Daimi birinci molarlar, ilk süren diş olmaları sebebiyle özellikle ağız hijyeni yeterli olmayan hastalarda genellikle ilk kaybedilen diş grubunu oluştururlar. Bu tezde, 1. molar diş eksikliği nedeniyle posterior sınırlı dişsiz boşluğu olan hastalarda, bu boşluğa komşu posterior dişlerdeki supra erupsiyon, devrilme, rotasyon, bukkolingual yer değiştirme ve dişsiz boşluğun kapanma miktarının ve tipinin belirlenmesi amaçlanmaktadır. Molar diş eksikliği olan 50 erişkin katılımcı çalışma grubuna, diş eksikliği bulunmayan 50 erişkin katılımcı da kontrol grubuna dahil edildi. Hastaların alt ve üst çenelerinden ölçü alındı ve alınan ölçülerden alçı modeller elde edildi. Hasta modelleri üç boyutlu dijital görüntülerin oluşturulması amacıyla üç boyutlu dental tarayıcıda taratıldı. Üç boyutlu dijital görüntüler üzerinde, dişsiz boşluğa doğru karşıt dişte görülen supra erupsiyon miktarı ve tipi, komşu dişlerde görülen rotasyon ve devrilme dereceleri, bukkolingual yerdeğiştirme ve dişsiz boşluğun kapanma miktarı ölçüldü.Çalışma grubunda antagonisti olmayan posterior dişlerin supra erupsiyon miktarı ortalaması 1,60 mm, kontrol grubunda ise 0,57 mm olarak ölçüldü. Bu sonuç istatistiksel olarak anlamlı olduğu bulundu (p<0,01). Supra erupsiyon miktarı istatistiksel olarak karşıt dişi olmayan maksillada mandibuladan daha fazla bulundu (p<0,01). Çalışma grubundaki ikinci premolar dişlerde ölçülen ortalama devrilme miktarı 6,16°, ikinci molar dişlerde ölçülen ortalama devrilme miktarı ise 16,42° olarak tespit edildi. Kontrol grubunda ölçülen devrilme miktarı ortalaması 2. premolar dişlerde 1,20°, 2. molar dişlerde ise 1,78° olarak ölçüldü. Hem 2. premolar hem de 2. molar dişlerden elde edilen devrilme miktarlarının çalışma ve kontrol grupları arasındaki farkı istatisiksel olarak anlamlı bulundu (p<0,01). Çalışma grubunda ikinci premolar dişlerde ölçülen ortalama rotasyon derecesi 15,82°, kontrol grubunda ise 1,47° olarak ölçüldü. Bu iki grup arasındaki fark istatistiksel açıdan anlamlı bulundu (p<0,01). Çalışma ve kontrol gruplarında ölçülen bukkolingual yer değiştirme miktarı kıyaslandığında istatistiksel olarak anlamlı bir fark bulunamadı (p>0,01). Dişsiz boşluğun kapanma miktarı ortalaması 2,67 mm olarak ölçüldü. Sonuç olarak denilebilir ki, diş çekimi sonrası dişsiz boşluk protetik olarak restore edilmezse dişsiz boşluğun antagonistindeki ve komşuluğundaki dişlerde vertikal ve horizontal pozisyon değişiklikleri meydana gelmektedir. Bu pozisyon değişikliklerinin önlenmesi için protetik restorasyon gereksinimi konusunda hastalara mutlaka çekim sonrası bilgi verilmelidir. The aim of study was to asses the extent and type of supraeruption, tipping, rotation, buccolingual displacement and space closure of posterior teeth adjacent to the posterior edentulousspace in patients with posterior bounded edentulous space resulting from the first permanent molar loss. Fifty patients with first permanent molar loss were included into the study group and 50 patients with full dentition were included into the control group. Upper and lower jaw impressions were taken and cast models were acquired. Patient models were scanned with a three dimensional dental scanner in order to produce 3D images. Using the acquired 3D digital images, the extent and the type of supraeruption of opposing teeth towards the edentulous space rotation and tipping degrees of the nonadjacent teeth, the extent of buccolingual displacement and space closure were measured. The mean value of supraeruption of unopposed posterior teeth in the study group was measured as 1.60 mm while this was 0.57 mm in the control group. The difference between the two groups was found to be statistically significant (p<0.01). Supraeruption values of unopposed teeth were found to be higher in the maxillae than in the mandible and this difference was statistically significant (p<0.01). In the study group the mean tipping values of the second premolars and the second molars were 6.16° and 16.42° respectively. In the control group, the mean tipping degrees of the second premolars and the second molars were 1.20° and 1.78° respectively. In terms of tipping, the difference between the study group and control group both in second premolars and molars were statistically significant (p<0.01). The mean rotation values of the second premolar teeth in the study group was 15.82° while in the control group this was 1.47°. The difference between the two groups was statistically significant (p<0.01). In terms of the buccolingual displacement values measured there was not a statistically significant difference between the study and control groups (p>0.01). The average amount of space closure was measured as 2.67 mm. As a conclusion, it can be said that if an edentulous space is left unrestorated prosthetically following tooth extraction, vertical and horizontal positional changes take place in the adjacent and antagonist teeth. After tooth extraction, patients should be informed about the necessity of prosthetic restoration in order to prevent such positional changes.
