Periodontitis dental biyofilmdeki mikroorganizmalara yanıt olarak ortaya çıkan kronik enflamatuvar bir durumdur. T hücrelerinin periodontitis patogenezindeki rolleri iyi tanımlanmıştır. Th17 proenflamatuvar sitokini IL-17 aracılığıyla periodontitiste patojenik bir rol üstlenmektedir. Bu nedenle Th17 hücre yanıtının nasıl kontrol edilebileceğini anlamak periodontal hastalık kontrolünde yardımcı olacaktır. Kurkumin antienflamatuvar ve antioksidan etki gibi çeşitli biyolojik aktivitelere sahip bitkisel kaynaklı bir fitoterapötiktir. Kurkuminin tedavi edici özellikleri romatoid artrit, obezite gibi çeşitli durumlarda araştırılmıştır ancak periodontal hastalıkta Th17 hücre farklılaşması ve yanıtı üzerindeki etkilerini değerlendiren bir çalışma bulunmamaktadır. Bu çalışmada deneysel periodontitis modelinde sistemik kurkumin uygulamasının lokal ve sistemik Th17 yanıtı ile alveoler kemik yıkımı üzerindeki etkisi değerlendirildi.Çalışmada 38 adet erkek Wistar rat 4 gruba ayrıldı. Grup 1: periodontitisin indüklendiği ratlar (n=10); Grup 2: periodontitisin indüklendiği, kurkumin uygulanan ratlar (n=10); Grup 3: periodontal olarak sağlıklı, kurkumin uygulanan ratlar (n=10);Grup 4: kontrol (n=8). Kurkuminin oral gavaj yoluyla uygulanmasına (30 mg/kg/gün) periodontitisin indüklenmesinden 1 gün önce başlandı ve 15 gün süresince devam edildi. Ratlar sakrifiye edildikten sonra mandibulaları morfometrik, histolojik ve immünohistokimyasal analizler için kullanıldı. Serum IL-1β, IL-6, IL-17, IL-23 ve TGF-β düzeyleri ELISA ile değerlendirildi.Biyokimyasal parametreler IL-23 haricinde gruplar arasında anlamlı farklılık göstermedi. En yüksek IL-23 düzeyleri Grup 2‟de bulundu (p<0,0125). IL-17 ve RORγt dişeti konsantrasyonları Grup 1‟de Grup 2‟den anlamlı olarak daha fazla bulundu (p<0,0125). Alveoler kemik kaybı Grup 1'de diğer gruplardan anlamlı olarak daha fazla bulundu (p<0,0125).Kurkumin deneysel periodontitis modelinde alveoler kemik kaybını IL-17 ve RORγt ekspresyonunu baskılayarak azaltabilir. Periodontitis is a chronic inflammatory condition initiated by an immune response to microorganisms in the dental biofilm. The role of T cells in the pathogenesis of periodontitis is well recognized. Th17, through its proinflammatory cytokine IL-17 plays a pathogenic role in periodontitis. Therefore, understanding how Th17 cell response can be modulated may help in the management of periodontal disease. Curcumin is a plant-derived phytotherapeutic with various biological activities including anti-inflammatory and antioxidant effects. Its therapeutic applications have been studied in a variety of conditions including rheumatoid arthritis, obesity but the effects of curcumin on the differentiation and response of Th17 cells in periodontal disease have not been evaluated yet. In this study, the effect of systemic curcumin application on local and systemic Th17 response and alveolar bone loss (ABL) were investigated in an experimental periodontitis model.38 male Wistar rats were used in the study and divided into four groups: Group 1: rats with periodontitis (n=10); Group 2: rats with periodontitis, application of curcumin (n=10); Group 3: periodontally healthy rats, application of curcumin (n=10); Group 4: controls (n=8). The administration of curcumin via oral gavage (30 mg/kg/day) was started 1 day before the induction of periodontitis and continued for 15 days. After rats were sacrificed their mandibles were collected for morphometric, histological and immunohistochemical analysis. ELISA was used to determine the serum levels of IL-1β, IL-6, IL-17, IL-23 and TGF-β.The biochemical parameters were not found statistically different between groups, except IL-23 (p<0.0125). The highest value of IL-23 was found in Group 2 (p<0.0125). IL-17 and RORγt gingiva concentrations were significantly higher in Group 1 than Group 2 (p<0.0125). Alveolar bone loss was significantly higher in Group 1 than the other three groups (p<0.0125).Curcumin may decrease alveolar bone loss in experimental periodontitis model via suppressing the expression of IL-17 and RORγt.
