tespit etmekti.Bu çalışmada 2,1-4,3 kg (ortalama 2,67 kg) ağırlığında 24 adet erkek beyaz Yeni Zelanda tavşanı kullanıldı. Hayvanlar 3 gruba ayrıldı. Her gruba intravezikal olmak üzere 1. gruba ısıtılmış SF, 2. gruba mitomisin, 3. gruba ısıtılmış mitomisin verildi. İşlemden 1 hafta sonra tüm hayvanlara sistektomi uygulanarak, hayvanlar sakrifiye edildi. Alınan mesane dokuları histopatolojik inceleme için Hematoksilen-Eozin ve Masson-Trikrom boyaları ile boyanarak histopatolojik skorlama sonuçlarına göre değerlendirildi.Histopatolojik olarak mesane dokularında epitelde vakuoler dejenerasyon, epitelde hiperplazi, bağ dokusunda hiperplazi ve bağ dokusunda hemorajik alanlar parametreleri ayrı ayrı değerlendirildi.Isıtılmış SF grubu, mitomisin grubu ile karşılaştırıldığında tüm histopatolojik parametreler açısından istatistiksel olarak anlamlı fark tespit edildi (p≤0,002).Isıtılmış SF grubu, ısıtılmış mitomisin grubu ile karşılaştırıldığında tüm histopatolojik parametreler istatistiksel olarak daha anlamlı tespit edildi (p≤0,001).Mitomisin grubu, ısıtılmış mitomisin grubu ile karşılaştırıldığında tüm histopatolojik parametreler açısından istatistiksel olarak anlamlı fark tespit edildi (p≤0,05).Bu sonuçlar göz önüne alındığında mitomisinin normal mesane dokusu üzerine histopatolojik olarak etkisi bulunmaktadır. Bu etkiler ısıtılmış mitomisin grubunda daha fazla saptanmıştır. Ürotelyal tümörlerin gelişiminde ilk aşamada epitelyal hiperplazi vardır. Bizim çalışmamızda da ısıtılmış mitomisin grubunda ısıtılmış SF grubuna göre epitelyal hiperplazi belirgin artmış olarak izlendi. Bu epitelyal hiperplazi kanser oluşum sürecinin ilk aşamasında rol alabilir. Ayrıca ısıtılmış mitomisin grubunda gözlenen diğer bulgular mikroskobik ya da makroskopik hematüri, kronik sistit, mesane kapasitesinde azalmaya sebep olabilir. Isıtılmış mitomisin verilecek hasta grubunun seçiminin netleştirilmesi için ileri klinik çalışmaların yapılmasına ihtiyaç vardır.

The aim of this study to identify histopathological effects of intravesical thermochemotherapy performed with mitomycin on to normal bladder tissue.In this study, 2.1 to 4.3 kg (mean 2.67 kg) in 24 male New Zealand white rabbits were used. Animals were divided into 3 groups. Each group intravesically 1. group was performed heated saline, 2. group was performed mitomycin, 3. group was performed heated mitomycin. 1 week after the procedure, all animals underwent cystectomy and were sacrificed. Obtained bladder tissues were stained with hematoxylin-eosin and Masson-Trichrome stain for histopathological examination and were evaluated according to histological scoring results.Vacuolar degeneration of the epithelium, hyperplasia of the epithelium, hyperplasia of connective tissue and hemorrhagic areas in the connective tissue parameters evaluated separately histopathologically in bladder tissues.Heated saline group compared with the mitomycin group and in all histopathological parameters were determined statistically significant differences (p≤0,002).

(p≤0,001).Mitomycin group compared with the heated mitomycin group in all histopathological parameters were determined statistically significant differences (p≤0,05).According to these results mitomycin has histopathological effects in normal bladder tissue. These effects were determined further in heated mitomycin group. The first stage of development of urothelial tumor is epithelial hyperplasia. İn our study, in heated mitomycin group epithelial hyperplasia was observed increased significantly according to heated saline group. This epithelial hyperplasia may take a role in the first stage of the formation process of cancer. In addition, other findings observed in the heated mitomycin group can cause microscopic or macroscopic hematuria, chronic cystitis, and decreased bladder capacity. İt is need to further clinical studies clarify the selection of patients will perform heated mitomycin.
