Tanım: Femur başının osteonekrozu (FBON), kemik mimarisinde bozulmaya ve deformiteye neden olması nedeniyle ciddi bir sorundur. Travmaya bağlı nedenlerle vasküler yapılardaki hasarın bir sonucu olarak ortaya çıkabildiği gibi herhangi bir travma olmadan da femur başında osteonekroz gelişebilmektedir. Erken evrelerde deformitenin gelişimi yavaşlatılabilse de patoloji ilerleyicidir. Bu çalışmada vasküler hasarlandırma yöntemiyle ratlarda femur başı osteonekroz modeli oluşturulup kor dekompresyon ve Epidermal Growth Faktörün tedavideki etkinlikleri araştırılmıştır. Materyal ve Metot: Ratların sağ femur başları disseke edildikten sonra servikal periost sıyırıldı. Femur boynuna yerleştirilen sıkı bir ip düğümüyle femur başı osteonekrozu oluşturuldu. Osteonekroz indüksiyonunu takip eden 2. Haftanın sonunda tüm gruplar opere edildi. İkinci haftada hastalıklı kontrol grubunda (Grup I) femur boynundaki düğüm çözüldü. Kor dekompresyon grubunda (Grup II) femur başını perfore etmeyen drilizasyon tedavisi uygulandı. EGF+ Kor dekompresyon grubunda (Grup III) drilizasyona ek olarak intraosseöz EGF enjeksiyonu yapıldı. Sekizinci haftanın sonunda tüm ratlar sakrifiye edildi. Radyolojik, histomorfolojik ve immünhistokimyasal analizler uygulandı. İstatisiksel olarak veriler analiz edildi. Bulgular: Sekizinci haftanın sonunda EGF uygulanan grupta femur başı alanının ve küreselliğinin diğer gruplara göre korunduğu görüldü. EGF grubunda histolojik olarak subkondral mesafenin korunduğu anlaşıldı. EGF grubunda femur başı alanı, çapı, trabekül kalınlığı opere olmayan tarafla karşılaştırıldığında istatistiksel fark görülmedi. İmmünhistokimyasal incelemelerde anti-CD31 boyamaları her 3 grupta da anlamlı farklılık gösterdi. Buna göre kor dekompresyon işlemi EFG ye göre daha az damarlanma artışı sağlamaktadır. Osteopontin seviyeleri II ve III. Grupta I. Gruba göre anlamlı artış gösterdi fakat tedavi grupları arasında istatistiksel fark oluşmadı. Tartışma ve Sonuç: EGF'nin intraosseöz ve perikapsüler uygulanması FBON modelinde trabeküler kemik mimarisini korumuş, osteonekrotik femur başında yeni kemik oluşumu ve mikro damarlanma sağlayarak etki göstermiştir. Bu bulgular, EFG+kor dekompresyon uygulamasının FBON tedavisinde kullanılabileceğini desteklemektedir.

Introduction: Osteonecrosis of the femoral head (ONFH) can lead to loss of femoral head architecture and deformity which cause serious complications. ONFH can develop according to the trauma induced vascular damage or as a result of non- traumatic disorders. In early stages, it is able to slow down the development of deformity, however pathological process is progressive. In this study, the efficiency of the core decompression and Epidermal Growth Factor (EGF) was investigated in the treatment of surgically induced femoral head osteonecrosis in a rat model. Material and Method: Right femoral head of the rats was dissected and cervical periosteum stripping was applied. A ligature was placed tightly around the femoral neck and avascular necrosis was induced. Rats were divided into the following groups; 1) the sham-operated group (the sham group), 2) core decompression group, and 3) core decompression and intraosseous EGF-injected group (the EGF group) (n=10/group). All groups were operated at the end of two weeks following the induction of ONFH. Femoral neck ligatures were removed in all groups. After removing ligatures, core decompression was applied in the second and third group, and EGF was injected intraosseously in the third group. At 8 weeks post-surgery animals were sacrificed and radiologic and histomorphometric assessments were performed. Results: Radiographs obtained at 8 weeks post-surgery showed better preservation of femoral head area and sphericity in EGF group than in the core decompression group. Subchondral thickness was preserved histologically in EGF group. In EGF group, femoral head area, diameter, and trabecular thickness showed no difference significantly when compared to the non-operated side. Immunohistochemical study of anti-CD31 staining showed significant differences in all 3 groups. According to the CD 31 staining, EGF group animals showed higher levels of vascularity in the secondary ossification center of infarcted right femoral heads. Osteopontin levels showed a significant increase in group II and III compared to the sham-operated group. Conclusion: The intraosseous and pericapsular injection of EGF preserved trabecular bone architecture and femoral head deformity by promoting micro vascularity and bone remodeling in an ONFH model that surgically induced in rats. These findings suggest that EFG + core decompression modality could be used to treat ONFH.
