Giriş ve Amaç: Kronik Böbrek Hastalığı (KBH)'nda biyolojik yaşın artmış kardiyovasküler mortalite ile ilişkisinin moleküler mekanizması tam olarak bilinmemektedir. Son yıllarda yapılan çalışmalar bu fenomenden sorumlu olan ortak faktörlerin hücresel yaşlanma ve telomer disfonksiyonu olabileceği hipotezini desteklemiştir. Hemodiyaliz hastalarında periferik kan mononükleer hücreler (PBMC)'de telomeraz aktivitesinin düşük olduğu bildirilmiştir. Telomeraz aktivitesinin KBH evreleri ve KBH progresyonu ile ilişkisi bilinmemektedir. Bu çalışmanın amacı telomeraz aktivitesinin KBH evreleri ile ilişkisini araştırmaktır.Materyal ve Metod: Çalışmaya evre 2-5 KBH tanısıyla takip edilen hastalardan, her evreden 30'ar hasta olmak üzere toplam 120 hasta ve bilinen herhangi bir hastalığı olmayan, rutin kontrol amacıyla hastanemizde tetkiklerini yaptıran bireylerden gönüllü 30 sağlıklı birey seçildi. Hasta ve kontrol grubundaki bireyler yaş ve cinsiyet açısından eşleştirildi. PBMC telomeraz aktivitesi telomeric repeat amplification protocol yöntemi ile ölçüldü.Bulgular: PBMC telomeraz aktivitesi açısından gruplar arasında anlamlı fark görüldü (p < 0.001). Buna göre sağlıklı kontrol grubunda en düşük (0.15 ± 0.02), evre 5 KBH'da en yüksek (0.23 ± 0.04) seviyelerde idi. Evre 2, 3 ve 4 KBH‘da telomeraz aktiviteleri benzer idi (p > 0.05). KBH hastalarında telomeraz aktivitesi ile KBH evresi (r = 0.412, p < 0.001), serum kreatinin (r = 0.404, p < 0.001), potasyum (r = 0.207, p = 0.023), intact parathormon (r = 0.245, p = 0.007) düzeyi ile pozitif; tahmini glomerüler filtrasyon hızı (eGFR) (r = -0.407, p < 0.001), serum sodyum (r = -0.179, p = 0.05), kalsiyum (r = -0.357, p < 0.001), LDL-kolesterol (r = -0.218, p = 0.017), hemoglobin ( r = -0.186, p = 0.042), trombosit (r = -0.252, p = 0.006) düzeyi ve vücut kitle indeksi (BMI) (r = -0.240, p = 0.008) ile negatif korelasyon saptandı. Lineer regresyon analizinde, KBH grubunda eGFR ve BMI'nın yüksek telomeraz aktivitesinin bağımsız öngördürücüleri oldukları tespit edildi (Model 1: R² = 0.151, eGFR için ß = -0.001, P < 0.001. Model 2: R² = 0.195, eGFR için ß = -0.001, P = 0.010; BMI için ß = -0.002, p = 0.012).Sonuç: KBH'lı hastalarda PBMC telomeraz aktivitesi sağlıklı kontrol grubuna göre anlamlı yüksektir. KBH evresi ilerledikçe özellikle evre 5'e geçildiğinde PBMC telomeraz aktivitesi artmaktadır. KBH'da PBMC telomeraz aktivitesi artışı eGFR ve BMI ile ilişkilidir.
Introduction and Objectives: The molecular mechanism of the relationship between biological age and increased cardiovascular mortality in Chronic Kidney Disease (CKD) remains unclear. Recent studies substantiate the thesis that common factors responsible for this phenomenon might be cellular aging and telomere dysfunction. Telomerase activity in peripheral blood mononuclear cells (PBMC) was reported to be lower in hemodialysis patients. The relationship between telomerase activity and stages and progression of CKD is unknown. The aim of this study is to investigate the association between telomerase activity and stages of CKD.Material and Method: A total of 120 patients (30 patients from each stages of 2-5) and 30 healthy volunteers applying to the clinic for their routine examinations were enrolled in the study. The individuals from patient and control groups were matched in terms of age and gender. PBMC telomerase activity was measured by telomeric repeat amplification protocol.Results: PBMC telomerase activity was significantly different between the groups (p < 0.001). Telomerase activity was found to be lowest in healthy controls (0.15 ± 0.02), and highest in patients with stage 5 CKD (0.23 ± 0.04). Telomerase activity was similar between patients with stage 2, 3, and 4 CKD (p > 0.05). There was positive correlation between telomerase activity and CKD stage (r = 0.412, p < 0.001), serum creatinine (r = 0.404, p < 0.001), potassium (r = 0.207, p = 0.023), and intact parathyroid hormone (r = 0.245, p = 0.007) levels and negative correlation between telomerase activity and estimated glomerular filtration rate (eGFR) (r = -0.407, p < 0.001), serum sodium (r = -0.179, p = 0.05), calcium (r = -0.357, p < 0.001), LDL-cholesterol (r = -0.218, p = 0.017), hemoglobin ( r = -0.186, p = 0.042), trombocyte (r = -0.252, p = 0.006), and body mass index (BMI) (r = -0.240, p = 0.008) values. In linear regression analyses, eGFR and BMI were found to be independent predictors of high telomerase activity in CKD group. (Model 1: R² = 0.151, for eGFR ß = -0.001, P < 0.001. Model 2: R² = 0.195, for eGFR ß = -0.001, P = 0.010; for BMI ß = -0.002, p = 0.012).Conclusion: PBMC telomerase activity is significantly higher in patients with CKD compared to healthy controls. Telomerase activity increases as the CKD stage proceeds, particularly in stage 5. The increase in PBMC telomerase activity is associated with eGFR and BMI.
