Amaç: Miyofasiyal ağrı sendromu (MAS) tedavisinde kuru iğneleme ve lazer tedavisinin etkinliğinin karşılaştırılmasıdır.Method: Çalışmada, üst trapez kasındaki tetik nokta veya noktalara bağlı MAS’ı olan 69 hasta değerlendirilmiştir. 1 hasta gebelik nedeniyle çalışma dışı bırakılmış, 7 kişi tedaviyi bırakıp çalışmayı tamamlayamamıştır. Hastalar randomize olarak iki gruba ayrılmıştır. Kuru iğneleme grubundan 31 kişi, lazer grubundan 30 kişi çalışmayı tamamlamıştır. Kuru iğneleme grubunda her bir tetik nokta, 22 gauge (1,5 inch) siyah enjektör ucu ile yaklaşık 10 dk süreyle iğnelenmiştir. Bu yöntem haftada 2 kez, 2 hafta süreyle toplam 4 seans uygulanmıştır. Lazer grubunda 808 nm (sürekli) - 905 nm (kesikli) dalga boyu, 18 V-50 VA güç çıkışı olan 'Mphi, Laser therapy MLS' marka, galyum-arsenid (GaAs) düşük yoğunluklu lazer aleti kullanılmıştır. Her bir tetik noktaya 900 Hz frekansında, transforaminal olarak 12 noktaya, 5 dk süreyle, 4.64 J/cm² yoğunlukta, total doz 174.720 J lazer tedavisi, haftada 5 gün 2 hafta süreyle toplam 10 seans uygulanmıştır. Tüm hastalara trapez kası germe egzersizlerini içeren ev programı verilmiştir. Değerlendirmede ağrı şiddeti için; vizüel analog skala (VAS) ve basınç ağrı eşiği (Pressure Pain Threshold – PPT), fonksiyonel durum için; boyun ağrısı ve dizabilite skalası (Neck Pain and Disability Scale-NPADS), duygu durumu için; Beck depresyon ölçeği (BDÖ) ve Beck anksiyete ölçeği (BAÖ) kullanılmıştır.Bulgular: Her iki grupta da, tüm parametrelerde tedavi sonrasında istatistiksel olarak anlamlı düzeyde iyileşme saptanmıştır. Gruplar arası karşılaştırmada, VAS, PPT ve NPADS değerlerinde kuru iğneleme grubu lehine istatistiksel olarak anlamlı farklılık saptanmıştır (p<0,05).Sonuç: Kuru iğneleme lazer tedavisine göre daha etkin olmakla birlikte her iki tedavi seçeneği de MAS’da yararlı bulunmuştur. Bununla birlikte iğne korkusu olan hastalarda ve kuru iğneleme tekniğinde deneyimsiz sağlık çalışanları mevcudiyeti durumunda, düşük doz lazer tedavisi kuru iğneleme tedavisine iyi bir alternatif olarak gözükmektedir.

Purpose: The aim of this study is to compare the effectiveness of the dry needling and laser therapy in the treatment of myofascial pain syndrome (MPS).Methods: In this study, 69 patients who have MPS associated with trigger point(s) in the upper trapesius muscle are evaluated. Totally 8 patients were out of the study, because one of them was pregnant and seven of them did not continue to the treatment. Patients were randomized into two treatment groups. There were 31 patients in the first dry needling group, 30 patients in the second laser group. In the dry needling group, 22 gauge (1,5 inch) black injector tips were used approximately ten minutes for each trigger point. This method was performed twice a week for two weeks in four seances, totally. In the laser group, 808 nm (continious) – 905 nm (discontinious) wave-lenght, 18 V-50 VA power output ‘Mphi, Lasertherapy MLS' trade, galium-arsenide (GaAs) low density laser device was used. In 900 Hz frequency, 4.64 J/cm² density, total dose 174.720 J laser therapy were applied to 12 points by transforaminal way approximately five minutes for each trigger point, five days in a week for two weeks in ten seances, totally. Home exercise programme, including streching exercises, were given to all patients. In the evaluation for the severity of pain; visual analog scale (VAS) and pressure pain threshold (PPT), for the functional state; neck pain and disability scale (NPADS), for the patient mood; Beck depression scale (BDS) and Beck anxiety scale (BAS) were used.Results: In both groups, it was found statistically significant improvement in all parameters after treatment. In the comparison of the two groups, VAS, PPT and NPADS values statistically significant difference was determined in favor of dry needling group (p<0,05).Conclusion: Although dry needling treatment was more effective than laser treatment, both of the treatment options have been found useful in MPS. However, in the case of patients with fear of needle and in the presence of inexperienced health workers for dry needling technique, low dose laser therapy in the treatment seems to be a good alternative of dry needling treatment.
