SDÜ Tıp Fakültesi Acil Servisi'ne Başvuran ve Akut Apandisit Tanısıyla Opere Edilen Hastaların Biyobelirteçleri ve Görüntüleme Yöntemlerinin Geriye Dönük Değerlendirilmesi Apandisit dünyada akut karının en yaygın nedenlerinden biri olarak kabul edilmiştir. Tanıda anamnez, fizik muayene, görüntüleme ve laboratuvar yöntemlerinden yararlanılmakla birlikte akut apandisit tanısında sorunlar devam etmektedir. Çalışmamızda akut apandisit tanısı koyarken laboratuvar bulguları ile görüntüleme yöntemlerinin duyarlılıkları karşılaştırıldı. Çalışmaya 01.06.2012 ile 01.06.2015 tarihleri arasında SDÜ Tıp Fakültesi Acil Servisine başvuran ve akut apandisit tanısı ile opere edilen 18 yaş üstü 292 hasta dahil edildi. Hastaların bilgilerine acil servis dosyalarından, ameliyat notlarından, epikrizlerden ve bilgi işlem sistemi veri tabanından ulaşıldı. Hastaların yaş, cinsiyet, WBC sayısı, nötrofil sayısı, NLO değeri, MPV sayısı, CRP düzeyleri, USG ve BT bulguları ve histopatolojik bulguları veri toplama aracı olarak bir takip formu oluşturulup bu formlara kaydedildi. Laboratuar ölçümlerinin ve görüntüleme yöntemlerinin patoloji pozitifliği ayırt ediciliğine yönelik ROC eğrileri oluşturularak, her bir ölçüme ait AUC ve kesim değeri belirlendi ve bu değerlerin patoloji ayırt edicilik performansları, sensitivite, spesifisite, +PV, -PV, +LR ve -LR kesim değerleri hesaplanarak birlikte değerlendirildi. Başvuran hastaların cinsiyet dağılımı 174 erkek, 118 kadın idi. Erkek/kadın oranı 1,6:1 olarak bulundu. Akut apandisit görülme sıklığı cinsiyete göre karşılaştırıldığında, istatistiksel olarak anlamlı fark tespit edildi. Hastaların ortalama yaşı 32,15±13,67 olarak bulundu. WBC değeri için AUC değeri 0,684 olarak hesaplandı. Akut apandisitin tanısında WBC'nin sensitivitesi %65,09, spesifitesi %62,1, pozitif tahmin değeri %92,2, negatif tahmin değeri %20,5, pozitif olabilirlik oranı (+LR) 1,71, negatif olabilirlik oranı (-LR) 0,56 ve tanısal doğruluğu %64,7 olarak saptandı. Nötrofil değeri için AUC değeri 0,698 olarak hesaplandı. Akut apandisitin tanısında nötrofilin sensitivitesi %72,15, spesifitesi %59,5, pozitif tahmin değeri %92,5, negatif tahmin değeri %23,6, pozitif olabilirlik oranı 1,78, negatif olabilirlik oranı 0,46 ve tanısal doğruluğu %70,54 olarak saptandı. Nötrofil lenfosit oranı için AUC değeri 0,647 olarak hesaplandı. Akut apandisit tanısında NLO'nun sensitivitesi %70,9, spesifitesi %51,3, pozitif tahmin değeri %90,9, negatif tahmin değeri %20,4, pozitif olabilirlik oranı 1,45, negatif olabilirlik oranı 0,56 ve tanısal doğruluğu %68,4 olarak saptandı. CRP değeri için AUC değeri 0,530 olarak hesaplandı. Akut apandisitin tanısında CRP'nin sensitivitesi %45,3, spesifitesi %48, pozitif tahmin değeri %83,9, negatif tahmin değeri %12,7, pozitif olabilirlik oranı 0,87, negatif olabilirlik oranı 1,13 ve tanısal doğruluğu %45,7 olarak bulundu. USG'nin akut apandisit için tanı değeri yüksek bulundu (AUC=0,629). Çalışmamızda ultrasonografinin sensitivitesi %82,9, spesifitesi %42,8, pozitif tahmin değeri %91,6, negatif tahmin değeri %25, pozitif olabilirlik oranı 1,44, negatif olabilirlik oranı 0,39 ve tanı doğruluğu %78,2 olarak tespit edildi. BT'nin akut apandisit için tanı değeri yüksek olduğu görüldü (AUC=0,865). Çalışmamızda abdominal BT'nin sensitivitesi %93,9, spesifitesi %78,9, pozitif tahmin değeri %96,4, negatif tahmin değeri %68,1, pozitif olabilirlik oranı 4,45, negatif olabilirlik oranı 0,07 ve tanı doğruluğu %91,8 olarak bulundu. Bu çalışmadan elde ettiğimiz verilere göre tanı koyarken laboratuar değerleri ve biyobelirteçler dikkate alınmalı, klinik şüphenin devamı halinde görüntüleme yöntemlerinden faydalanılmalıdır. Ancak akut apandisit tanısında hiçbir ek tanısal yöntemin tek başına tanı koymada yeterli olmadığını ve kullanılan bütün yöntemlerin klinisyene tanıda destek olması gerektiğini düşünmekteyiz.

ROC curves were intended for positive distinguish of laboratory measurements and imaging methods. Determination of AUC and cutoff values for every measurement and distinguishing performances of these values were assessed with sensitivity, specifity, +PV, -PV, +LR and –LR. The gender distribution of patients were 174 male and 118 female. Male to female ratio was found 1,6: 1. Compared the incidence of acute appendicitis based on gender, statistically significant differences were detected. The average age of the patients was calculated as 32.15 ± 13.67. AUC was calculated to be 0.684 for the WBC count. We believe that all the method should be used in tests required to support clinicians in diagnosis. However, no additional diagnostic methods in the diagnosis of acute appendicitis is not enough by itself.
