SDÜ Araştırma Uygulama Hastanesi Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji Polikliniği'nde Kronik Hepatit B Tanısıyla Tenofovir veya Entekavir Tedavisi Alan Hastaların Değerlendirilmesi Kronik hepatit B tedavisinde güncel amaç, etkili HBV DNA supresyonu sağlanarak HBV DNA'nın ölçülemez düzeyde tutulması ve mümkünse HBsAg kaybı ve serokonversiyonunun sağlanmasıdır. Böylece KHB'ye bağlı siroz ve HSK gibi komplikasyonların gelişme riski azaltılmaya çalışılmaktadır. Günümüzde KHB tedavisinde, interferonlar ve nükleoz(t)id analogları kullanılmakta olup, nükleoz(t)id analogları içerisinde, entekavir ve tenofovir, güçlü viral süpresif etkileri ve yüksek genetik bariyerleri sayesinde düşük direnç oranlarına sahip olmaları nedeniyle tedavide ilk seçenek ilaçlar haline gelmişlerdir. Bu çalışmada KHB tedavisinde ilk seçenek haline gelen bu ilaçların etkinlik ve güvenilirlik açısından karşılaştırılmaları amaçlanmıştır. Bu çalışmada polikliniğimizde kronik hepatit B tanısı ile izlenen, en az 1 yıldır tenofovir veya entekavir tedavisi alan hastalarda, bu tedavilerin virolojik, serolojik ve biyokimyasal olarak etkinliği ve tedavi yan etkileri retrospektif olarak değerlendirilmiştir. Tenofovir ve entekavir grupları, tedavi esnasında yapılan tetkikler ile değerlendirilerek ALT normalleşme zamanı ve oranı, HBV DNA saptanamayacak düzeye inme - negatifleşme zamanı ve oranı, HBeAg pozitifliği mevcut olan hastalarda serokonversiyon gelişme oranı, HBsAg kaybı, HSK gelişimi, klinik ve laboratuvar yan etki gelişimi açısından karşılaştırılmıştır. Tedavi öncesi yaş, cinsiyet, ALT ve HBV-DNA düzeyleri, histolojik aktivite indeksi ve fibrozis skoru açısından aralarında istatiksel olarak anlamlı fark olmayan 90 hastanın 45'i tenofovir, 45'i entekavir tedavisi almaktaydı. ALT normallleşme zamanı tenofovir grubunda ortalama 7,29 ± 3,44 ay, entekavir grubunda 12,10 ±13,52 ay olarak saptandı. Tedavinin 12, 24, 36, 48 ve 60. aylarında ALT normalleşme oranları tenofovir grubunda sırasıyla %80, %81,3, %69,2, 85,2, %0; entekavir grubunda sırasıyla %80, %74,1, %83,3, %75, %83 olarak saptandı. HBV-DNA'nın saptanabilir düzeyin altına inme süresi tenofovir grubunda ortalama 11,44 ± 6,52 ay, entekavir grubunda 14,00 ± 12,94 ay idi. Tedavinin 12, 24, 36, 48 ve 60. aylarında HBV-DNA' nın saptanabilir düzeyin altına inme oranı tenofovir grubunda sırasıyla %75,6, %81,8, %95,7, %100, %100; entekavir grubunda sırasıyla %60, %70, %84,4, %87, %90 olarak saptandı. HBeAg serokonversiyonu oranı, tenofovir grubunda %50, entekavir grubunda %42,9 saptandı. İki grupta da HBsAg kaybı gelişen hasta saptanmadı. Bu veriler değerlendirildiğinde, tenofovir ve entekavir grupları arasında, ALT normalleşme zamanı ve oranı, HBV DNA saptanamayacak düzeye inme/negatifleşme zamanı ve oranı, HBeAg serokonversiyonu oranı açısından istatiksel olarak anlamlı fark saptanmamıştır. İki grup arasında klinik yan etki gelişimi açısından anlamlı fark saptanmaz iken, laboratuvar yan etki görüme sıklığı tenofovir grubunda anlamlı olarak fazla saptandı. Hastalar naiv ve tedavi deneyimli olarak iki ayrı subgruba ayrıldığında da, tenofovir ve entekavir tedavileri arasında etkinlik açısından istatiksel olarak anlamlı bir fark saptanmadı. Bu çalışmadaki veriler ışığında sonuç olarak; KHB tedavisinde tenofovir ve entekavir tedavileri gerek etkinlik, gerekse güvenilirlik açısından eşdeğer görünmektedir. Yine de, daha geniş katılımlı ve uzun süreli çalışmalarla bu veriler desteklenmelidir.

