Oral ve Maksillofasial Cerrahide kemik defektlerinin rekonstrüksiyonu yaygın kullanım alanına sahiptir. Son yıllarda, çeşitli biyomateryallerin tek başına veya greft materyalleri ile beraber kemik iyileşmesini artırma kapasiteleri yoğun araştırma konusu olmuştur. Bu çalışmanın amacı, hyalüronik asit ve/veya kemik greftinin sıçan mandibulasında oluşturulan defektlerde kemik iyileşmesi üzerine olan etkinliğinin değerlendirilmesidir.Prospektif ve randomize tasarlanan çalışmamıza 40 adet 12 haftalık 450-550 gram ağırlığında Sprague-Dawley cinsi erkek sıçan dahil edildi. Deney hayvanları her grupta 10 sıçan olacak şekilde 4 gruba ayrıldı. Genel anestezi altında, ekstraoral yaklaşımla mandibula lateral yüzüne ulaşım sağlanması ardından, trefin frez yardımıyla ramusta 5 mm çapında bikortikal defekt oluşturuldu. Grup 1’de yer alan deneklerde oluşturulan defektler boş bırakıldı. Grup 2’deki deneklerde defektlere sadece kemik grefti; Grup 3’dekilere sadece hyalüronik asit; Grup 4’dekilere ise kemik grefti ve hyalüronik asit karışımı yerleştirildi. Doku katmanları süturlanarak bölge primer olarak kapatıldı. Her bir denekte uygulanan işlem karşı taraf mandibulada tekrar edildi. Postoperatif 6. haftanın sonunda hayvanlara ötenazi uygulandı. Mandibula sağ yarılarından alınan örnekler boyanarak mikroskobik düzeyde kemik iyileşmesi açısından ve osteopontin, osteriks ve alkalen fosfataz yoğunluğu açısından incelendi. Mikroskopik kemik iyileşmesi 7’li skala ile immünokimyasal boyanma yoğunlukları 4’lü skala ile ölçümlendirildi. Sol yarıdan alınan örneklerin kemik mineralizasyonu mikrobilgisayarlı tomografi yardımıyla ölçüldü. Veriler Kruskal-Wallis ve Bunferroni-Dunn testleri ile analiz edildi.Boş bırakılan defektlerde histolojik düzeyde kemik iyileşmesi en yüksek değeri 7 olan skalaya göre 1,25 iken sadece greft ve sadece hyalüronik asit yerleştirilen gruplarda 4,65; greft+hyalüronik asit yerleştirilen grupta ise 5,60 olarak ölçüldü. Boş bırakılan defektlerdeki iyileşme biyomateryal uygulanan defektlerdeki iyileşmeden anlamlı olarak düşüktü (p<0.05); ancak biyomateryal kullanılan gruplar arasındaki fark anlamlı düzeyde değildi.Her bir gruptaki örneklerin osteopontin, osteriks ve alkalin fosfataz belirteçleri için boyanma yoğunlukları orta şiddette pozitif/şiddetli pozitif düzeyde olup gruplar arasında anlamlı fark olmadığı görüldü. Örneklerde mineralizasyonu ölçümlendiren YGA değerleri en yüksek hyalüronik asit uygulanan (tek başına veya greft ile beraber) gruplara ait idi; diğer gruplarla aradaki fark istatiksel olarak anlamlıydı (p<0.05). İstatiksel olarak anlamlı olarak en yüksek DGA değeri ise sadece hyalüronik asit kullanılan grupta izlendi (p<0.05).Sonuç olarak, hyalüronik asit tek başına veya kemik grefti ile beraber kullanımında çene kemiklerinde oluşan defektlerin iyileşmesine pozitif yönde katkı sağlamaktadır. In recent years, several biomaterials, alone or in combination with bone grafts, have been the focus of research for their capacity to enhance bone healing. The aim of this study was to evaluate efficacy of hyaluronic acid with or without bone graft on the healing of bony defects created in the rat mandible.In our prospective and randomized study, 40 Sprague- Dawley breed, 12-week old, 450-550 gram-weigh male rats were included. Animals were divided into 4 groups having 10 rats. Under general anesthesia, following access to the lateral surface of the mandible through extra-oral approach, a 5 mm-diameter bicortical defect was created using a trephine drill. The defects in animals in Group 1 was left empty. Bone graft was placed into defects in animals in Group 2, hyaluronic acid was placed into those in Group 3 and graft and hyaluronic acid mixture was placed into those in Group 4. The wound was primarily closed by suturing tissues in layers.The same procedure was repeated on the other side for each animal. The animals were sacrificed at the end of postoperative 6-week. Samples taken from the right side of the mandible were stained and examined for microscopic bone healing and for the expression of osteopontin, osterix and alkaline phosphatase. Microscopic bone healing was scored using 7-point scale whereas intensity of immunohistochemical staining was scored using 4-point scale. Bone mineralization of samples from the left side was quantified using microcomputed tomography. Data were analyzed by Kruskal–Wallis and Bunnferroni-Dunn tests.Using the scale in which the highest score is 7, healing in defects that was left empty had a score of 1,25 while those received either of the biomaterials alone had a score of 4,65 and those received both bone graft and hyaluronic acid had a score of 5,60. Healing of the defect that remained empty was significantly poorer than those in each of the groups that received biomaterials (p<0.05); but no significant differences found between those received biomaterials either alone or in combination. Each group had a mean score of moderately positive/ severely positive intensity of staining for osteopontin, osterix and alkaline phosphatase, no difference between groups were obtained. Highest HGA for mineralization was achieved in those groups that received hyaluronic acid (alone or in combination with graft), the difference with the other two groups was significant (p<0.05). Significantly highest LGA was achieved in the group that received hyaluronic acid alone (p<0.05).In conclusion, hyaluronic acid either alone or in combination with bone graft, positively contributes to healing of bone defects in jaws.
