Bu çalışmanın amacı, büyüme gelişim dönemindeki bireylerde mini plak ve dişlerden destek alan Forsus FRD EZ2 apareyinin iskeletsel, dentoalveoler ve yumuşak dokular üzerindeki etkilerinin değerlendirilmesi ve karşılaştırılmasıdır. Bu prospektif klinik çalışmada, bireyler iskeletsel ankraj Forsus ve konvansiyonel ankraj Forsus olmak üzere 2 ana gruba ayrılmıştır. Birinci grup mini plak ankrajı ile tedavi edilen 15 bireyden (ortalama yaş 13,06±1,21), ikinci grup dişsel ankraj ile tedavi edilen 15 bireyden (ortalama yaş 13,59±0,79) oluşmaktadır.Birinci grupta mini plaklar bilateral olarak simfiz bölgesine yerleştirilmiştir.Seviyeleme sonrasında, maksiller arkta 0,016x0,022 inç paslanmaz çelik arka geçildiğinde, mandibular ark braketlenmeden Forsus FRD EZ2 apareyi mini plağa uygulanmıştır. İkinci grupta, seviyeleme sonrasında her iki arka 0,016x0,022 inç paslanmaz çelik uygulanmış ve Forsus FRD EZ2 apareyi maksiller 1. moların headgear tüpü ile mandibular ark telinde kanin ile 1. premolar braketlerinin arasına uygulanmıştır. Tedavi etkilerini değerlendirmek için fonksiyonel aparey uygulanmadan önce (T1) ve aparey çıkarıldıktan sonra (T2) sefalometrik filmler alınmıştır. Lateral sefalometrik filmler kullanılarak, T1 ve T2 dönemleri arasındaki değişiklikler eşleştirilmiş-t testi ve Student t-testi ile değerlendirilmiştir.Araştırmamızın sonuçlarına göre, her iki grupta da mandibular büyüme stimüle edilmiş, maksiller büyüme inhibe edilmiştir. Efektif mandibular uzunluk önemli miktarda artmıştır. Maksiller molarların distalizasyonu ve intrüzyonu, maksiller keserlerin ekstrüzyonu ve retrüzyonu her iki gruptada önemli miktarlardadır. Konvansiyonel Forsus grubunda mandibular molarların mesializasyonu ve ekstrüzyonu, mandibular keserlerin labiale devrildiği görülürken, iskeletsel ankraj Forsus grubunda mandibular molarların ekstrüzyonu, mandibular keserlerin retrüzyonu görülmüştür. Her iki grupta da overbite ve overjet azalırken, yumuşak doku profilinde iyileşme görülmüştür. Her iki grupta da büyüme gelişim döneminde mandibular retrognati kaynaklı Sınıf II maloklüzyonun tedavisinde iskeletsel, dentoalveoler ve yumuşak doku değişimlerinin kombinasyonu ile düzelme elde edilmiştir. Fakat iskeletsel ankraj Forsus metodunun, mandibular dentisyon üzerinde herhangi bir dentoalveoler yan etkiye sahip olmaması sebebiyle avantajlıolduğu düşünülmektedir. The aim of this study is to evaluate and compare the skeletal, dentoalveolar and soft tissue effects of the Forsus FRD EZ2 with miniplate and dental anchorage for the treatment of skeletal Class II malocclusion in growing individuals.The individuals included in the prospective clinical study were divided into two main groups as skeletal anchorage Forsus and conventional anchorage Forsus groups. The first group consisted of 15 patients (mean age 13,06 ± 1,21) who were treated with miniplate anchorage, the second group consisted of 15 patients (mean age 13,59 ± 0,79) who were treated with dental anchorage. In the first group, two miniplates were placed bilaterally on the mandibular symphysis. After the leveling phase, 0.016x0.022 inch stainless steel archwire was inserted in the maxillary arch and consequently, the Forsus FRD EZ2 appliance was adjusted to the miniplates without leveling the mandibular arch. In the second group, after the leveling phase 0.016 × 0.022 inch stainless steel arch wires were engaged in both arches, and Forsus FRD EZ2 appliance attached to headgear tubes of maxillary first molars and to the mandibular archwire, between the canine and the first premolar brackets. Lateral cephalograms were taken before functional appliance placement [T1] and after functional appliance removal [T2] to evaluate treatment effects. The changes between T1 and T2 phases were evaluated by means of the Paired-t test and Student’s t-test using the lateral cephalometric films.According to our results, stimulation of mandibular growth and inhibition of maxillar growth were achieved in both of the treatment groups. Effective mandibular length increased significantly in both groups. Retrusion and extrusion of maxillary incisors, distalization and intrusion of maxillary molars were significant in both treatment groups. As extrusion and mesialization of mandibular molars and labial tipping of mandibular incisors were observed in conventional Forsus group, extrusion of mandibular molar and retrusion of mandibular incisors were observed in skeletal anchorage Forsus group. Overbite and overjet were reduced and soft tissue profile improved in both of treatment groups. In the growing individuals with class II malocclusions with mandibular retrognathia improvements were obtained via a combination of skeletal, dental, and soft tissue changes. In conclusion skeletal anchoraged Forsus method is believed to be more advantageous as it has no dentoalveolar side effects on mandibular dentition.
