Bu çalışmanın amacı, pik pubertal dönemdeki maksiller retrognatiye sahip İskeletsel Sınıf III anomalili bireylerde, Alt-RAMEC uygulamasıyla birlikte kemik destekli intermaksiller elastik kullanımının iskeletsel, dental ve yumuşak dokulardaki etkilerinin incelenmesidir.Araştırmaya dahil edilen maksiller retrognatisi bulunan 34 İskeletsel Sınıf III anomalili hasta, 17 bireyden oluşan iki gruba ayrıldı. Hareketli aparey grubunda (yaş ortalaması 12,13±0,58; 8 erkek, 9 kız), mandibuler lateral ve kanin diş arasına mini plak, maksiller ikinci premolar ile birinci molar arasına mini implant (1,6x10 mm) yerleştirildi. Üst çenede oklüzyonu kapalı hareketli bir plak takıldı ve mini implantlar ile mini plaklar arasına 225 gr kuvvette Sınıf III lastik uygulandı. Alt-RAMEC grubunda (yaş ortalaması 12,11±0,59; 9 erkek 8 kız), ortasında Hyrax vida bulunan oklüzyonu kapalı bonded bir apareyle 9 haftalık Alt-RAMEC protokolü uygulandı.Protokol sonunda genişletme apareyi çıkarılarak, aynı seansta oklüzyonu kapalı hareketli aparey yerleştirildi. İskeletsel ankraj üniteleri Hareketli aparey grubuyla aynı şekilde yerleştirilerek 225 gr kuvvette Sınıf III lastik uygulandı. Hareketli aparey grubunda tedavi süresi 1,23±0,13 yıl, Alt-RAMEC grubunda 1,007±0,15 yıldır. Her iki gruptan da tedavi başında ve sonunda sefalometrik filmler alındı.Sefalometrik ölçümlerde grup içi değişimlerin değerlendirilmesinde eşleştirilmiş t testi, gruplar arası farkların değerlendirilmesinde ise bağımsız örneklem t testi kullanıldı.Bütün hastalarda anterior çapraz kapanış düzelmiş ve yumuşak doku profili iyileşmiştir. Tedavi sonunda her iki grupta da, SNA°, ANB°, A-y, 1-NA (mm, (°)),1-NB (mm,(°)), Sn/GoGn°, Co-A, Co-Gn, (FH┴N)-A değerlerinde anlamlı miktarda artış gözlenirken, SNB° ve (FH┴N)-Pg (mm) değerlerinde anlamlı derecede azalma görülmüştür ve gruplar arasında istatistiksel olarak benzer bulunmuştur. Aylık maksiller protraksiyon oranı Alt-RAMEC grubunda, Hareketli aparey grubuna göre istatistiksel olarak önemli miktarda fazla bulunmuştur. Ü1-NA (mm) ve Ü1/PP° değerlerindeki artış ise, Hareketli aparey grubunda anlamlı miktarda daha fazla bulunmuştur. Her iki grupta da fonksiyonel orta hat sapması anlamlı derecede düzelmiştir. Kız ve erkek bireyler ayrı ayrı incelendiğinde, kız bireylerde SNB° değerindeki azalmanın ve ANB° değerindeki artışın, erkeklere göre önemli derecede fazla olduğu görülmüştür.Bu çalışmada, Alt-RAMEC uygulamasıyla birlikte kemik destekli intermaksiller elastik kullanımının, yalnızca kemik destekli intermaksiller elastik kullanımına göre tedavi etkileri, üst keser konumu dışında benzer bulunmuştur.Bununla birlikte, Alt-RAMEC uygulamasıyla daha kısa sürede pozitif overjet sağlanmıştır. The aim of this study is to investigate the skeletal, dental and soft tissue effects of Alt-RAMEC protocol combined with bone-anchored intermaxillary elastics in the Skeletal Class III patients with retrognathic maxilla in the peakpubertal period.34 Skeletal Class III patients with retrognathic maxilla included in the study were divided into two groups. In the Removable appliance group (mean age: 12,13± 0,58; 8 boys, 9 girls), two mini plates were inserted between mandibular lateral and canine bilaterally and two mini implants (1,6x10 mm) were inserted between maxillary second premolar and first molar. A removable and occlusally closed plate was used in the maxilla. The mini plates and mini implants were loaded with Class III intermaxillary elastics with 225 gr force per side. In the Alt-RAMEC group (mean age: 12,11±0,59; 9 boys 8 girls) 9 weeks of Alt-RAMEC protocol was performed with an occlusally closed bonded appliance with a Hyrax screw. At the end of the protocol, a removable and occlusally closed plate was used instead of the expansion appliance. The skeletal anchorage units were inserted in the same way with the Removable appliance group and Class III elastics with 225 gr force were applied.Total treatment time was 1,23±0,13 and 1,007±0,15 years in the Removable appliance and Al-RAMEC groups, respectively. Lateral cephalograms were taken in the beginning and at the end of the treatment. Intragroup changes were assessed with paired-t tests and intergroup differences were assessed with independent sample tests.Anterior crossbite was eliminated and soft tissue profile was improved in all patients. There were statistically significant increases in the SNA°, ANB°, A-y, 1-NA (mm, (°)), 1-NB (mm,(°)), Sn/GoGn°, Co-A, Co-Gn, (FH┴N)-A variables in both groups while SNB° angle and (FH┴N)-Pg were decreased significantly. These changes were found to be statistically similar between groups. Maxillary protraction rate was significantly more in the Alt-RAMEC group compared with the Removable appliance group. The increases in the U1-NA (mm) and U1/PP° were found to be significantly more in the Removable appliance group. Functional deviation of the midline was reduced significantly in both groups. The decrease in the SNB° angle and the increase in the ANB° in girls were found to be significantly more compared with the boys in the study. The effects of Alt-RAMEC combined with bone-anchored intermaxillary elastics were found to be similar with the usage of bone-anchored intermaxillary elastics only, except the upper incisor position in this study. However, positive overjet was achieved in a shorter time with the Alt-RAMEC application.
