Giriş: Verrukalar, Human Papillomavirüslerin yol açtığı deri ve mukozaların benign proliferasyonlarıdır. Hastaların yaklaşık yarısında bir yıl içinde, 2/3'ünde 2 yıl içinde spontan gerilerken, hastaların kalan üçte birinde ise dirençli hale gelmektedirler. Verrukaların neden bazı kişilerde spontan iyileşip bazı kişilerde dirençli seyrettiği ve diğer yönlerden sağlıklı bireylerde, immün sisteminin verruka oluşumunu baskılamada neden yetersiz kaldığı tam olarak anlaşılamamıştır. Çalışmamızda tedavilere yanıtsız, dirençli verrukası olan ve diğer yönlerden sağlıklı bireylerde immün sistemin değerlendirilmesi amaçlandı. Yöntem ve Gereç: Bu araştırmaya Süleyman Demirel Üniversitesi Tıp Fakültesi Dermatoloji Kliniği'ne başvuran, en az iki tedavi yöntemine yanıt vermeyen ve iki yıldan uzun süren verrukası olan, ek bir hastalık ve ilaç kullanımı olmayan 34 dirençli verruka hastası ve 34 sağlıklı kontrol alındı. Hastalardan ve sağlıklı kontrol grubundan alınan periferik venöz kan örneklerinde flowsitometri yöntemi ile CD4+ T hücre, CD8+ T hücre, T regülatör hücre ve CD4/CD8 T hücre oranları, IFN-, TNF-α, IL-2, IL-4, IL-5, IL-10 sitokin düzeyleri ölçüldü. Bulgular Dirençli verruka hastalarının periferik kan örneklerinde CD4+ T hücre, CD8+ T hücre ve CD4/CD8 T hücre oranları kontrol grubuna göre hafif düşük bulunmakla beraber anlamlı düzeyde azalma saptanmadı (sırasıyla p:0,564, p:0,975, p:0.840). T regülatör hücre oranları ise kontrol grubuna göre anlamlı olarak yüksek bulundu (p:0,026). IFN-, TNF-α, IL-2, IL-4, IL-5, IL-10 düzeyleri kontrol grubuna göre anlamlı olarak yüksek bulundu (sırasıyla p:0.039, p:0.015,p:0,010, p:0,004, p:0,014, p:0,005, p:0.005). Sonuç: Çalışmamızda dirençli verrukası olan hastalarda CD4+ ve CD8+ T hücre oranları kontrol grubuna göre hafif düzeyde düşük bulunurken, T regülatör hücre oranlarının yüksek bulunması ve T regülatör hücre oranı arttıkça CD4+ T hücre ve CD8+ T hücre oranlarında azalma saptanması verrukalara karşı direnç gelişiminde T regülatör hücrelerin önemli rollerinin olduğunu düşündürmüştür. Bunun yanında dirençli verrukası olan hastalarda TNF-α, IFN-, IL-2, IL-4, IL-5 ve IL-10 sitokin düzeylerinin yüksek bulunması, dirençli HPV infeksiyonlarında immün sistemin uyarıldığını ancak yeterli immün yanıtın oluşamadığını düşündürmüştür. Verrukalara karşı direnç gelişiminde T hücre oranlarında ve sitokin düzeylerindeki değişiklikler yanında bu hücre ve sitokinlerin birbirleriyle etkileşimlerinin önemli olabileceği sonucuna varılmıştır.

They spontaneously regress in 1 year in half of the patients and in 2 years in 2/3 of the patients, while they become resistant in the remaining one third of the patients. The reason why verrucas spontaneously recover in some patients while becoming resistant in others and the reason why the immune system of otherwise healthy patients cannot suffice in suppressing verruca formation have not yet been completely found out. The present study aims to evaluate the immune systems of patients with resistant verrucas who are otherwise healthy. Method and Instruments: A control group of 34 healthy patients and a group of 34 patients with resistant verrucas who applied to Süleyman Demirel University Medical Faculty Dermatology Clinic, did not respond to at least two treatment methods, had verrucas lasting for more than two years and do not have an additional disease or medication utilization have been included in the study. CD4+ T cell, CD8+ T cell, T regulatory cell and CD4/CD8 T cell rates and IFN-, TNF-α, IL-2, IL-4, IL-5, IL-10 cytokine levels were detected via flow cytometry method in peripheral venous blood samples from the patients and control group. Findings: In the peripheral blood samples from patients with resistant verrucas CD4+ T, CD8+ T and CD4/CD8 T cell rates have been found to be slightly less when compared to the control group. However, a significant scarcity has not been detected. (p:0,564, p:0,975, p:0.840 respectively). The T regulatory cell rates in patients have been found to be significantly higher when compared to the control group (p:0.026). IFN-, TNF-α, IL-2, IL-4, IL-5, IL-10 levels have been found to be significantly higher in patients when compared to the control group (p:0.039, p:0.015,p:0,010, p:0,004, p:0,014, p:0,005, p:0.005 respectively). Conclusion: While CD4+ and CD8+ T cell rates have been found to be slightly less in patients with resistant verrucas when compared to the control group, the observed highness in T regulatory cell rates and the observed decrease in the CD4+ T and CD8+ T cell rates as T regulatory cell rate increases suggests that T regulatory cells may have important roles in forming resistance against verrucas. In addition, in patients with resistant verrucas, the observed highness of TNF-α, IFN-, IL-2, IL-4, IL-5 and IL-10 cytokine levels suggests that the immune system in HPV infections is stimulated, but a sufficient immunity response is not developed. It has been concluded that in addition to the variations in the T cell rates and cytokine levels, the interactions among these cells and cytokines may be important in developing resistance against verrucas.