Evaluation of Chronic Hepatitis B Patients at SDU Research Hospital Clinic of Infectious Diseases and Clinical Microbiology Treated With Tenofovir or Entecavir Therapy The current goal in the treatment of chronic hepatitis B is to keep the HBV DNA at undetectable level by providing the DNA suppression; and if possible, to ensure the HbsAg loss and seroconversion. By this way, it is aimed to decrease the risk of the occurance of the complications associated to CHB such as cirhosis and HCC. Nowadays, the interferons and nucleos(t)ide analogs have been used for the treatment of CHB. Amongst nucleos(t)ide analogs, entecavir and tenofovir have become the first-line drugs in CHB treatment because of their low resistance rates resulting from their strong viral suppressive effects and high genetic barriers. In this study, it is aimed to compare these drugs which have become the first-line therapy in CHB treatment in terms of their efficacy and safety. In this study, virologic, serologic and biochemical efficacy and side effects of these treatments of the patients who are diagnosed with chronic hepatitis B and at least 1 year tenofovir or entecavir-treated in our clinic, were evaluated retrospectively. Tenofovir and entecavir groups are assessed by the tests performed during treatment and compared according to ALT normalization time and rates, time to undetectable serum HBV DNA levels, rates of undetecteble serum HBV DNA levels, HBeAg seroconversion and HBsAg clearence, HCC development, clinical and laboratory adverse effects development. Amongst 90 patients who had no statistically significant difference in age, gender, ALT and HBV DNA levels and histological activity index and fibrosis before the treatment; 45 patients were receiving tenofovir and 45 patients were receiving entecavir. The mean time to ALT normalization in the tenofovir group was 7,29 ± 3,44 months and in the entecavir group it was 12,10 ± 13,52 months. Rates of ALT normalization at 12, 24, 48 and 60 months were obtained as 80%, 81,3%, 69,2%, 85,2%, 0% in tenofovir group and 80%, 74,1%, 83,3%, 75%, 83% months in entecavir group. The mean time to achieve undetectable serum HBV DNA levels was 11,44 ± 6,52 months in tenofovir and 14,00 ± 12,94 months in entecavir groups. The rates of undetectable HBV DNA levels at 12, 24, 36, 48 and 60 months were 75,6%, 81,8%, 95,7%, 100%, 100% in tenofovir group and 60%, 70%, 84,4%, 87%, 90% in entecavir group. Rates of HBeAg seroconversion was %50 and %42,9 in tenofovir and entecavir groups. None of the patients in any group had HBsAg clearence. The mean time to ALT normalization, rates of ALT normalization, the mean time to achieve undetectable serum HBV DNA levels, the rates of undetectable HBV DNA levels in the two groups did not differ significantly. While there was no significant difference in clinical side effects between the two groups the frequency of the laboratory side effects were found to be significantly higher in the tenofovir group. When naive and treatment experienced patients were divided into two subgroups, there was no statistically significant difference between tenofovir and entecavir treatments by the efficacy of treatment. In conclusion, both of tenofovir and entecavir are seem to be equivalent in terms of efficacy and safety in CHB treatment. Nevertheless, these data should be supported by long-term studies with broader participation.
